1.A Case of Isolated Rectal Tuberculosis.

Isolated rectal tuberculosis is defined as focal lesions of the rectum in the absence of demonstrable lesions in the small and large bowel, and it is a very rare form of intestinal tuberculosis. Common presenting features are rectal bleeding, diarrhea, constipation, tenesmus, and constitutional symptoms. Of them, rectal bleeding is the most common symptom. There are ulcerative, hypertrophic and combined types in the colonoscopic classification. Ulcerative type is the most common findings. The size of the ulcers are variable. The margins of the ulcer are irregular, swollen, erythematous and nodular. The base is covered with whitish to yellowish exudates and shows a granular appearance. For diagnosis of rectal tuberculosis it is important to obtain specimens at more than 6 regions in the base and margins of the ulcer. Important histologc findings are granuloma and demonstration of acid fast bacilli. I report a case of isolated rectal tuberculosis confirmed by colonoscopic study and histology.

2.No Association between DRD2 polymorphism Ser311/Cys311 and Bipolar Disorder in Korean Population.

OBJECTIVES: This study was designed to investigate the association between Ser311/Cys311 polymorphism in the dopamine D2 receptor gene and bipolar disorder in korean population. METHOD: Ser311/Cys311 polymorphism in the dopamine D2 receptor gene was typed with PCR in 86 bipolar disorders and 100 normal controls. RESULTS: Genotype of Ser/Ser, Ser/Cys, Cys/Cys were 82 (95.3%), 4 (4.7%), 0 (0.0%) respectively in the bipolar disorders, 96 (96.0%), 4 (4.0%), 0 (0.0%) respectively in the controls. Allele frequencies of Ser and Cys were 168 (97.7%), 4 (2.3%) in the bipolar disorders, 196 (98.0%), 4 (2.0%) in the controls. There were no differencies in genotype distribution and allele frequencies of dopamine D2 receptor gene polymorphism Ser311/Cys311 between in the bipolar disorders and in the controls. CONCLUSION: These result suggests dopamine D2 receptor gene polymorphism Ser311/Cys311 is not causally related to the development of bipolar disorder in korean population.

In recent years, marked increase in incidence of congenital syphilis has occured throughout the world due tochanges in social norms and development of penicillin-resistant strains. Early diagnosis plays an important rolein congenital syphilis as the clinical manifestations may simulalte many other conditions in the paediatric agegroup. The authors analized 52 cases of congenital syphilis admitted tot he department of paediatrics, ChosumUniversity Hospital, clinically and radiologically. Among them, 18 cases were born in this hospital and 34 caseswere admitted from OPD, during the period of 8 years from Jan. 1975 to Dec. 1982. The reults obtained were asfollows; 1. In 28 of 34 cases (82%), the first clinical manifestations were below the age of 3 months. 2. Amongthe 52 cases, a male predominance was observed with a male to female ratio of 2:1. 3. The serologic test (VDRL) ofthe 52 studied cases showed reactive in 49 cases (94%), and that of syphilitic mothers except 6 cases, rective inall studied cases. 4. The major manifstations of the 52 cases were bone tenderness (12%) and swelling of thejoints(7%) inskeletal system, hepatosplenomegaly (79%) and skin lesions (73%) in extraskeletal one. 5. Theradiological skeletal changes were detected in 45 of 52 cases(87%), and the commonest findings were detected in 45of 52 cases (87%), and the commonest findings were metaphsitis(83%) and periostitis(81%). The most characteristictype of metaphysitis were transverse trophic line(74%) and zone of rarefaction (65%). 6. The comonest bones to beaffected were growing metaphyses of the long bones, particulary about the wrist and the knee. The order offreqeuncy were radius (80%), ulna(80%), tibia(77%), femur(69%) and hymerus(40%).

7.Subtypes of Intestinal Metaplasia in Chronic Gastritis.

BACKGROUND/AIMS: Gastric carcinoma is the most common malignant neoplasm in Korea and is known to be associated with intestinal metaplasia (IM) of gastric epithelium, of which type III IM is suggested to play a special role in the carcinogenesis. This study is to evaluate the subtypes of IM and to measure each subtype in chronic gastritis. Methods: From October 1997 to September 1999, 321 patients with endoscopic chronic gastritis were evaluated the grade of chronic gastritis and IM by histologic and histochemical stain. RESULTS: Chronic inflammation and IM were remarkably severe in men and Helicobacter pylori (H. pylori) positivity was 69.4% in men and 65.2% in women. The frequency of IM was 50.1%, of which 65.4% was in men and 34.6% in women. The frequency of IM subtypes was 43.2%, 11.7% and 45.1% for type I, II and III respectively. Sixty percent of IM was noted in more than 50 years of age and ninety percent in more than 40 years of age. The frequency of type III IM was 61.6% in men and 38.4% in women. Sixty one percent of type III IM was noted in more than 50 years of age and ninety percent in more than 40 years of age. Interestingly, fifty three percent of type III IM was noted in men more than 40 years of age. The prevalence of H. pylori infection in type III IM was similar to in type I and II. The glandular atrophy in type III IM was remarkably severe than that in type I and II. Conclusions: The results of this study show that the proportion of type III IM in chronic gastiritis is remarkably high in Korean and age and sex distribution is similar to that of gastric carcinoma.

9.The Computerized Indexing of Korean Journals of Radiology with Personal Computer.

PURPOSE: To develop an efficient journal searching program. MATERIALS AND METHODS: A graphic user interface oriented program was developed with Microsofts Access 2.0 and its Deveolper Tool Kit under the Windows system. The program needed four floppy diskettes {4.5 megabyte) for in stallment. With the author, title, journal name, volume and pages, index words, and abstracts from JKRS and JKSMU as the input data, the utility of the program was tested for its journal searching ability in 486 DX II with 8 megabyte main memory. RESULT: The author, titlie, index word, abstract, and variable queries were carried out from 3,200 articles within 13 seconds. The test of the program satisfied and useres, radiologists, and physicians. CONCLUSION: The program is recommended for seaching of journals as it is very eary to use and userfriendly.

10.Colonic Perforation during Gastrografin Reduction of Intussusception in Children: Two Cases Reports.

In the intestinal perforation during hydrostatic reduction for intussusception, the diluted isotonic Gastrografin may prevent body fluid shifting to peritoneal space, and granulomatous reaction resulting in intestinal obstruction, which can be induced by Barium on the peritoneal surface. We have experienced two perforations from 563 Gastrografin reductions during the last five years. One of the perforated cases had no complication on one year follow up after ileocecectomy, but the other, who was in the state of shock before the reduction, died at 133rd day after the operation. The cause of death was aspiration during the feeding. We report the above two cases of intestinal perforation by Gastrografin reduction without evidence of fatal complication.
