3.Health Status for the Elderly Living in a Home for the Aged.

BACKGROUND: The number of the elderly living in a home for the aged is increasing as the socio-culture changes, but health management for the institutionalized elderly has been discontinuos, centered on the single treatment of the disease. The aim of this study is to collect baseline data which is for development of the health maintenance program for the elderly living in a house for the aged. METHODS: We reviewed the journals related to the health maintenance program for the elderly. We selected the contents about health management program for the elderly. 3 domain of the contens were done. The 3 domain were physical health (interview for health habit, physical exam, clinical test), mental health (Korean version of geriatric depression scale, Korean version of mini mental status examination), assessment of functional status (katz activities of daily living, lawtons instrumental activities of living).. 158 elderly people living in a home for the aged in Seoul have been researched since August 1996. RESULT: Total subjects were 158 (men 73, women 85). The average age was 78.1(+/-7.2). The rate of smoking, alcohol drinking, exercise were 32%, 20.4%, 34.4%, each. The rate of assitive device use was 28.3%. The prevalence of sleep distur-bance, urinary incontinence were 69.7%, 17.3%. The prevalence of gait disturbance was 38.4%. The occurrence rate of falling for last 1 year was 29.6%. The prevalence of fear of falling was 57.2%. The positive rate of VDRL was 12.3%, the prevalence of anemia was 48.7%. The prevalence of degenerative arthritis, hypertension were 44.2%, 31.3%, each. The prevalence of depression, dementia were 71.3%, 56.7%, each. The rate of elderly person dependent on one and more ADL was 8.1%. CONCLUSION: We found the health status of the elderly living in a home for the aged. On the basis of the results of the study, it is needed that continuous application of the program for management of the elderly in a home for the aged, keeping searching proper contents of examination and methods of measurement.

5.Job Stress and Work-Related Musculoskeletal Symptoms of 119 Emergency Medical Technicians.

PURPOSE: This study was done to investigate the prevalence of job stress and musculoskeletal symptoms, and to identify the factors that affect work-related musculoskeletal symptoms of the 119 Emergency medical technicians (EMT). METHOD: From August 26 to September 10, 2010, the data were obtained from 456 EMT working in Daejeon city or Choongnam province. For data analysis, descriptive statistics and multiple logistic regression were performed using SAS version 9.1. RESULTS: Physical environment, job demand, interpersonal conflict, and occupational climate were stressful to 119 EMT. The prevalence of musculoskeletal symptoms of female EMT was higher than those of the male EMT's. After gender and age were adjusted, there ware significant relationships between musculoskeletal symptoms and some risk factors including occupational class, hours of intensive musculoskeletal use, previous injury or work-related injury, physical burden, and job stress. Overall, a higher degree of job stress increased musculoskeletal symptoms. CONCLUSION: Job stress is a major cause of musculoskeletal symptoms. To prevent and manage musculoskeletal disease of 119 EMT, there is a need to develop a management program for musculoskeletal symptoms to reduce occupational stress, considering gender differences.

6.The Attitude toward Marriage, Pregnancy and Abortion among Korean Nursing Students.

PURPOSE: There recurred controversies related to legal allowance of abortion since newly advancing prenatal genetic testing technology. This survey was aimed to identify the attitudes toward marriage, and pregnancy (AMP) and terminating pregnancy (ATP) among Korean nursing students. METHOD: A descriptive study was performed from October to November 2012 in South Korea. 424 nursing students responded to two types of tools developed by authors. Questionnaire for ATP is consisted of 10 items with 5-points Likert Scale. Questionnaire for AMP is 12 items containing 5 categories. RESULTS: Mean score of ATP was 36.90. Students that are enrolled in bachelor degree had a significant higher ATP. Students who are currently dating with an opposite gender showed much higher level of ATP than those are not. Lower group of delayed the time of getting marriage and pregnancy showed highest score of ATP, significantly. CONCLUSION: Individual factor must be primarily considered when assessing ATP of nursing students, for example current relationship with opposite gender and educational background, attitude toward marriage and pregnancy. Further studies are needed to identify personal factors of the young adults affecting on the ATP.

7.Anisotropy Measurement and Fiber Tracking of the White Matter by Using Diffusion Tensor MR Imaging: Influence of the Number of Diffusion-Sensitizing Gradient Direction.

