1.Pathogenicitic Characterization of Purified beta-Hemolysin - Produced by Vibrio mimicus.

In order to investigate the main factor of pathogenicity in V. mimicus, we have studied the toxic effects of j3-hemolysin produced by V. mimicus. The purified hemolysin of V. mimicus was active erythrocytes from three animal species including mouse, rabbit and rat, but the hemolysin was most active against rabbit erythrocyte. The hemolysin lysed cultured cell and killed mouse. Rapid death of mouse was observed with rather small doses of the toxin. Intravenous injection of 20 mg of the purified toxin killed mice within 25 sec. The hemolysin also had a lethal effect on intraperitoneal injection into mice although less than on intravenous injection. Purified hemolysin injected rabbits had large morphological change in jejunum. In electron micrograph of thin sections of the human erythrocytes, cells were threated with the hemolysin at 37 C for 5 min., significant changes were not observed. But after 10 min., hemolysis was observed and after 60 min., complete degradation of human erythrocyte was observed.

2.Reproducibility of Anal Manometric Measurement.

PURPOSE: Anorectal manometry has become a routine investigation for the evaluation of patients with anorectal disorders. However, the interpretation of such studies is confounded by the fact that controversial data are reported with regard to the same events. The aim of this study was to measure the reproducibility of the pressure profiles of anorectal manometry in healthy controls for standardization of their measurements. METHODS: Manometric study was performed on different days with 7 days interval in 22 male healthy subjects with the use of a pneumohydraulic capillary perfusion system. RESULTS: For resting pressures with rapid pull-through (RPT) technique, maximum pressure revealed 43.58, 19.8% and mean pressure, 16.02, 14.6% in the order of reproducibility coefficient and coefficient of variation. For resting pressures with stationary pull-through (SPT) technique, maximum pressure; 17.22, 12.2% and mean pressure; 14.66, 26.4%, respectively. For squeezing pressures with RPT technique, maximum pressure; 53.37, 14.3% and mean pressure; 66.32, 23.6%. For squeezing pressures with SPT technique, maximum pressure; 72.80, 11.9%, mean pressure; 93.10, 30.5% and coughing pressure; 69.42, 15.8%, respectively, For anal canal length (ACL) with RPT technique, at resting state; 7.10, 7.9% and at squeezing state; 14.55, 13.7%, respectively. For high pressure zone (HPZ) with RPT technique, at resting state; 6.68, 16.3% and at squeezing state; 11.06, 23.5%. For HPZ with SPT technique, at resting state; 11.28, 25.4% and at squeezing state; 10.04, 17.5%, respectively. For radial asymmetry (RA) with RPT technique, at resting state; 13.76, 42.3% and at squeezing state; 7.86, 22.9%. For RA with SPT technique, at resting state; 24.6, 58.6% and at squeezing state; 14.28, 46.7%, respectively. CONCLUSION: Measurements of resting and squeezing pressure are more reproducible by SPT technique, in which technique it seems that maximum value is more preferred as a representative value. ACL and HPZ are may well measured on resting pressure with SPT technique. Radial asymmetry shows the best reproducibility on squeezing pressure with RPT.

4.The Type of Diseases in Elderly Patients.

