1.Patterns of Recurrence after Curative Resection of Hepatocellular Carcinoma: Radiological Type.

PURPOSE: To obtain the useful information about tumor behavior of hepatocellular carcinoma by analyzing the recurrence pattern after curative resection. MATERIALS AND METHODS: Forty-two patients who underwent curative resection of hepatocellular carcinoma were analyzed. Based on the firstly detected radiologic recurrence, we classified recurrence patterns into three types, type I;marginal recurrence, type II;intrahepatic nodular(multiple, single) and type III;extrahepatic. We also analyzed its duration of recurrence after curative resection. RESULTS: Eighteen patients showed no radiologic recurrence during follow-up period(mean 19.2 months). Twenty-four patients showed radiologic recurrence at 29 sites and its mean duration was 11.0 months. Recurrence patterns were seven cases in type I, 14 cases in type II(multiple 11, single three), eight cases in type III including two omental, two extrahepatic lymph node, two brain, one spine, and one intracranial metastasis. Among the extrahepatic recurrences, five cases showed only distant metastasis without hepatic recurrence. Mean duration of recurrence according to the site was 16.0 months in type I, 12.6 months in type II, 12.3 months in type III. Among 20 patients with single site recurrence, type 11(9.0 months) was earlier than type 1(10.3 months). CONCLUSION: lntrahepatic recurrence is more common than extrahepatic recurrence. Intrahepatic nodular, especially multinodular recurrence, is more common than marginal recurrence. Intrahepatic recurrence has tendency to develope earlier metastain earlier than marginal recurrence.

2.Prevalence of Diabetes Mellitus and Glycosuria in School Children.

An epidemiological survey on the glycosuria was conducted on the senier students of middle and high school in Seoul, from March 1981 to March 1982. Selected examinee totalled to 254,835 of which 138,116 were male & 116,719 were female. The initial, second and third urine sugar test were done with Combur-test strip(r). Glucose tolerance test was performed on those who were found to have persistent glycosuria. The followings showed the results of the survey. Among the 254,835 examinees, 1,240 were found with initial positive glycosuria. The glycosuria positive rate revealed 0.52% for male, 0.44% for female. Its average was 0.49%. It also revealed that male students in high school had higher positive rate of glycosuria. 2) The persistent glycosuria positive rate revealed 11.58/100,000 for male, 15.42/100,000 for female and its average was 13.34/100,000. 3) Out of the 33 cases with the persistent glycosuria, 60.6% of them were diagnosed as diabetes mellitus. 4) The overall prevalence rate of diabetes was 7.85/100,000. The prevalence rate of male was5.79/100,000, and female was 10.28/100,000. Consequently the rate of male to female was 1:1.78, showing that rates were higher for females than males. The. survey also revealed that the prevalence rate of diabetes was increased with advancing grade. 5) The 70% of the total person who were diagnosed as diabetes, did not recognize of their disease by themselves till the survey was conducted. 6) The frequency of subjective diabetic symptoms were polydipsia, polyuria and polyphagia in order. *Combur-test(r)are registered trade marks of the Boehringer mannheim GmbH Company.

3.Invasiveness and Proliferative Activity of Oral Squamous Cell Carcinoma : Immunohistochemical Study Using Laminin, Type IV Collagen, and Ki-67 Antibody.

Since the invasiveness and metastatic potential of the carcinoma and the proliferation of the tumor cells are the decisional factors in determining the prognosis of carcinoma, it has been a principal area of study to foresee these factors in early stage of the disease process. The purpose of the study was to propose a method to precisely determine the prognosis of tumor. The investigation was performed, using laminin, type IV collagen, and Ki-67 antibody, to reveal the invasiveness and proliferation of the squamous cell carcinoma which is the most prevalent cancer in the area of oral and maxillofacial complex. Frozen tissue sections, 6-8mum in thickness, were prepared from the surgical excision of fresh squamous cell carcinoma for the experimental group. The control group was composed of sections from the normal oral mucosa of the contralateral side. Following incubation in an appropriate primary antibody including laminin, type IV collagen and Ki-67, LSAB method was employed and the tissue sections were treated with DAB for light microscopic immurlohistochemistry. Semiquantitative analysis of the basement membrane was composed of five different positivity of laminin and type IV collagen according to the intensity of the staining. Mean number of Ki-67 positive cells per 2000 total cells in one slide were calculated along with the standard deviation. The obtained results were as follows : 1. In the basement membrane of normal oral mucosa, antibodies against laminin and type IV collagen showed regular thickened, continuous line. Positive reaction was also detected in the vascular walls. Ki-67 labelling index was 4.10+/- 0.80%, and the positive cells were distributed mainly in the basal and parabasal cell layers. 2. In basement membrane of the proliferating tumor adjacent to carcinoma, antibodies against laminin and type IV collagen showed irregular thin, discontinuous line. Ki-67 labelling index was 18.95+/-4.67%, and the parabasal cell layer exhibited increased number of positive cells. 3. In the basement membrane of the main proliferating tumor, antibodies against laminin and type IV collagen showed almost indistinct interrupted line and/or almost indistinct line. Ki-67 labelling index was 31.56+/-5.51%, and the parabasal and spinous cell layer indicated increased cell positivity. 4. Invasive islands showed almost negative reaction to the antibody against type IV collagen, and almost indistinct line of the laminin antibody was observed. Based upon these findings, the loss of laminin and type IV collagen and the Ki-67 labelling index were the highest in the basement membrane of invasive islands. Therefore, it is thought that invasive islands were more deeply involved, than main proliferating tumor, in the invasiveness and proliferation of tumor, and the more the invasiveness of the tumor, the more the cell proliferative activity.

