1.The Affinity of Calmodulin-Affigel for Inositol Triphosphate Kinase From Bovine Brain.

The one event on signaling mechanism is the cleavage by adenyl cyclase of ATP into second messenger, cyclic AMP. The other transfer system of inositol metabolism, it is widely recognized that hydrolysis of the minor membrane lipid phosphoinositide bisphosphate (PIP₂) initiated by occupation of certain receptors and catalyzed by phospholipase C, lead to toe generation of the two intracellular messengers, inositol triphosphate (IP₃) and diacylglycerol (DG). IP₃ is converted to inositol tetrakisphosphate (IP₄) by IP₃ kinase. In the present study, it is that purification of calmodulin is used by phenyl-Sepharose CL-4B chromatography, it's molecular weigh, 17,000 in SDS-polyacrylamide gel electrophoresis. In order to observe the affinity between calmodulin (CaM)-Affigel 15 and IP₃ kinase, and isolated IP₃ kinase, was applied in CaM-Affigel with Ca²⁺ equilibrium buffer and EGTA equilibrium buffer. We compared with binding and elution effect of IP₃ kinase in several condition of buffer. In affinity of binding, Ca²⁺ equilibrium buffer was in the most proper condition, and elution, CaM/Ca²⁺buffer (CE 1 10.36, CE2 12.76pM/min/mg of protein) was effected much more than EGTA buffer (E2 1.48, E 2.43pM/min/mg of protein), but CaM/Ca²⁺stimulate the activity of IP₃ kinase. And then, several detergents such as sodium deoxycholate, tween 20, cholic acid, polyethylene glycol, chaps were applied. The 0.2% chaps buffer (E2 23.19, E3 8.05pnM/min/mg of protein) was the most effective in elution of IP3 kinase.

2.The Diagnosis Distribution in Health Examination of Old Aged People and Associated Sociodemographic Factors.

BACKGROUND: According as old aged people have an increased interest in early detection of disease and health promotion, old aged people who take health examinations are increased. So this study is aimed to survey common problems about geriatric health in community and their frequency by way of investigation on diagnosis in health examination data and to evaluate the effects of health risk factors on each disease patterns. METHODS: The medical records of the clients who have taken health examinations for the first time from March 1995 to February 2000 at a health examination center of a university hospital in Seoul were reviewed. The category and number of judged diagnosis, sociodemographic factors are compared and analyzed. RESULTS: The selected medical records accounted to 22,393. The study subjects consist of 11,544(51.6%) male. Mean age is 52.0+/-10.5 years(range 16~90). 65 years or more old aged people of them are 2,612(11.7%), the number of judged diagnosis per one old person is 4.84+/-2.22(range 0~9) and young people took less diagnosis is 35.7%, functional diagnosis is 7.9%. The common diagnoses are liver disease(7.07%), decreased physical strength(6.68%), hyperlipidemin(6.53%),obesity(5.90%), osteoporosis or postmenopausal syndrome(4.72%), need for immunization against viral hepatitis(4.69%), hypertension(4.20%) functional gastrointestinal disease(3.93%). The number of total diagnosis and organic diagnosis is larger in older age, no exercise, female,smoker, no-spouse, lower education(p<0.05). The number of other diagnosis is larger in no exercise, smoker, female, younger age(p<0.05). CONCLUSION: According to health examination, total diagnosis and organic diagnosis are more in old aged group than in young. Functioinal diagnosis and other diagnosis is less in old aged group than in young.

3.A Case of Tubular Apocrine Adenoma.

Tubular apocrine adenoma(TAA) is a rare benign neoplasm usually found on the scalp. It has been designated as an apocrine histogenesis on the basis of its ultrastructural characteristics, enzyme, and immunohistochemical phenotype. Histopathologically the neoplasm consists of tubules or cysts, which show signs of apocrine secretion and sometimes needs to be differentiated from papillary eccrine adenoma. We report a typical case of TAA on nostril which shows differentiation toward apocrine in nature.

4.Rupture of Renal Artery in a Patient with Behçet's Disease.

The vascular involvement is seen in 8% to 24% of patients with Behçet's syndrome. Arterial lesions are less frequently observed and the involvement of renal artery is very rare. Indeed, there is only one case report of ruptured renal aneurysm due to Behc et's disease. We report a case of renal artery rupture, which was likely caused by Behçet's disease. Even though we cannot completely rule out the other causes of renal artery rupture with this patient, the rupture was likely due to vascular involvement of Behçet's disease. Dermatologists always should consider the possibility of major vessel involvement when they treat patients with Behçet's disease, even if the incidence is very low and the patient may not complain of any systemic symptoms. INTRODUCTION: Behçet's disease was first defined by Hulusi Behçet's in 1937 as a multisystemic disease with the triad of recurrent aphthous stomatitis, genital ulceration and relapsing iritis. Its etiology remains obscure. It is accepted that the pathologic process of Behçet's disease is an immunologic vasculitis. Among vascilar lesions, the venous system is the major affected site. Arterial lesions, such as aneurysms or occlusions, are rarely reported, but sometimes adversely affects the course of the disease. Aneurysm formation is known to be life-threatening because of the risk of rupture.