BACKGROUND: It is a recent trend that the birth and mortality rates has been decreased and average life span prolonged in proportion to the socioeconomic and scientific advances. The goal of this study is to suggest basic data for comprehensive and continuous management of elderly patients. METHODS: Based upon ICD-10 (International Classification of Disease), the author analysed the outpatient's diseases 19,367 and inpatient's diseases 6,783 beyond the age of 65 who visited outpatient department and were hospitalized to the Kosin University, Kosin Medical Center in Pusan from January to December in 1997 RESULTS: The ratio of elderly patient in outpatient was 4.5% and inpatient was 15.0%. The number of outpatient each disease group were disease of the circulatory system 5,367 (27.7%), neoplasms 4,007 (20.7%), disease of the digestive system 2,635 (13.6%), endocrine, nutritional and metabolic disease 1,798 (9.8%) in order. Frequent diseases in individual system of outpatient were followed; malignant neoplasms of digestive organs 2,642 (13.6%), hypertensive diseases 2,053 (10.6%), diseases of esophagus, stomach and duodenum 1,804 (9.3%), ischemic heart disease 1,101 (5.7%) in order. The number of inpatient each disease group were disease of the neoplasms 2,138 (31.5%), disease of circulatory system 1,191 (17.6%), disease of the digestive system 730 (10.8%), endocrine, nutritional and metabolic disease 489 (7.2%) in order. Frequent diseases in individual system of inpatient were followed; malignant neoplasms of digestive organs 956 (14.1%), malignant neoplasms of respiratory and intrathoracic organs 351 (5.2%), hypertensive diseases 348 (4.2%), diabetes mellitus 325 (4.1%) in older. CONCLUSION: This result indicate that the elderly patients abounded mostly with disease of circulatory, neoplasm, disease of the digestive system and endocrine, nutritional and metabolic disease.

5.The Multiplex RT-PCR for the Serotyping of Hantaviruses.

Hantaviruses are negative-strand RNA viruses that contain three segmented genome and belong to the family Bunyaviridae. Due to such an unique structure of segmented RNA genome, these viruses have a possibility to produce reassortant that have genomic sets mixed with different segments originated from both parental virus during the genetic interaction. To investigate whether this phenomenon occurs actually, Hantaan virus (HTN) could be coinfected with Seoul virus (SEO) to Vero-E6 cell. And also, HTN could be coinfected with Prospect hill virus (PH) to investigate the rate of genetic reassortment according to the genetic distance among the HTN, SEO and PH viruses. As a available method to differentiate the reassortant, we used the multiplex RT-PCR that detect and differentiate the serotype of hantaviruses in mixed infection. Each progeny clone could be screened by multiplex RT-PCR. So, we have constructed the multiplex RT-PCR system that separated amplifications for each segment of Hantaviruses. Our multiplex RT-PCR system provide sensitive, specific and simplified tools for the rapid differentiation of hantaviruses serotypes and also the diagnosis of Hantavirus infections.

7.In vitro Transcription of Prospect Hill virus S Genome Segment Produced by RT-PCR.

Prospect Hill virus (PHV) is related antigenically but distinct to Hantaan virus. As a member of genus Hantavirus, PHV has three segmented RNA genome. Among these segments, Small segment(S) encodes nucleocapsid protein (NP) as structural protein and also may do functional nonstructural protein(NSs). We performed in vitro transcription to produce vRNA of PHV S genome. For the first step of in vitro transcription of S genome of PHV, the S RNA segrnent which is 1,675 nucleotides long was amplified by RT-PCR using PCR primers built according to cDNA sequence of PHV S genome. Next, a new PCR primer appended above downstream primer to T7 phage promoter sequence was reconstructed to obtain PCR product containing T7 promoter. Then another PCR was performed. Using this PCR product as the template, in vitro run-off transcription without cloning by transcriptional vector was performed to obtain viral- sense RNA transcript. Thereafter, the size of transcript was assessed by running on formaldehyde agarose gels. Since the transcription reactants contain a-S UTP, the transcript is detectable by autoradiography. The transcript was also detectable by northern hybridization with a-P dCTP- labelled PHV amplicon probe (319 bp) and the initiation start point of run-off transcription was also determined by primer extention analysis. Our data indicate that the in vitro transcript could be produced from the PCR product amplified by PCR primer containing T7 phage promoter without cloning into a phage transcription vector.

Hydranencephaly is congenital absence of the cerebral hemispheres which are replaced by a large fluid-filled cavity. The brain stem and basal ganglia are well formed and rudiments of frontal k occipital cortex may be present. We experienced a case of hydranencephaly caused by both internal carotid artery stenosis. We diagnosed it through the brain CT sonogram and doppler sonogram. A brief review of the related literatures was made.
