2.A Comparison Study of the Staphylococcal Exotoxins and Staphylococcal Enterotoxin A-specific IgE Antibody between Childhood and Adulthood Atopic Dermatitis.

BACKGROUND: The skin of patients with atopic dermatitis (AD) exhibits a striking susceptibility to colonization with Staphylococcus aureus (S. aureus). Superantigens produced by S. aureus and their specific IgE antibodies are thought to be important precipitating factors of AD, but there are few reports evaluating these 2 factors at the same time, particularly in adult AD patients. OBJECTS: Our purpose was to investigate the differences in the culture degree of S. aureus from the lesion, non-lesion, and control group of child and adult AD patients, to research the correlation between the exotoxin production, total IgE, anti-SEA IgE and the disease severity by SCORAD index, to ascertain the differences between child and adult AD patients. METHODS: The clinical severity of 30 child (2 to 15 years of age) and 30 adult patients (16 to 40 years of age) with AD was evaluated by using SCORAD index. S. aureus was isolated from lesional and non-lesional skin of AD patients, and from healthy controls. Staphylococcal exotoxins were detected by using reversed passive latex agglutination toxin detection kits. Anti-SEA IgE antibody was determined by using AlaSTATt assay RESULTS: S. aureus colonizations were found in 11 (36.7%) of the lesional skin, in 5 (16.7%) of the non-lesional skin of 30 child AD patients, and in 26 (86.7%), in 20 (66.7%) of 30 adult AD patients, respectively. The colonization rates of S. aureus in child patients were much lower than those in adult patients, both form lesional skin and non-lesional skin. Staphylococcal exotoxins were detected in 5 (45.5%) of the 11 colonizations from lesional skin, in 2 (40%) of the 5 colonizations from non-lesional skin of children, and in 10 (38.5%) of the 26 colonizations, in 9 (45%) of the 20 colonizations of adults, respectively. Staphylococcal enterotoxin A (SEA) was most frequently detected in both groups. S. aureus colonization was correlated with the severity of AD in childhood, but not in adulthood. However, there were no statistical significances between severity of AD and others such as exotoxin production, and the level of total IgE and anti-SEA IgE in both groups. CONCLUSION: The colonization of S. aureus was more common in adult AD patients than child AD patients. Anti-SEA IgE level was much higher in adult AD patients than in child AD patients. It is tempting to speculate that the colonization of S. aureus and exotoxin production might be related to the disease. duration rather than clinical severity of AD.

3.A Case of Malignant Fibrous Histiocytoma.

Malignant fibrous histiocytoma (MFH) is a pleomorphic sarcoma which most frequently arises in the soft tissue. About two-thirds of the tumors are located within skeletal muscle; relatively few cases have been reported arising in the skin. The micromorphology of the tumor is characterized by storiform histiocyte-like cells, malignant giant cells and bizzare mitoses. We report here a case of cutaneous MFH developed in xeroderma pigmentosum occurred in a middle-aged man.

4.A Comparison Study on the Skin Irritation and Recovery by Irritants.