5.Spinal Fluid Cytology of Retinoblastoma.

Of all the primary central nervous system tumors, the medulloblastoma, glioblastoma multiforme, ependymoma and pineal germinoma tend to exfoliate in the cerebrospinal space. With all other types of the tumor, abnormal cells may seldom be definitely identified in the cerebrospinal fluid. Up to now the tumor cells have been rarely found in CSF cases of retinoblastoma. We have experienced a case of advanced retinoblastoma that showed exfoliated cells in spinal fluid.

7.Squamous Cell Carcinoma Arising from Chronic Osteomyelitis: A report of four Cases

Squamous cell carcinoma from the draining sinus of chronic osteomylitis has been recognized as a rare complication. The incidence of this complication is between 0.23% and 1.6% in cases of osteomyelitis. The cause of development of carcinoma at the site of chronic osteomyelitis is not well understood. This is a disease of middle aged men, and tibia is the most common site. Biopsy of squamous cell carcinoma which arises in the proliferating edge of the cutaneous ulcer and invades the bone, should include tissues from all sites of ulcer and bone marrow spaces. Histological diagnosis may be difficult because of preexisting metaplasia and pseudoepitheliomatous hyperplasia. Amputation at the adequate level is the treatment of choice. Four cases of squamous cell carcinoma involving tibia, metatarsal bone or calcaneus are reported with review of literatures.

8.Psychiatric Diagnosis and Hypersexual Behavior Inventory, Sexual Addiction Screening Test, and Beck Depression Inventory/Beck Anxiety Inventory of the Sexual Offenders.

The current study evaluated DSM-IV psychiatric diagnoses and investigated the psychological characteristics of sexual offenders by using the Hypersexual Behavior Inventory(HBI), Sexual Addiction Screening Test (SAST), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI). Fifty male sexual offenders and 36 male healthy controls completed the psychiatric diagnosis and 30 of the 50 male sexual offenders completed the psychological tests listed above. Thirty-two of the 50 sexual offenders were patients with paraphilias, these sexual offenders had been admitted by the Ministry of Justice to the Institute for Forensic Psychiatry in Gongjui-si due to sexual violence. Participants displayed high lifetime rates of psychiatric disorders: 32 (64%) had paraphilias, 16 (32%) had depressive disorders, 10 (20%) had anxiety disorders, 9 (18%) had impulse control disorders, 6 (12%) had schizophrenia, 2 (4%) had bipolar disorders, and 16 (32%) had personality disorders or some other Axis II disorder. Scores for the sexual offenders were significantly higher both on the HBI and the SAST than the comparison group. Scores for the sexual offenders were also higher for the BDI and the BAI as compared to the control group, but this difference was not statistically significant. Results indicate that sexual offenders may have sexual addictions and be hypersexual. In addition to depressive and anxious psychopathologies, they also seem to have sexual psychopathologies. These results suggested that the psychopathology of sexual offenders may be different than that of a control group.

9.Clinical Comparison of Neonatal Urinary Tract Infection Caused by Klebsiella pneumoniae Versus Non - klebsiella pneumoniae.

PURPOSE: The purpose of this study was to describe the clinical characteristics of neonatal urinary tract infection (UTI) caused by Klebsiella pneumoniae and non- Klebsiella pneumoniae UTI. METHODS: We compared clinical characteristics of 84 neonatal patients with UTI caused by Klebsiella pneumoniae who were hospitalized at the Department of Pediatricsat Han Dong University, Sunlin Hospital during the period between May, 1994 and August, 1998. The cases were divided into two groups depending upon causative organisms' Klebsiella pneumoniae UTI vs non-Klebsiella pneumoniae UTI, and the clinical characteristics of these groups were compared. RESULTS: Escherichia coli was the most common bacterial pathogen causing neonatal UTI, followed by Klebsiella pneumoniae. There was no significant difference in the sex distribution of Klebsiella pneumoniae UTI, but non-Klebsiella pneumoniae UTI showed male predominence. There were no significant differences in the incidences of hematologic, urologic, radiologic findings and perinatal complications in between these 2 groups. CONCLUSION: Klebsiella pneumoniae is the second most common pathogen causing neonatal UTI. There were no specific differences in the laboratory, symptomatologic, and radiologic findings in these two groups.

The possible complications after parotidectomy are facial nerve palsy, Frey's syndrome, sialocele, salivary fistula, hematoma infection, and flap necrosis. Postoperative sialocele and salivary fistula are relatively uncommon and the management varies. We experienced a rare postoperative complication of excessive and prolonged drainage of saliva after superficial parotidectomy and removal of an necessary parotid gland tumor. The patient could be treated successfully with conservative management such as compressive dressing, restriction of oral intake of irritant food, maintenance of oral hygiene and the use of amitriptyline(antidepressant).
