1.Clinical Utility of Limited T2-Weighted-Only Lumbar Spine MRI in Pain Intervention Clinics

Purpose:
This study aimed to determine whether limited T2-weighted-only lumbar spine MRI is clinically available to patients visiting pain intervention clinic for radicular pain.
Materials and Methods:
A total of 110 patients (mean age: 63.5 ± 16.1 years) with low back pain (LBP) or sciatica who underwent limited T2-weighted-only lumbar MRI using T2-weighted sagittal and axial sequences were enrolled. All MRI scans were reviewed by two radiologists for degenerative spinal lesions that most correlated with each patient’s symptoms. All patients received spinal injections. Treatment response was recorded based on patients’ subjective evaluations and an 11-point numerical rating scale. Clinical characteristics and changes in the diagnosis of patients who had undergone previous MRI or additional MRI within six months were also analyzed.
Results:
Using limited T2-weighted-only lumbar spine MRI, acquisition time and cost for imaging examinations were effectively reduced. Pathologic lesions corresponding to clinical symptoms were detected in most (95%) patients. Symptoms had improved in 78.1% of patients at the 6-month follow-up. Further imaging work-up was not performed for any subject during the study period because there was clear indication (no doubt) for other diagnoses on the limited T2-weighted-only lumbar spine MRI or during the clinical follow-up.
Conclusion
Limited T2-weighted-only lumbar spine MRI comprising T2-weighted sagittal and axial images may sufficiently detect degenerative changes of the lumbar spine in patients with LBP and sciatica. Treatment responses are satisfactory, effectively reducing scan time and costs.
2.Investigating Effects of Nano- to Micro-Ampere Alternating Current Stimulation on Trichophyton rubrum Growth.

BACKGROUND: Fungi are eukaryotic microorganisms including yeast and molds. Many studies have focused on modifying bacterial growth, but few on fungal growth. Microcurrent electricity may stimulate fungal growth. OBJECTIVE: This study aims to investigate effects of microcurrent electric stimulation on Trichophyton rubrum growth. METHODS: Standard-sized inoculums of T. rubrum derived from a spore suspension were applied to potato dextrose cornmeal agar (PDACC) plates, gently withdrawn with a sterile pipette, and were applied to twelve PDACC plates with a sterile spreader. Twelve Petri dishes were divided into four groups. The given amperage of electric current was 500 nA, 2 µA, and 4 µA in groups A, B, and C, respectively. No electric current was given in group D. RESULTS: In the first 48 hours, colonies only appeared in groups A and B (500 nA and 2 µA exposure). Colonies in group A (500 nA) were denser. Group C (4 µA) plates showed a barely visible film of fungus after 96 hours of incubation. Fungal growth became visible after 144 hours in the control group. CONCLUSION: Lower intensities of electric current caused faster fungal growth within the amperage range used in this study. Based on these results, further studies with a larger sample size, various fungal species, and various intensities of electric stimulation should be conducted.

3.Nipple Suspension Using Biopsy Bottle after Surgical Correction for Inverted Nipple.

Inverted nipple presents both cosmetic and functional problems. Various surgical treatments have been introduced to correct the deformity, but frequently ended up with recurrences. To reduce the recurrence rate, stable suspension is required during wound healing period after surgical correction. Authors present a new suspension device using a biopsy bottle. To apply this device, first of all, the retracted nipple is held with 2 traction sutures using 4-0 nylon, and then the surgical correction is performed. After the correction is completed, the traction sutures are tied to bottom of biopsy bottle through the previously prepared holes. The biopsy bottle covers areolar and nipples complex with suspension of traction sutures. The suspension goes on for 1 week. From August of 1997 to December of 2002, we corrected inverted nipples in 75 patients. After operation, we applied these devices to all nipples for suspension. 54 cases of corrected nipples in 58 cases, which could be followed up, were kept corrected for more than 6 months. The authors feel that this idea is not only useful but also provides a simple, economical and efficient to suspend nipple after surgery.