4.Separation of Unfused Olecranon Epiphysis in an Adult Baseball Pitcher: A Case Report

Unfused olecranon epiphysis has been reported rarely. We found an unfused olecranon tip within joint and a few small spurs on the apposing olecranon fossa and removed it successfully through the arthroscope. We think that it is a part of secondary ossification center of the olecranon and tbat it was separated through the epiphyseal plate by repetitive impaction of the tip of the olecranon into the olecranon fossa during pitching. In conclusion, fusion of the intra-articular part(articular center) of the olecranon epiphysis can not occur by nature. Repetitive extension overload during pitching may cause symptoms, and could be treated with arthroscopic control.

7.The Prevalence Rate of Ocular Symptoms and Diseases in the Urban and Rural Populations.

In search of the effect of air pollution on the eye, the authors examined ocular symptoms and diseases in both an urban(Seoul) and two rural populations (Suwon and Changsung). The Prevalence rate of ocular symptoms and diseases between 109 occupational drivers of more than 3 years career in Seoul area which is one of the air polluted urban areas and 150 populations in the two different rural areas as a control group were compared. The prevalence rate of ocular symptoms was 89% in Seoul, 10% in Suwon and 23% in Changsung area, and those of ocular diseases were 35%, 6% and 10% in the same areas, respectively. The prevalence rate of ocular symptoms and diseases is respectively 3 to 4 times more prevalent in occupational drivers than in the control groups. Many other etiologic factors such as occupation, labor condition, environmental stress or different way of living may influence the prevalence rate between the two groups, however, it would be presumed that the main causative factor to cause ocular symptoms and diseases is probably due to air pollution.

8.The Effect of Subconjunctival Injection of Tathion on Some Keratitis.

Authors experienced five different cases of keratitis such as metaherpetic keratitis, bullous keratitis, chemical keratitis and superficial punctate keratitis have been markedly improved by subconjunctival injection of Tathion (30-50mg), a glutathion prepartion. All cases showed remarkable improvement with better visual acuity following the treatment. Especially it was another choice of treatment in persisting case of metaherpetic keratitis with parenchymal infiltration. No side effects have been observed during this procedure.

10.Case report for Blepharospasm requiring careful considerations in the application of the blepharoplasty.

With the aging process, the senile lids are manifested by the excessive skin, decreased height of palpebral fissures with interference of the vision. Sometimes these patients often complained the blepharitis on the lateral canthal area and phtophobia. But there are many diseases to differentiate from senile lids such as dermatochalasia, blepharochalasia, hypertrophy of orbicularis oculi, herniation of orbital fat, myasthenia gravis, and blepharoptosis. Blepharospasm must also be differentiated because of sysmptoms similar to the above described senile lids. Essential blepharospam is an idiopathic progressively debilitating desease which involve bilateral spasm of the orbicularis oculi. So just only an conventional blepharoplasty could not get an improvement of symptoms and some times it makes the symptoms worsen. This case report is about blepharospasm patients visiting for blepharoplasty. And the patients got relatively the satisfied results with the injection of botulinum A toxin, aided by neurologist, instead of blepharoplasty. In conslutions, The blepharospasm in required to detect in preoperative evaluation for blepharoplasty. Once the blepharospasm is diagnosed, it is better to cooperate with neurologist and to do the blepharoplasty.
