1.Structural Equation Modeling on Contraception Behavior of Unmarried Men and Women in Korea: Gender Difference.

PURPOSE: The purpose of this study was to test and validate a model to predict contraception behavior in unmarried men and women. METHODS: Data were collected from a questionnaire survey of 180 unmarried men and 186 unmarried women 20 years of age or over who had sexual relationships in the past 6 months. Participants were from Seoul, Kyunggi, Daegu, and Busan and data collection was done from February 19 to April 16, 2013. RESULTS: Model fit indices for the hypotheoretical model fitted to the recommended levels. Out of 15 paths, 11 were statistically significant in both. Predictors of contraception behavior in unmarried men and women were intention to use contraception and self-efficacy for contraception. Exposure to sexual content was directly significant to the intention in men only. Self-efficacy for contraception was affected by perceived threat of pregnancy and gender role attitude. In women, the two predictors were also significant except for the effect of exposure to sexual contents. CONCLUSION: Results indicate that an intervention program which increases self-efficacy in unmarried men and women contributes to effective contraception behavior. In addition, proper sexual education programs using positive aspect of mass media can help develop active participation for contraception behavior.

2.Hyperinfection of strongyloides stercoralis.

Strongylodiasis is universal in distribution but is most abundant in countries with a tropical climate. Although infestation by Strongyloides stercoralis is usually limited to the intestines, dessemination of this helminth in debilitated host can be lead to death with various clinical disorders. characterized by profound malabsorption, diarrhea, electrolyte imbalance, gram negative or opportunistic fungal sepsis, coma and death. Cell-mediated immunity contributing significantly to the control of helminthic infections, may be suppressed by carcinoma, immunosuppressive chemotherapy and use of corticosteroids. Diagnosis of Strongyloidiasis is achieved by an examination of samples of feces, duodenal aspirates and sputum of patients for Strongyloides stercoralis. Treatment of strongyloidiasis is twofold : correction of the immunosuppressive state by withdrawal of immunosuppressive drug, if possible, and vigorous treatment with thiabendazole. Testing for strongyloidiasis is especially recommanded before treating a patients should be monitored for infection by Strongyloides stercoralis and other opportunistic infection. We are reporting a case patient with Strongyloides stercoralis hyperinfection and pulmonary tuberculosis who had been. used corticosteroid for persisting polyarthritis.

3.Contraception Behavior and Related Factors in Unmarried Female and Male.

PURPOSE: This study aimed to identify differences in contraception behavior and related factors between unmarried female and male. METHODS: A cross-sectional survey design with a convenient sampling was used. From Seoul, 107 women and 96 men were recruited. A structured questionnaire was self-administered from July 14, 2008 to September 2, 2008. RESULTS: The average ages of women and men were 26.9 and 27.8 years old, respectively. Fifty-six percent of women and 85.4% of men have experienced sexual intercourse. There were no differences between female and male in attitude toward contraception, subjective norm, and contraception behavior, while female had higher score of self-efficacy for contraception (p=.02) and intention of contraception (p=.02). There was positive correlation with subjective norm (r=.22, p<.05), self-efficacy (r=.53, p<.01) in male and attitude (r=.32, p<.05), self-efficacy (r=49, p<.01) in female. According to the result of multiple regression, the significant factor of contraception behavior was self-efficacy for both female (beta=.49, p=.00) and male (beta=.53, p=.00). CONCLUSION: To improve contraception behavior for unmarried female and male, it is necessary to develop proper sex education program to enhance self-efficacy for contraception. Adjusted education program by this result will contribute to increase sexual health for female and male.

4.Bone SPECT, Simple Radiography , and Operative Findings in Osteoarthritic Knee with or without Anterior Cruciate Ligament Injury.