4.Related Factors of Depression according to Individual Attributes and Regional Environment: Using Multi-Level Analysis

Background:
This study is aimed to verify individual and regional-level factors affecting the depression of Koreans and to develop social programs for improving the depressive status.
Methods:
This study used individual-level variables from the Korean Community Health Survey (2018) and used the e-regional index of the Korean Statistical Information Service as the regional-level variable. A multi-level logistic regression was executed to identify individual and regional-level variables that were expected to affect the extent of depressive symptoms and to draw the receiver operating characteristic curve to compare the volume of impact between variables from both levels.
Results:
The results of the multi-level logistic regression analysis in regards to individual-level factors showed that older age, female gender, a lower income level, a lower education level, not having a spouse, the practice of walking, the consumption of breakfast higher levels of stress, and having high blood pressure or diabetes were associated with a greater increase in depressive symptoms. In terms of regional factors, areas with fewer cultural facilities and fewer car registration had higher levels of depressive symptoms.The comparison of area under the curve showed that individual factors had a greater influence than regional factors.
Conclusion
This study showed that while both, individual and regional-level factors affect depression, the influence of the latter was relatively weaker as compared to the first. In this sense, it is necessary to develop programs focused on the individual, such as social prescribing at the local or community-level, rather than the city and nation-level approach that are currently prevalent.
5.False-positive Intracranial Aneurysms on MR Angiography: Incidence and Causes.

PURPOSE: The purpose of this study is to analyze the incidence and causes of false-positive intracranial aneurysms on MR angiography (MRA). MATERIALS AND METHODS: We retrospectively evaluated 210 cases that were incidentally diagnosed as intracranial aneurysms on MRA. All the cases were confirmed by CT angiography (CTA) or digital subtraction angiography (DSA). We investigated the incidence, location, size, and causes of the false-positive aneurysms on MRA. RESULTS: Of 210 cases, 28 cases were confirmed as false-positive aneurysms. Most of the lesions were located on the middle cerebral artery (n = 9, 7 lesions located in the bifurcation and 2 lesions located in the trunk) and the anterior communicating artery (n = 5). The diameters of most of the lesions (78.6%) were smaller than 3 mm. Causes were junctional dilatation (n = 14), vascular tortuosity (n = 6), normal variation (n = 3) and unknown causes (n = 3). CONCLUSION: Understanding the risks and pitfalls of false-positive intracranial aneurysms determined on MRA would be helpful for improving the diagnostic accuracy of screening MRA and for reducing the number of unnecessary invasive examinations.

6.Novel Anti-Angiogenic and Anti-Tumour Activities of the N-Terminal Domain of NOEY2 via Binding to VEGFR-2 in Ovarian Cancer

The imprinted tumour suppressor NOEY2 is downregulated in various cancer types, including ovarian cancers. Recent data suggest that NOEY2 plays an essential role in regulating the cell cycle, angiogenesis and autophagy in tumorigenesis. However, its detailed molecular function and mechanisms in ovarian tumours remain unclear. In this report, we initially demonstrated the inhibitory effect of NOEY2 on tumour growth by utilising a xenograft tumour model. NOEY2 attenuated the cell growth approximately fourfold and significantly reduced tumour vascularity. NOEY2 inhibited the phosphorylation of the signalling components downstream of phosphatidylinositol-3’-kinase (PI3K), including phosphoinositide-dependent protein kinase 1 (PDK-1), tuberous sclerosis complex 2 (TSC-2) and p70 ribosomal protein S6 kinase (p70S6K), during ovarian tumour progression via direct binding to vascular endothelial growth factor receptor-2 (VEGFR-2). Particularly, the N-terminal domain of NOEY2 (NOEY2-N) had a potent anti-angiogenic activity and dramatically downregulated VEGF and hypoxia-inducible factor-1α (HIF-1α), key regulators of angiogenesis. Since no X-ray or nuclear magnetic resonance structures is available for NOEY2, we constructed the threedimensional structure of this protein via molecular modelling methods, such as homology modelling and molecular dynamic simulations. Thereby, Lys15 and Arg16 appeared as key residues in the N-terminal domain. We also found that NOEY2-N acts as a potent inhibitor of tumorigenesis and angiogenesis. These findings provide convincing evidence that NOEY2-N regulates endothelial cell function and angiogenesis by interrupting the VEGFR-2/PDK-1/GSK-3β signal transduction and thus strongly suggest that NOEY2-N might serve as a novel anti-tumour and anti-angiogenic agent against many diseases, including ovarian cancer.

