1.Are There Clinical Factors Relating to Penile Pain associated with Intracavernous Injection of Prostaglandin E1 in Patients with Erectile Dysfunction?.

PURPOSE: We performed this study to identify the factors relating to penile pain after intracavernosal injection of prostaglandin E1. MATERIALS AND METHODS: We studied 116 patients with erectile dysfunction, who were evaluated by color duplex ultrasonography after intracavernosal injection of prostaglandin E1. Flow parameters and several clinical factors including age, the presence of vascular risk factors, and the severity or duration of erectile dysfunction were compared according to the degree of pain after intracavernosal injection. RESULTS: Most of patients (79% of total) receiving intracavernosal injections felt penile pain, and severe pain interfering with sexual activity was reported by 27% of patients. We could not find any clinical factor that correlated with the pain, nor was there a consistent association of pain with any particular hemodynamic parameter of the penis (p>0.05). CONCLUSIONS: The pain developing after intracavernosal injection of prostaglandin E1 might be related to individual tissue or receptor properties of the penile cavernous tissue.

2.Clinical Results of Silicone Intubation for Epiphora Patients.

In this study, silicone intubation was performed for punctal stenosis in 26 eyes (group 1), incomplete nasolacrimal duct obstruction in 65 eyes (group 2), and complete nasolacrimal duct obstruction in 62 eyes (group 3).Follow up period was 3~14 months (mean 6 months).After intubation, 22 eyes (84.6%) in group 1, 50 eyes (76.9%)in group 2, and 25 eyes (40.3%)in group 3 showed no epiphora.The success rates based on age were 100%under 10 years, 67.2%between 10 years and 50 years, and 65.4%above 50 years.81.8%in male patients and 58.3%in female patients showed improvement.The most common complication was itching sensation.In summary, silicone intubation can be an alternative to dacryocystorhinostomy in patients who have punctal stenosis and incomplete obstruction of nasolacrimal duct system.

3.The Potency of Mivacurium during Halothane or Enflurane Anesthesia in Infants and Preschool Children.

BACKGROUND: The dose-responses of neuromuscular blocking agents may be influenced by many factors including age and inhalation anesthetics. This study was designed to determine the dose-response relationships of a new, short-acting muscle relaxant, mivacurium during nitrous oxide-halothane or nitrous oxide-enflurane anesthesia in two age groups, infants and 1 to 6 years old preschool children. METHODS: Neuromuscular blockade was monitored by recording the accelerographic activity of the adductor pollicis muscle resulting from supramaximal stimulation at the ulnar nerve at 2 Hz for 2 seconds at 10-second intervals. To estimate dose-response relationships, 24 infants or children of two anesthetic subgroups for each age group received single bolus doses of 45~100 g/kg of mivacurium. The ED50 and ED95 were estimated from linear regression plots of log-dose vs probit of twitch depression. The lag time, onset time and maximal depression of twitch height for the selective medium dose were mesured. RESULTS: The ED50 and ED95 for the infants group were 38.2 and 53.3 g/kg during halothane anesthesia, and 29.8 and 48.6 g/kg during enflurane anesthesia, respectively. And, those for preschool children group were 49.4 and 90.7 g/kg during halothane anesthesia, and 32.3 and 81.4 g/kg during enflurane anesthesia, respectively. There was a parallelism of the dose-response curve between halothane and enflurane anesthesia in either age group. Also, there was statistically significant difference in the maximal twitch depression for the selective medium dose of mivacurium between halothane and enflurane anesthesia in either group. CONCLUSIONS: The potency of mivacurium during enflurane anesthesia is higher than that during halothane anesthesia in infants and preschool children, and during either inhalation anesthesia the dose of mivacurium is less required in infants than preschool children.

4.An Experimental Study on the Role of Blood Vessels in the Formation of Peritumoral Abnormal MR Signal Intensity.