In so-called primary osteoarthritic knees, there may be secondary osteoarthritis due to the instability from chronic cruciate ligament and/or meniscal tear. The purpose of this study is to compare the findings of bone single-photon emission computed tomography(SPECT) and those of simple radiography in osteoarthritic knees which we consider as primary on the status of anterior cruciate ligament(ACL). We reviewed the preoperative bone SPECT and simple radiography of forty-three osteoarthritic knees in 23 patients who underwent total knee arthroplasty between 1995 and l996. We divided the cases into two groups on the status of ACL: thirty intact ACLs(Group I), thirteen insufficient ACLs(Group II). Meniscal tear and/or ACL insufficiency were found in 38 of 43 knees, As regards with presence of osteophytes and sclerotic changes on simple radiograph, there was no significant differences between group I and II. Joint space narrowing was more prominent and diffuse in group I(medial 29, lateral 24) than group II(medial 7, lateral 6). Bone SPECT showed diffuse uptake in group I(medial 30, lateral 23), but less uptake on the lateral compartment(medial 13, lateral 3) in group II. In ACL-intact osteoarthritic knees, joint space narrowing on simple radiograph and hot uptake on bone SPECT were more prominent and diffuse than in ACL-insufficient knees. Clinical relevance is still uncertain and further investigation is needed.

6.A Dual (Brachial and Contralateral Femoral) Approach for Subintimal Angioplasty of Long Ilio-Femoral Occlusive Disease Including the Iliac Ostium.

As techniques and device technology have improved, the success rates and long patency of ilio-femoral occlusive disease have also improved. In the case of extensive iliac occlusive disease, however, wire passage and handling remain a challenge due to the relatively weak guiding catheter backup support with the contralateral femoral approach. There has been no report on methods to overcome this problem. We performed a successful percutaneous translunimal angioplasty for long ilio-femoral occlusive disease including the iliac ostium by a dual approach including simultaneous brachial and contralateral femoral arteries for subintimal angioplasty.

8.Areola Reconstruction: FTSG and Micropigmentation.

The reconstruction of nipple and areola after breast mound reconstruction is fundamental in achieving final outcome. A variety of methods have been applied to make natural nipple and areola. The purpose of this study is to compare the operative methods and experience between FTSG and micropigmentation. Nipple-areolar complex was reconstructed after free TRAM flap for 33 breasts from May 1995 to August 2001. 20 patients underwent micropigmentation(Permark (R)) for areola and 13 patients underwent FTSG. Full thickness skin was obtained from inguinal area for FTSG method. Micropigmentation for areola was done 2 months after nipple reconstruction. There was no specific complication reported after areola reconstruction, but some differences in outcomes were noted between the two methods of areola reconstruction. First of all, there was an advantage in time saving for FTSG method due to simultaneous reconstruction of nipple and areola. However, FTSG has several disadvantages such as longer operation time, sometimes prominent scar formation of the inguinal and circumareolar region, feeling of shame due to location of the donor site, the lightening of the areolar color with time, and finally the color difference between the nipple and areola. Micropigmentation has advantages with simple procedure, short operation time, no donor site scar, no shameful feelings for dressing, no color change of areola with time, and easier color selection for the areola and nipple. It has disadvantages of double operation due to separate nipple and areola reconstruction, need for additional instruments, and experience in selection of well matched paste. FTSG from the inguinal region often had been used to produce the similar color of areola and was thought to be the standard method. However, we thought that areolar reconstruction utilizing micropigmentation is much better than FTSG in terms of cosmesis and convenience for operation.

10.A Case of Cedecea davisae Peritonitis in a Liver Cirrhosis Patient.

Cedecea davisae is a motile, Gram-negative rod in the family Enterobacteriaceae which is positive for lipase, DNase and catalase, and negative for gelatinase and oxidase. This bacterium is rarely isolated in the clinical specimens. We isolated C. davisae from the ascitic fluid of a 49-year old male patient with liver cirrhosis who was diagnosed as acute bacterial peritonitis. Bacterial identification was performed by API 20E and VITEK. Antimicrobial susceptibility test showed that the isolate was susceptible to cefotaxime, piperacillin, and imipenem. Peritonitis of this patient was improved by imipenem therapy. This is the first reported case of peritonitis caused by this organism.
