1.Congenital Adrenogenital Syndrome: Two Cases Report.

A study was made on the two cases of congenital adrenogenital syndrome. Cliteroplasty wag performed on the two cases in addition to adrenalectomy in one case. Good results were obtained following the treatment.

2.A Study on the Serum Lipoperoxide Level in the Normal Korean and in the Cases with Various Diseases.

Serum Lipoperoxide levels were studied in 50 normal Korean and in the following patients; 26 patients with diabetes mellitus, 20 patients with hypertension, 14 patients with ischemic heart disease, 8 patients with cerebrovascular accident and 6 patients with nephrotic syndrome. The results obtained are summarized as follows; 1. In normal Korean, the average serum lipoperoxide level was 2.62+/-0.27 nmoles M.D. A./ml, with no difference by the sex. 2. In the cases with various disease which in supposed to be related to abnormal lipid metabolism, that is, diabetes mellitus, hypertension, ischemic heart disease, nephrotic syndrome, the serum lipoperoxide value showed statistically significant elevation compared to that of the normal control (p<0.01), but in the patients with cerebrovascular accident the elevation was not significant (p>0.05). 3. The serum lipoperoxide level was coarsely related with the serum concentration of cholesterol and total lipid, the correlation coefficient of which were 0.34 and 0.35 respectively (p<0.05).

4.A Study of Anctiviy with CAstsnea Crenata Flos.

There are many herbs used in the treatment of dermatophytosis as home medicine and Castanea crenata Flos (C.C.F.) is one of that used in Kyung gi-do district. With that regard we tried to examine in vitro antifungal activity of the C.C.F. with its water extract and alcohol extract as the test materials. Until now many authors have studied the antifungal activity with several plant extracts, as like the alcohol extracts of Zanthoxylum piperatum Fructus and Berberis Korea palibin or Fennel oil, and reported that these plant extrscts or essential oiI have good antifungal activities in vitro and in vivo. On that they said that the principal fractions those have strong fungistatic activity were all alikely the fatty acids chemically similar with the well known undecylenic acid. At this in vitro experiment the extracts of C.C.F. were diluted. serially in the Sabourauds dextrose agar and certain portions of fungi (the strains of Trichophyton (Tr.) mentagrophytes and Tr. rubrum) were transplanted into each medium containing different concentrations of test materials then observed its growth for 2 weeks at room temperature. After that we measured the sizes of colony grown in various conditions and compired with that of the normal medium to determine the fungistatic effectiveness. As the result, in the view of the colony sizes, Tr. mentagrophytes and Tr. rubrum equally revealed marked inhibition of its growth only in the media with water extract over the concentration of 10mg/ml in the mediurn, and the critical fungistatic dilution was 20 mg/ml. We could not identified the principal fraction in the water extract of C.C.F.which exhibited antifungal activity. In the comparison of the critical fungistatic dilution with the alcohol extract of Zanthoxylum piperaturn Fructus and Fennel oil, water extract of C.C.F. required tenfold higher concentration for the same effect. However in regarding the utilizing habits of C.C.F. in horne medicine, as topical wet dressing with water boiled solution, we might expect its effect practically. iVow there are problems to be investigated about the chemical natures and physical properties of the C.C.F. and for the antifungal fractions of it, in addition further in vivo study we thought would be interesting.

5.Development of B-eNSP (Baptist Electronic Nutritional Screening Program) Using Integrated Medical Information System and Clinical Efficiency.

PURPOSE: The aim of this study was to develop a baptist electronic Nutritional Screening Program (B-eNSP) that could be easily used to identify patients with nutrition at risk or malnutrition on their admission to hospital using an integrated medical information system and to evaluate validity, reliability and efficiency of B-eNSP. METHODS: From June 1 2009 to June 21 2009, we enrolled 496 patients for prospective research. We analyzed age, sex, weight, height, weight loss in the last 3 months, serum albumin, alteration of dietary intake, gastrointestinal symptom, functional capacity, diagnosis and its relationship to nutritional requirements, physical examination and Subjective Global Assessment (SGA). B-eNSP included body mass index (BMI), weight loss in the last 3 months, serum albumin. Each component was scored. Sensitivity and specificity were calculated to evaluate the validity of the B-eNSP. The receiver operating characteristic (ROC) curve was drawn by using B-eNSP to choose a cut-off value that maximizes sensitivity and specificity and Yoden Index. Comparison with SGA and the reliability of the B-eNSP was done using kappa statistics. RESULTS: The maximum Yoden Index was 0.866 and the cut-off value of the ROC curve was 2. A B-eNSP score higher than 2 was defined as nutrition at risk or malnutrition. Reliability of the B-eNSP was in accordance with SGA by kappa 0.845. CONCLUSION: The B-eNSP can be used efficiently to identify patients with nutrition at risk or malnutrition by simply using an integrated medical information system.

8.The Association Between Degenerative Arthritis of the Lumbar Spine and Obesity

Degenerative arthritis of the lumbar spine is one of the main causes of the low back pain over the fifth decade of life. Nowadays, it seems that the number of obese person increases gradually and the role of Obesity in the development of articular degeneration remains controversial. The authors studied 100 cases of the primary degenerative arthritis of the lumbar spine clinically and statistically and compared 50 cases among these with 2 control groups, with and without low back pain, from Mar. 1982 to Sep. 1982. The results were as follows: 1. Among the patients with the degenerative arthritis of the lumbar spine, the ratio of male to female was 1:4, and the age group with the highest frequency was the 6th decade (47%) 2. 44% was obese in the arthritis group and 26% in the control group with low back pain and 12% in the control group without low back pain. 3. The relative risk of the hypothesis that the arthritic patient is obeser than the patient with low back pain but without degenerative changes on X-ray was 2.3, but the hyposthesis was proved to be statistically insignificant. The relative risk of the hypothesis that the arthriticpatient was obeser than the patient without low back pain and degenerative changes on X-ray was 6.3 and proved to be statistically significant.

10.Clinical and Histopathological Observation in 34 Patients with Dermatofibroma.

We observed clinical and microscopic features in 34 patients with dermatofibroma. Of these features, peculiar epidermal changes were observed in six (17.6%) cases. These were keratoacanthosis(1 case), acanthosis nigricans-like(2 cases), pseudoepitheliomatous hyperplasia.(1 case), hair follicle-like basal cell proliferation(1 case) and early basalioma-like(1 case) changes.
