2.Congenital Cavernous Lymphangioma in Wrist

Lymphangioma is composed of lymphatic tissue and filled with clear or chylous fluid that develops most often in the skin and subcutaneous tissue of neck but relatively rare in extremities. It is perhaps a congnital malformation rather than a true neoplasm, and so most of these lesions are usually present at birth and have a tendency to grow very slowly. If it is developed with a cavernous type of lymphangioma, it will have more aggressive and infiltrative tendency because their margin is usually adhesive with adjacent normal tissue. A case of cavernous lymphangioma on the right wrist of an eight month old female infant, treated by surgical excision is followed up over one year and presented here with a review of the literature.

3.Transvaginal Sonographic Evaluation of Uterine Wall Thickness on Prior Cesarean Scar.

OBJECTIVES: Estimation of the anterior lower uterine segment (LUS) thickness difference who underwent prior cesarean delivery measured with transvaginal sonography at or after 36 gestational weeks (sonographic thickness) and a ruler during elective cesarean section (operation thickness) Methods: One hundred sixty women who underwent prior cesarean delivery had the thickness of their LUS measured with transvaginal sonography at or after 36 gestational weeks. The LUS thickness was measured with a ruler during elective cesarean section. We compared group I whose LUS was fairly well visualized 4 cm or more from the uterine cervix to group II which had less than 4 cm. RESULTS: The mean sonographic thickness of LUS was 1.7 0.8 mm and that of operation thickness was 1.9 0.5 mm. The mean difference of the two (thickness difference) was 0.5 0.5 mm. In 31.3% the sonographic thickness was same as the operation thickness and in 70.7% of the total with 0.5 mm or less difference, the sonographic thickness could be regarded as accurate. The thickness difference with the sonographic thickness with 2 mm or more was smaller than those with 1 mm or less (0.4 0.5; 0.6 0.6) and that of group I was smaller than that of group II (0.4 0.4; 0.9 0.6) (p< .05). CONCLUSION: The thickness difference was 0.5 0.5 mm and it was smaller when the LUS thickness is 2 mm or over, clearly visible 4 cm or over from the cervix.

5.Effects of Cortex morl on the activation of rat peritoneal mast cells by human seminal plasma.

BACKGROUND: It has been reported that Cortex mori ( Morus alba L, Sangbaikpi ; CM ), the root bark of mulberry tree, plays a role in inhibiting mast cell activation. Human seminal plasma ( HSP ) modulates immune systems and activates rat peritoneal mast cells ( RPMC ). However, the inhibitory agents of HSP - induced mast cell activation have not been disclosed yet. OBJECTIVE AND METHOD: This study was undertaken to determine the effects of CM on HSP-induced mast cell activation by morphologic and functional methods. Morphological changes of RPMC by HSP alone or CM plus HSP were observed with the inverted light microscope. And the amounts of histamine and calcium were measured by radioisotopic enzymatic histamine and radioisotopic calcium assays. RESULTS: By inverted microscopy, HSP-induced RPMC degranulation occurred in a dose-dependent fashion. After pretreatment of RPMC with CM, there was no HSP-induced degranulation. Degranulation index (DI) of RPMC treated with Hanks balanced salt solution (HBSS), CM or HSP was 7.0+/-4.3, 9.0+/-6.2, 56.5+/-16.9, respectively. After pretreatment of RPMC with CM, HSP-induced DI was 11.0 +/-7.1. These results indicate that CM inhibited HSP-induced degranulation of RPMC. HSP-induced RPMC histamine release (HR) was dose-dependent. HR of RPMC treated with HBSS, CM or HSP was 0.18+/-0.10, 0.30+/-0.19, 4.48+/-0.86 ug/ml, respectively. After pretreatment of RPMC with CM, HSP-induced HR was 0.52+/-0.21 ug/ml. These results indicate that CM inhibited HSP-induced HR from RPMC. Intracellular calcium level ( ICL) of RPMC was also increased according to the concentration of HSP. ICL of RPMC treated with HBSS, CM or HSP was 6.1+/-1.0, 9.0+/-2.1, 30.2+/-6.5 pmole, respectively. After pretreatment of RPMC with CM, HSP-induced ICL of RPMC was 11.6 +/- 3.3 pmole. These results indicate that CM inhibited HSP-induced calcium uptake of RPMC. 9.Effect of Endoscopic Ethanol Injection in Upper G-I Bleeding.

Upper G-I bleeding is a dangerous situation and effeetive control method without surgery is desirable. A hemostatic technique utilizing the dehydrating and fixative properties of pure ethanol was developed in Japan. This technique was performed through an endoscopic injector, in divided dose of 0,1 ~ 0.2 ml into several spots in the area surrounding the bleeding vessel in patients upper G-I bleeding excluding esophageal varies. We applieii this technique in treatment of 11 cases showing active bleeding of fresh blood clots. Rebleeding occurred in 2 cases(20%) and complete hemostasis was achieved in 9 cases(80%). We think this technique is safe and simple as an endoscopic hemostatic measure.
