1.Serous Cystadenoma of the Pancreas: A case report.

Serous cystadenoma of the pancreas, also known as microcystic adenoma or glycogen-rich cystadenoma, is an unusually benign tumor. It is usually large and composed microscopically of many small cysts lined by small, cuboidal or flattened cells containing abundant glycogen. It has been suggested that serous cystadenoma probably arise from the ductular cells or centroacinar cells. Herein, we report on a case of serous cystadenoma of the pancreas in a 55-year-old female. The tumor, measuring 13.5x11.5x10.0 cm, was located in the head of the pancreas and the cut surface revealed a sponge-like appearance due to innumerable tiny cysts containing clear serous fluid. Microscopic analysis showed cystic spaces lined by cuboidal cells with intracytoplasmic glycogen.

2.Effect of CORE Program on Cognitive, Social and Emotional Characteristics of Sex Offenders.

The purpose of this study was to develop the CORE Program for sex offenders and to determine its effectiveness. The CORE Program was designed with an aim to achieve cognitive restructuring, improve the ability to empathize in interpersonal relationships, and enhance self-esteem and intimacy. We conducted this program over 48 sessions for 28 sex offenders. The effectiveness of the program was evaluated using the Interpersonal Responsiveness Index (IRI), Self-esteem Questionnaire (SEQ), UCLA Loneliness Scale (UCLALS), Coping Using Sex Inventory (CUSI), Rape Myth Acceptance Scale (RMAS), and Wilson's Sex Fantasy Questionnaire (WSFQ). The data were analyzed using paired t-tests. Our results showed no significant changes in the SEQ, UCLALS, and IRI scores after the treatment program. However, the scores for the CUSI, RMAS, and WSFQ significantly improved after this program. In this paper, we demonstrate the effectiveness of the CORE Program for sex offenders. We also discuss the limitations of our study and provide suggestions for future research. Our findings indicate that this treatment program should be provided to sex offenders for preventing recidivism.

3.Analysis of Function and Evaluation of Knee Stability after Arthroscopic Reconstruction of ACL with BPTB Autograft.

PURPOSE: To analyze the anterior stability and functional results after the arthroscopic ACL reconstruc-tion with bone patellar tendon bone autograft in the ACL deficient knee. MATERIALS AND METHODS: From March 1993 to May 1998, 24 patients were treated by arthroscopic ACL reconstruction with BPTB autograft and followed more than 1 year(range, 12~82 months, average: 30.5 months). Objective stability was estimated under the loading of 15 lb and 20 lb using KT 2000 arthrometer. Functional evaluation system of OAK knee score and Lysholm knee score was used. RESULTS: Average anterior displacement under the loading of 15 lb was 5.4+/-2.4 mmin the reconstructed knee, 4.2+/-1.8 mmin the normal side and under the loading of 20 lb, 8.3+/-3.1 mmin the reconstructed knee, 6.0+/-2.5 mmin the normal side. The percentage of anterior displacement within 3 mmdifference compared with normal side revealed 75% under the loading of 20 lb(89 N) and 87.5% under the loading of 15 lb(68 N). Functional results according to OAK system showed an average 83.8 score, and excellent in 9 cases, good in 9. Lysholm score revealed an average score of 80 and excellent in 5, good in 8. CONCLUSIONS: The arthroscopic ACL reconstruction with BPTB autograft results in good stability and functional results. Effective quadriceps muscle strengthening exercise and rehabilitation is necessary to prevent limitation of range of motion, quadriceps muscle atrophy and anterior knee pain post-operatively.

4.Status of Occupational Therapists on Unilateral Neglect Test Tools Usage and Symptom Classification.

PURPOSE: The aim of this study was to investigate the current status of the use of neglect test tools and the awareness for egocentric neglect (EN) and allocentric neglect (AN). METHODS: A survey questionnaire was distributed to occupational therapists attending continuing education at the Daegu-Gyeongbuk branch on April 16. A total 143 responses were collected, and with the exception of 5 incomplete responses, 138 of them were analyzed. RESULTS: The most commonly used unilateral neglect test tools were the line bisection test (86 responses, 62.3%), cancellation test (35 responses, 25.4%), copy and drawing test (7 responses, 5.1%). In a follow-up survey of 86 respondents who used line bisection tests as the most commonly used test, the majority (82 responses, 95.3%2) were found to use the 20-line test paper. In question about the familiarity and education experience for test manual, only 20 responses (23.2%) and 3 responses (3.5%) answered ‘Yes’. In a question about the experience of a manual and the article for the manual, 25 responses (29.1%) answered ‘Yes’. In the question regarding the separation of EN and AN on the unilateral neglect test, 44 responses (31.9%) were ‘distinction’ and 94 responses (68.1%) were ‘no distinction’. CONCLUSION: Information on the guidance and interpretation of the line bisection test is lacking and the concept of EN and AN was insufficient.

5.Clinical Significance of Carbon Monoxide Induced Myocardial Injury.