BACKGROUND: Corrosive irritants can be defined as irritants which in provoked weak reactions or subclinical reactions induce impairment of the water barrier function and increase of transepidermal water loss (TEWL) while non-corrosive irritants can be defined as irritancy of low degree but with no increase of TEWL. Sodium lauryl sulfate (SLS) has been considered as the standard example of corrosive irritant and nonanoic acid (NAA) is an example of a noncorrosive irritant. OBJECT: This study was done to evaluate the skin irritation and recovery by corrosive and non-corrosive irritant in normal human subjects. METHODS: 0.1, 0.2, 0.5, 1, 2, 5% solution of SLS and 1, 2, 5, 10, 20, 50% solution of NAA were applied on volar forearm skin in 15 normal healthy subjects. TEWL and Erythema index (E-index) were measured at before (BL), 30 minutes (D0), 1st day (D1), 2nd day (D2), 1st week (W1), 2nd week (W2), 3rd week (W3) after the removal of the patches. RESULTS: Increase of TEWL was accelerated according to concentrations in cases of SLS, whereas increase of TEWL according to concentrations was very weak in cases of NAA. SLS induced a significantly higher TEWL increase than NAA at corresponding concentrations. In both SLS and NAA, E-index was increased according to concentrations with no difference in reaction pattern. SLS and NAA induced similar degrees of E-index at corresponding concentrations. TEWL value was highest at 30 minutes and 1 day after removal of the patch in both SLS and NAA. TEWL was recovered to baseline value at 2 weeks after removal of the patch test in case of low concentrations, at 3 weeks after removal of the patch in case of high concentrations. E-index value was highest at 30 minutes, 1 day, and 2 days after removal of the patch in both SLS and NAA. The period of recovery to baseline varied depending on the concentrations. E-index was recovered to baseline value at 1 day after removal of the patch test in case of low concentrations, and was not recovered to baseline value after 3 weeks in case of high concentrations. CONCLUSION: Corrosive irritant, SLS, showing similar degree of erythema with non-corrosive irritant, NAA, induced much more damage to stratum corneum barrier function at corresponding concentrations. Skin injuries induced by corrosive irritants would need more prolonged recovery time than skin injuries by non-corrosive irritants, and TEWL measurement would be even more sensitive than E-index measurement in case of corrosive irritants, while both TEWL and E-index measurement could be useful in case of non-corrosive irritants.

5.Systemic Scleroderma in Childhood.

The onset of systemic scleroderma in the first. decade of lif occurs in less than 1.5% of instances. Systemic sclerodrma in childhood have been characterized by less severe visceral involvement and a benign course We report a boy, aged 7 years, with a history of Raynaid; phenomenon and dysphagia. He had a mask-like face, flexion contracture of hands and knee joints, digital pitting scars, ichthyosiform skin change on leg and hyperpigrnented induration on all extremities and trunk. Antinuclear antibodies were positive in nucleolar type and Scl-70 antiocy was also present. On radiologie study, the pattern of reflux esophagitis and resorption of the listal ends of all phalanges were nated. Histopathological findings were compatible with sclerodma. He was treated with Penicillamine-D and oral corticosteroic with good improvernent.

8.Tufted Angioma:Clinicopathologic Surveys and the Response to Intralesional Steroid.

BACKGROUND: Tufted angioma is an uncommon slowly progressive vascular tumor found typically in infants and young children with characteristic histologic findings, so called "cannonball" appearance. OBJECTIVE: The purpose of this study was aimed to investigate the clinical and histopathological characteristics of tufted angioma and the response to intralesional steroid. METHODS: Clinical information of 10 patients with tufted angioma diagnosed in Severance hospital and Pundang CHA hospital from 1983 to 1999 was obtained from the medical records and clinical follow-ups. We re-evaluated 10 biopsy specimens obtained from them with routine H&E staining. RESULTS: Five male and five female patients were included. In 9 patients the lesion appeared before 2 months of age. Four had a lesion at birth. The thigh was the most common site. The clinical symptoms were diverse, but characteristically tenderness was present in most cases. In all the patients the lesions had a tendency to spread progressively. Microscopically, numerous, distinct, variably sized, tightly packed capillary and endothelial cellular lobules were scattered in the dermis. There were characteristic semilunar spaces adjacent to the capillary tufts. Six patients received intralesional triamcinolone. This treatment was found to be effective in 5 patients who experienced remarkable improvement. The improved cases had similar histologic findings which were composed of cellular mass more than lumen formation. We classified our specimens into two categories, one with more cellular mass and the other with more lumen formation in relative proportion. The former was different from the latter in that it had more solid appearance and more definite margin. And we realized that it was useful to divide into these two categories since its response to treatment could be different. CONCLUSIONS: Tufted angioma is a relatively uncommon disease with characteristic histopathologic findings. It seems not to regress spontaneously. So early treatment is required to pre-vent further spreading up to the extent. We treated 6 patients with intralesional injection of triamcinolone and 5 patients experienced marked improvement which had more cellular mass more than lumen formation histopathologically.