PURPOSE: To assess the role of blood vessels in the formation of peritumoral abnormal signal intensity which exaggerates the size of malignant tumor on MR images. MATERIALS AND METHODS: We performed MR-microangiographic-pathologic correlation using implanted VX-2 carcinoma in 16 rabbit thighs 1-28 days after tumor implantation. The shape and distribution of abnormal vessels were analyzed on microangiography and on histologic examination in correlation with peritumoral abnormal signal intensity on MR images. RESULTS: Dilated peritumoral blood vessels gave rise to irregular, tortuous tumor vessels penetrated into the tumor. With the tumor growth, hypervascular tumor vessels in peritumoral area and central avascular areas were increased. These hypervascular areas on microangiography were corresponded with abnormal signal intensity on MR images. CONCLUSION: Hypervascularity could be a cause of peritumoral abnormal signal intensity which exaggerates the size of experimentally induced malignant musculoskeletal tumors on MR images.

5.Transphyseal extension of osteosarcoma: MRI and pathologic correlation.

We retrospectively analysed plain radiographs and MR imaging of 16 conventional osteosarcoma cases(8 children, 8 adults) which underwent amputations or limb salvage operations. Pathologic correlation was performed with gross and microscopic sections to evaluate 1) whether the open epiphyseal plate can function as a barrier against transphyseal spread of osteosarcoma and 2) the diagnostic value of MR imaging for the detection of the detection of the epiphyseal involvement of osteosarcoma. In children with open epiphyseal plates, conventional radiographs suggested transphyseal tumor growth in one of eight cases(12.5% and MR imaging in seven cases(87.5%). Pathologic examination confirmed epiphyseal involvement in six of seven cases noted with MR imaging(75%). On the the other hand, in adult patients with closed epiphyseal plates, conventional radiographs showed transphyseal tumor growth in six of eight cases(75%), while MR imaging and pathologic exam demonstrated tumor invasion in all cases(100%). We conclude that open epiphyseal plate does not function as and effective barrier against tumor extension, and MR imaging is an excellent method in detecting the extent of transphyseal tumor growth.

6.A Comparison of Fibrillatory F Wave with Left Atrial Size and Biopsy Findings in Rheumatic Valvular Heart Disease.

A total of the 16 cases of rheumatic vavular heart desease were examined in order to compare fibrillatriy F wave with left atrial size and biopsy findings from October 1986 to April 1987 at the Ewha Womans University Hospital. The results were as follows : 1) Of the 16 patients, there were 5 males and 11 females. The mean age of the patients was 41.9+/-12.7 years old. 2) Among the 16 patients PTF-V1 was lesser than -0.04nn-sec, the left atrial size measured by M-mode echocardiography was 57.1+/-8.9mm. Inverse correlation between PTF-V1 and the left atrial size measured by M-mode echocardiography was observed(P<0.01). 3) No significant correlation between PTF-V1 and left atrial was found(P>0.01). 4) There was a tendency that the size of the left atrium was, the more degeneration, necrosis and fibrosis of the left atrial wall appeared through pathological findings.

9.SPECTROPHOTOMETRIC ANALYSIS OF THE EFFECT OF TOOTHBRUSHING ON EXTERNALLY STAINED IPS EMPRESS PORCELAIN.

The purpose of this study was to evaluate the long term tooth-brushing effect on the color change of specially treated IPS Empress porcelain surface. Staining technique with blue stain and liquid was used. The surfaces of the specimen were treated with 5% and 10% Hydrofluoric acid, 50mm and 250mm alumina sandblast, and then blue stain and liquid were used for external stain. After 29,200, 58,400, 87,600, 116,200 brushing strokes (equivalent to 2, 4, 6, 8 years each), color changes of the stained layer were measured with spectrophotometer(CM-3500d, MINOLTA, Thkyo, Japan). The result of this study was obtained as follows : 1. The color changes were great after 29,200 tooth brushing strokes in every group, but from 29,200 strokes to 116,800 strokes, there were no significant color changes in each group.(p>0.05) 2. The greatest color changes were observed in 5% HF treated group and the least color changes were observed in 50 micrometer aluminar sandblast treated group in every stroke(p<0.05), but no ststistical difference between groups compared with control group(p>0.05). According to these results, pretreatment of I.P.S. Empress porcelain with alumina sandblast improved the color of IPS Empress porcelain significantly on this condition, but long term follow-up will be needed.
