1.Lectin Binding Pattern of the Duodenal Mucosa in the Developing Rat.

For demonstrating the changes of lectin biding pattern in the developing rat, nine biotinylated lectin were applied with ABC method. The affinities for SBA, PNA, RCA-1, sWGA, and Con A were observed on 15 day fetal rat, and an additional UEA-1 affinty on 17 day fetal rat was detected. From the next developing rats, lectin binding pattern was signicantly altered and these changes were greater in suckling rats than others. The notable changes of the duodenal mucosa in developing rats were as follows : A new affinities for SBA, sWGA and UEA-1 in the villous columnar cells and PNA, RCA-1, sWGA and Con A in the crypt columnar cells were detected from suckling rats. The SBA, PNA, BSL-1, RCA-1, sWGA and UEA-1 affinities in the Golgi zone of villous columnar cells showed a tendency to increase from the suckling rats, and a new affinities for SBA, PNA, BSL-1, RCA-1 and sWGA in that of crypt columnar cells revealed from same rats. All lectin affinities used except DBA and Con A were observed in the brush border and maintained a higher level until adults, however, no BSL-1 affinity was detected from the weaning rats. The UEA-1 affinity of goblet cells was detected from 19 day fetal rat and not observed from the weaning rats. And the affinity for DBA and SBA of the goblet cells was demonstrated from the suckling and weaning rats respectively. All lectin affinities examined were observed in the duodenal gland and increased from the suckling rats.

2.Changes of Interleukin-6, C-reactive Protein, and Lipid Peroxide Levels in the Umbilical Venous Plasma of Preterm Birth with or without Chorioamnionitis.

OBJECTIVE:To investigate interleukin-6 (IL-6), C-reactive protein (CRP), and lipid peroxide levels in the umbilical venous plasma of preterm birth with or without histologic chorioamnionitis and to evaluate their roles in the pathophysiology in preterm labor and perinatal outcome. METHODS:This cohort study included 66 cases of preterm delivery with preterm labor and intact membranes (PTL) (n=39) and preterm premature rupture of membranes (PPROM) (n=27). The umbilical venous blood samples were collected at the time of delivery. IL-6, CRP, and lipid peroxide levels were measured by ELISA Kit, latex agglutination assay, and thiobarbituric acid reaction. Histologic chorioamnionitis was diagnosed by the presence of neutrophil infiltration into the subamnionic space. RESULTS:The prevalence of histologic chorioamnionitis was significantly higher in PPROM (59.3 %, 16/27) than in PTL (20.5%, 8/39). IL-6, CRP, and lipid peroxide levels in the umbilical venous plasma of histologic chorioamnionitis were significantly higher than those without histologic chorioamnionitis. IL-6, CRP, and lipid peroxide levels in the umbilical venous plasma of PTL with histologic chorioamnionitis were significantly higher than those of PTL without histologic chorioamnionitis. CRP levels in the umbilical venous plasma of PPROM with histologic chorioamnionitis were significantly higher than those of PPROM without histologic chorioamnionitis. Three suspected neonatal sepsis patients have increased IL-6 and lipid peroxide levels in the umbilical venous plasma compared with patients without neonatal sepsis. IL-6 levels in the umbilical venous plasma of histologic funisitis were significantly higher than those without funisitis. CONCLUSION:Preterm birth with chorioamnionitis is associated with an increased level of IL-6, CRP, and lipid peroxide in umbilical venous blood. Preterm birth with chorioamnionitis may have an effect on perinatal outcome.

While endovascular aneurysm repair (EVAR) is prevailing for the treatment of abdominal aortic aneurysm (AAA) in modern vascular practice, PURPOSE: we conducted nationwide questionnaire survey to investigate the current status of AAA treatment and their results in Korea. METHOD: We reviewed the replies from 28 hospitals (33 departments) to the questionnaire inquiring annual number, clinical features, mode of treatment and results of AAA patients during the period from Jan. 2000 to Jul. 2004. Results: 980 AAA patients were reported including 292 ruptured AAA (29.8%) and 688 non-ruptured AAA (70.2%). For treatment of AAA, 834 (85.1%) surgical repairs (SRs) and 111 (11.3%) endovascualr aneurysm repairs (EVARs) were performed while 35 patients (3.6%) died of AAA rupture before operation. The locations of AAA were infrarenal in 889 (90.7%), juxtarenal in 62 (6.3%), and suprarenal in 29 patients (3.0%). Among 834 patients undergoing SR, 577 patients (69.2%) had non-ruptured AAAs and 257 patients (30.8%) had ruptured AAAs. Mean operative mortality rate was 4.1% after elective SRs, 30.7% after SR for ruptured AAAs, and 2.3% after EVARs. The reported brand name of stent graft devices were various including domestic custom-made in 56 (50.5%), imported brand in 18 (16.2%) while 37 (33.3%) stent grafts were not reported their brand name. The frequencies of type I and III endoleaks after EVAR were reported 5.8% and 5.8% respectively in 86 patients with an available data. CONCLUSION: SR has been used as a major treatment option in Korea for the treatment of AAA patients while EVAR is increasing. The mortality rate of SR of AAA was comparable to western multi-center trial reports but mortality or morbidity rates of EVAR were unable to know in this questionnaire survey.