PURPOSE: Recent development of diffusion tensor imaging enables the evaluation of the microstructural characteristics of the brain white matter. However, optimal imaging parameters for diffusion tensor imaging, particularly concerning the number of diffusion gradient direction, have not been studied thoroughly yet. The purpose of this study was to evaluate the influence of the number of diffusion gradient direction on the fiber tracking of the white matter. MATERIALS AND METHODS: 13 healthy volunteers (ten men and three women, mean age 30 years, age range 23-37 years) were included in this study. Diffusion tensor imaging was performed with different numbers of diffusion gradient direction as 6, 15, and 32, keeping the other imaging parameters constant. The imaging field ranged from 1 cm below the pons to 2-3 cm above the lateral ventricle, parallel to the anterior commissure-posterior commissure line. FA (fractional anisotropy) maps were created via image post-processing, and then FA and its standard deviation were calculated in the genu and the splenium of the corpus callosum on each of FA maps. Fiber tracking of the corticospinal tract in the brain was performed and the number of the reconstructed fibers of the tract was measured. FA, standard deviation of FA and the number of the reconstructed fibers were compared statistically between the different diffusion gradient directions. RESULTS: FA is not statistically significantly different between the different diffusion gradient directions. By increasing the number of diffusion gradient direction, standard deviation of FA decreased significantly, and the number of the reconstructed fibers increased significantly. CONCLUSION: The higher number of diffusion gradient direction provided better quality of fiber tracking.

8.A comparison between obstructive hydrocephalus and non-obstructive hydrocephalus on midsagittal brain MRI.

Hydrocephalus refers to distension of the cerebral ventricles, usually as the result of obstruction somewhere along the pathway of the cerebrospinal fluid circulation. Hydrocephalus ex vacuo means ventricular enlargement in the presence of cerebral atrophy or irreversible loss of brain parenchyma. According to morphologic change of ventricular enlargement, the mechanism and results of treatment are quite different. MRI improves diagnostic accuracy in patients with hydrocephalus because of its ability to show small obstructing lesion and anatomical changes. To evaluate differential points, we compared patients with obstructive hydrocephalus and non-obstructive hydrocephalus. T1-weighted sagittal images were retrospectively analyzed in 19 patients with obstructive hydrocephalus, 22 non-obstructive hydrocephalus, and 36 normal subjects. On midsagittal section, we evaluated mamillopontine distance, thickness and shape and position of the corpus callosum, and antero-inferior portion of the 3rd ventricle. The average mamillopontine distance was 11.5mm for normal subjects, 8.2mm for obstructive hydrocephalus group, and 13.6mm for non-obstructive hydrocephalus group(p < 0.01). Dilatation of the anterior inferior portion of the 3rd ventricle was seen only in 11 patients of obstructive hydrocephalus group. Average thickness of the corpus callosum at the level of the foramen of Monro was 7.5mm for normal subjects, 5.1mm for obstructive hydrocephalus group, and 4.4mm for non-obstructive hydrocephalus group(p < 0.01). There was a difference in the shape of corpus callosum between obstructive hydrocephalus and non-obstructive hydrocephalus group. It is concluded that mamillopontine distance, thickness and shape of the corpus callosum, dilatation of the antero-inferior 3rd ventricle could be good differential points between obstructive hydrocephalus and non-obstructive hydrocephalus.

9.The Role and Importance of T1 Weighted Axial Image on the Diagnosis of Anterior Cruciate Ligament Tear.

PURPOSE: The study is for the evaluation of the role and importance of the T1- weighed axial image on the MR diagnosis of the anterior cruciate ligament(ACL) tear. MATERIALS AND METHODS: We reviewed the axial findings of injured ACL on MR images of arthroscopically confirmed 41 patients. The points of ^CL abnormality on axial image were centered on the substance signal change, thickness change, focal bulge or irregularity of medial border. RESULTS: ACL complete tears were present in 31 patients and partial tears in 5 patients and, ligamentous atrophy or loss by chronic tear in 5 patients. Completely torn ACL were seen as diffuse swelling with abnormal substance signal(A) in 24 cases, focal bulge or irregularity of medial border(B) in 6 cases, normaIcy in 1 case. 9 cases among type A cases also showed type B features. Partially torn ACL were seen as roedial marginal irregularity in 3 cases, mild and diffuse swelling in 1 case, normaIcy in 1 cases. Atrophied or absent ACL fibers by chronic tears in 5 cases were demonstrated on axial images as abnormally thin fibers or so-called "empty lateral wall". Detection rate for ACL abnormality suggestive of fear on Tl-weighted axial image very high(94%, 39/41 ). CONCLUSION: Because Tl-weighted axial image serves as another good reference of plane for direct visualization of injured ACL with high abnormality detection rate, it would aid in the diagnosis of ACL tear.