8.Significance of Clopidogrel Resistance Related to the Stent-Assisted Angioplasty in Patients with Atherosclerotic Cerebrovascular Disease.

OBJECTIVE: To evaluate the prevalence and risk factors of clopidogrel resistance, and association between thromboembolic complications and clopidogrel resistance in patient with stent-assisted angioplasty for atherosclerotic cerebrovascular disease. METHODS: Between September 2006 and June 2008, clopidogrel resistance test was performed on 41 patients who underwent stent-assisted angioplasty for atherosclerotic cerebrovascular disease. It was performed before drug administration and about 12 hours after drug administration (loading dose : 300 mg, maintain dose : 75 mg). Two patients were excluded, and 41 patients were included (mean : 67.59+/-7.10 years, age range : 41-79). Among 41 patients, 18 patients had intracranial lesions, and 23 had extracranial lesions. We evaluated the prevalence, risk factors and complications related to clopidogrel resistance. RESULTS: Twenty-one patients (51.2%) showed clopidogrel resistance [intracranial : 10 patients (55.6%), extracranial : 11 patients (47.8%)] and no clopidogrel resistance was seen in 20 patients. Hypercholesterolemia was an indepedent risk factor of clopidogrel resistance. Stent-assisted angioplasty was technically successful in all patients, but acute in-stent thrombosis occurred in 5 patients with intracranial lesions (4 patients with clopidogrel resistance and 1 without clopidogrel resistance). Acute thrombi were completely lysed after intra-arterial infusion of abciximab. CONCLUSION: There was relatively high prevalence of clopidogrel resistance in patients with atherosclerotic cerebrovascular disease. Hypercholesterolemia was an independent predictive factor of clopidogrel resistance. Acute in-stent thrombosis was more frequently seen in the clopidogrel resistant group. Therefore, clopidogrel resistance test should be performed to avoid thromboembolic complications related to stent-assisted angioplasty for atherosclerotic cerebrovascular disease, especially patients with hypercholeterolemia and intracranial lesion.

9.Single Center Experience With Hyperthermic Intraperitoneal Chemotherapy.

PURPOSE: Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) have been proposed for controlling peritoneal seeding metastasis in some kinds of cancers, including those of colorectal origin, but their safety and oncological benefits are subjects of debate. We present our early experience with those procedures. METHODS: Data were retrospectively collected from all patients with peritoneal carcinomatosis (PC) and pseudomyxoma peritonei (PMP) treated using CRS and HIPEC at Yonsei Cancer Center between July 2014 and July 2015. Short-term outcomes and risk factors for postoperative complications were analyzed. RESULTS: Twenty-three patients with PC (n = 18) and PMP (n = 5) underwent CRS and HIPEC. Median follow-up and age were 2 months and 54 years, respectively. The median peritoneal carcinomatosis index score was 15, and CC0-1 was achieved in 78.3% of all patients. The median operation time and bleeding loss were 590 minutes and 570 mL, respectively. Grade-IIIa/grade-IIIb complications occurred in 4.3% (n = 1)/26.1% (n = 6) of the patients within 30 days postoperatively, and no 30-day mortalities were reported. Factors related to postoperative complications with CRS and HIPEC were number of organ resection (P = 0.013), longer operation time (P < 0.001), and amount of blood loss (P = 0.003). All patients treated with cetuximab for recurred colorectal cancer had grade-III postoperative complication. CONCLUSION: Our initial experience with CRS and HIPEC presented about 30% grade-III postoperative complications. Therefore, expert surgeons need to perform those procedures with great caution in selected patients who might benefit from it.

10.A Case of Fibrous Dysplasia Involving the Nasal Septum.

Fibrous dysplasia is a developmental disease of bone in which the normal substance of the interior of the bone is replaced by fibro-osseous connective tissue. It exhibits varying degrees of osseous metaplasia histologically. Craniofacial fibrous dysplasia frequently affects the maxilla, frontal bone, and mandible. The nasal septum is rarely affected. There is only one reported case of fibrous dysplasia arising in the septum internationally. Hence, we report a case of fibrous dysplasia of the nasal septum with a review of literature.
