1.Peripheral blood stem cell collections using Haemonetics MCS3p.

BACKGROUND: Reconstitution of hematopoiesis by means of peripheral blood stem cells(PBSC) has been widely used in patients with hematologic malignancies or solid tumors. We analyzed our experiences of PBSC collections with a MCS3p(Haemonetics Corp. Braintree, MA,USA) and investigated factors that may affect the collection of PBSC. METHOD: The forty-seven harvests in 11 cancer patients(median age 48, 4 males: 7 females) were performed during marrow recovery after administration of anticancerous regimens and and hematopoietic growth factors. RESULTS: Median number of WBC & mononuclear cells(MNCs) in preapheresis was 4,700/pl and 1,472/pl, retrospectively. A total of 11.2 x 10'(range 5.4-17.0) MNCs were collected from 7.2 l (5.5-8.9) blood processed per procedure. The mean percentage of CD34 positive cells in the product was 1.9%(0.9-2.9). Collection efficiency was 58.1%. While age of patients or total processed blood volume did not affect for total number of MNC collected, the number of WBCs including MNCs on the day of harvest was an importanr factor for higher yield of MNCs(P<0.05). CONCLUSION: We concluded that Haemonetics MCS3P. Is a fully automated blood cell separator with good collection efficiency for hematopoietic progenitor cells.(Korean J Blood Transfusion 10(1): 35-41,1999)

2.Aneurysmal Bone Cyst Recurred in The Neck of Femur

A case of aneurysmal bone cyst is reported in a 23 years old male, who complained of pain and tenderness on the right lateral aspect of thigh, This case had been treated with radical curettage and autogenous bone graft, but eight months later the radiological evidence of recurrence was present. So he was reoperated and he has been recovered relatively gradually, Roensgenograms of 7 montbs duration after reoperation showed abundent new bone formtion and no evidence of recurrence.

3.Ewing's Sarcoma: A Case Report of Ewing's Sarcoma

Ewing's Sarcoma is a rapidly growing malignant neoplasm that arises from primitive cells of the bone marrow in young persons. This case, a seventeen year-old male, complained of pain and swelling on the both knee. He had been treated under the impression of osteomyelitis at several local clinics. Clinically, malaise and high fever were continued during admission period. Microspically it was confirmed as Ewing's Sarcoma and then he was discharged. Presently he is under the radiation therapy.

6.Autologous Vs Allogenic Chondrocytes Transplantation for Full Thickness Chondral Defects in Rabbit's Patella.

Biologic resurfacing of the damaged joints is an area of great interest and clinical promise because of the limited potential ofdamaged articular cartilage healing. Several methods such as spongiolization. joint dehridement and ahrasion of suhchondral hone. perichondral grafts, and osteochondral grafts have heen used to repair cartilage defects, but the results were not satisfactory. Rccently autologous chondrocyle transplantation with a pcrioslcal patch was paid an altention for its advantage , the regeneration with hyalin cartilage. But it have many disadvantages such as too expensive cost. second staged operation, and technically difficult to isolatc chondrocytes from a small volume of donor site, so we performed that a definecl cartilaee delect in the ribbit patella was treated with transplanta1ion of in virto expanded allogenic chondrocvtes and then compared with an autologous chondrocytes transplantation. Adult rabbits were used to transplant autogenously and allogenously and allogenically harvested and in vitro cultured chondrocytes into patellar chondral lesions that had been made previously 3x 3mmin size , extending down to the calcified zone. Chondrocytes were isolated in the femoral condyle of the opposite knee or othe rabbit knee. And then enzymatic digestion ( collagenase A and DNase I ) was performed for 5 hours room temperature in a spinner bottle and cells were seeded in a 25cm2 culture flask in Dulheccos modified essential medium (DMEM), supplemented with l0% fetal hovine serum (FBS). The culture medium was changed twice weekly. After 14 days of culture, the cells were isolated hy irypsinization and transplanted into previously made chondral defects with an autogenous periosteal patch taken from the medial aspect of tibia. Healing ol' the defects was assessed by gross examination, immunohistochemical stain, and light microscope with hematoxylin-eosin stain at 8, 16, and 24 weeks. Allogenic and autologous chondrocytes transplantation significantly increased the amount of newly tormed repair tissue compared to that found in control knees in which the Jesion was solely covered hy a periosteal patch. The repair tissue, however, had a tendency of incomplete bonding to adjacent cartilage. This study shows that allogenic and autologous articular chondrocytes that have heen expanded for 2 weeks in vitro can stimulate the healing phase of chondral lesion. There is no signilicant diffcrence hetween allogenic and autologous chondrocytes transplantation.

7.Reconstruction of Anterior Cruciate Ligament with Iliotibial Band Transfer

Ligamentous injury of the knee which could be easily occur in sports or traffic accidents resulted in more and severe disability than that of other joints. Rupture of the anterior cruciate ligament which restrain hyperextension, excessive rotation and anterior motion of tibia on femur is the most frequent and significant injury of the knee. Its treatment and clinical significance is one of the most controversial issue and its incidence tends to increase. From 1983 to 1985, modified Scott operation were performed on 10 patients at the department of orthopedic surgery, Chung Ang university hospital. Following results are obtained. 1. No limitation of range of motion of the knee was developed. 2. Intra-articular fixation by use of pull-out wire was done and special consideration was done for the site of fixation on tibia plateau and method of dilatation of posterior capsule. 3. Of the ten patients, nine had negative anterior drawer test, postoperatively.

8.Five Cases of Autoimmune Bullous Disease Presenting Localized Facial Lesions.

We describe five patiens with autoimmune bullous der matosis who presented cutaneous lesions localized on the face. In these patients iuumunopathologically confirmed diagnoses were penphigus erythematosus, bullous pemphigoid, cicatricial pernphigoid(Brunsting-Perry variant), epidemic lysis bullosa acquisita and bullous systemic lupus erythematosus in each. Except, for the case of pemphigus erythematosus, facelimited cutnious lesions can be seen rarely with these imrnunobullous diseases. We suggest that some external, in malfactors such as UV light and photosensitivity might be regarded as the precipitating factors fo the clinical rnanifestations of those skin lesions, and which should be included in the differential diingosis of vesiculobullous diseases involving the face.

9.The Study on Inhibit Activity of Dermatophytes by Some Salt Solutions.

The study was made in order to investigate the minimal inhibit concentration (M.I.C) to Trichophyton(T.) gypseum and Trichophyton rubrum by same salt solution(ie: NaCl, KCl and Sodium thiosulfate). The stanis were obtained at the out-patient clinic of Seoul National University hospital. The results are as follows: 1. The M.I.C. of NaCl manifest 10%, and Trichophyton rubrum shows more higher than that of T. gypseum. 2. The M.I.C. of Sodiurn thiosulfate manifest 10%, and T. gypseum shows more higher M.I.C. than that of T. rubrum. 3. The M.I.C. of KCI manifest 15% for T.rubrum and shows 20% for T. gypseum.

10.Respiratory Sleep Physiology.

Regulation of respiration differs significantly between wakefulness and sleep. Respiration during wakefulness is influenced by not only automatic control but also voluntary and behavioral control. Sleep is associated with definite changes in respiratory function. With the onset of sleep, voluntary control of ventilation that overrides automatic control during wakefulness becomes terminated. Also ventilatory response to various stimuli including hypoxemia and hypercapnia is decreased. With these reasons respiration during sleep becomes fragile and unstable so that marked hypoxemia can be happened in patients with lung disease especially during REM sleep. Obstructive sleep apnea may also be developed if upper airway resistance is increased in addition to these blunted ventilatory responses.