PURPOSE: This study was performed for investigation of epidemiology, clinical characteristics, and serial value of cardiac troponin level of patients who had myocardial injury due to Carbon monoxide poisoning. METHODS: This study reviewed 98 cases of Carbon monoxide poisoning patients who visited Emergency Department from January 1, 2008 to October 31, 2013. We categorized them by two groups, one with elevation of cardiac troponin level and the other with normal level. We had comparison between two groups data using statistical analysis. RESULTS: Among 98 patients of Carbon monoxide poisoning who were admitted to hospital, 10 patients were excluded. 88 patients who were included to our study, 70 patients with normal value of Troponin, and 18 patients with elevated troponin level. Of all patients, Carbon monoxide inhalation due to suicided trial patients has more higher proportion in elevated troponin level group compared with normal group (40 (57.1%) vs 15 (83.3%), P=0.041). Furthermore, corrected QT interval, length of hospital stay, number of ICU admission, also were showed higher value in elevated troponin level group. CONCLUSION: Carbon monoxide induced myocardial injury is associated with subside trial, prolongation of correted QT interval, length of hospital stay, and number of ICU admission.

6.Gastric Carcinoma with Lymphoid Stroma: Pathologic and immunohistochemical study of 14 cases.

A total of 672 surgically resected gastric carcinomas were reviewed, and fourteen cases(2.1%) of "gastric carcinoma with lymphoid stroma" were identified. The tumor was located mostly in cardia and body portion(86%). Thirteen patients one was lost to follow up were alive without recurrence of tumor. Grossly, this carcinoma was characterized by an expanding growth pattern or a multinodular pattern. Histologically, this carcinoma showed an irregular trabecular and alveolar arrangement and densely infiltrated lymphoid cells with lymph follicles. The tumor cell nests were widely separated by non-desmoplastic lymphoid stroma. On immunohistochemical study, T cells were evenly distributed throughout the tumor lesion with intimate contact with individual carcinoma cells, but B cells were mainly present within the germinal centers of lymph follicles. Thus, the organized immune response combined with cell mediated and humoral immunities against the carcinoma cells may be a defense mechanism of the host in this type of gastric carcinoma.

8.Prevalence of and Risk Factors for Levofloxacin-Resistant E. coli Isolated from Outpatients with Urinary Tract Infection.

PURPOSE: Levofloxacin has been widely used because of its broad spectrum and excellent penetration of the prostate. But levofloxacin-resistant Escherichia coli (E.coli) has been reported all over the world. We analyzed the annual levofloxacin resistance of E. coli and its risk factors. MATERIALS AND METHODS: From 2005 to 2009, we retrospectively analyzed patients who had undergone a urine analysis and a urine culture at the Outpatient Section of the Department of Urology of Kwangju Christian Hospital. Among them, we chose 509 patients infected by E. coli and evaluated the resistance rate to levofloxacin and its risk factors. RESULTS: The annual rates of levofloxacin resistance of E. coli were 29.49% in 2005, 26.51% in 2006, 40.21% in 2007, 43.20% in 2008, and 31.75% in 2009. A close correlation with the resistance rate was shown in cases that had underlying neurogenic bladder (p=0.002, odds ratio [OR]=4.236), a history of ciprofloxacin (p<0.001, OR=3.753) and levofloxacin (p<0.001, OR=2.996) administration for at least 48 hours in the past year, urolithiasis (p=0.003, OR=3.317), and older age (p<0.001, OR=1.027). CONCLUSIONS: This study from 2005 to 2009 showed that the levofloxacin resistance rates of E. coli were high at over 25%. The risk factors that affected the levofloxacin resistance rates of E. coli were underlying neurogenic bladder, ciprofloxacin administration history, urolithiasis, levofloxacin administration history, and older age. Levofloxacin should be prescribed cautiously in patients with these risk factors until the pathogen is identified.

10.Expression of Oncogene Product in the Colorectal Carcinoma.

The expression of p62"' and p2l can be seen in many solid tumor, but the pattern and incidence of expression were different according to organ, countries, and examiners, thus it is not definitely defined. Total 67 colorectal carcinoma in paraffin sections are analysed by immunohistochemically for evaluation of the p62and p21- expression according to the age, sex, chief complaints, location, differentiation, modified Dukes stage, using the specific monoclonal antibodies. The results were summarized as follows : The age of patients ranged from 32 years to 82 years. The mean age was 57.6 years. The expression of p 62y' and p21 was not correlated with age. Male was 29 cases(43.3%) and female was 38 cases(56.7%). The male to female ratio was 1:1.31. The and 95.0% in moderately differentiated adenocarcinoma, 90.0% and 90.0% in poorly differentiated <,;, adenocarcmoma, 100.0% and 100.0% in mucinous carcinoma. The positive rate of p62!and and p2l' was 94.1% and 88.2% in Dukes stage B,, 96.0% and 96.0% in Dukes stage B,, 100.0% and 100.0% in Dukes stage C,, 100.0% and 88.9% in Dukes stage C, and 100.0% and 100. o% in Dukes stage D. The expression of p62 y' in metastatic colorectal carcinoma showed diffuse and strongly positive reaction than primary colorectal carcinoma. The expression of p21 in primary colorectal carcinoma showed diffuse and strongly positive reaction than metastatic colorectal carcinoma.
