2.Two Cases of Verrucous Carcinoma of the Larynx.

Verrucous carcinoma arising from the larynx is a specific clinical entity which is unfamiliar to many otolaryngologists. As judged by reported cases, the mucous membranes of the head and neck are sites of predilection. This lesion makes up approximately 1 to 4% of all laryngeal carcinoma. The biologic behavior of verrucous carcinoma of the larynx is characterized by warty appearance, slow growth, local invasion, scarcity of metastases and benign histology. Most authors prefer surgical treatment for patients with verrucous carcinoma. Radiation therapy appears to hold a higher risk of recurrence, anaplastic transformation of tumor. Recently, the authors have experienced two cases of verucous carcinoma originated from the larynx in old male patients. The two patients were treated by surgical excision.

3.Episodic Central Neurogenic Hyperventilation in an Awake Patient with Unilateral Pontine Infarction.

Central neurogenic hyperventilation (CNH) is characterized by sustained tachypnea inspite of an elevated arterial PaO2, pH and a low arterial PaCO2. CNH is common in patients with brainstem injury accompanied by a decreased level of consciousness but this also has been described in some alert patients with an invasive brainstem tumor. We report one case with CNH resulting from a unilateral pontine infarction. His consciousness was clear, and CNH spontaneously disappeared without any medication.

4.The Operative Treatment of Radial Head or Neck Fracture: The Sub-classification of Mason Type II Fracture.

PURPOSE: To evaluate the results of treatment according to the sub-classification of the Mason type II fracture. MATERIALS AND METHODS: From 1999 to 2003, according to the sub-classification of the Mason type II fracture, 33 patients were treated with miniplate in displaced neck fracture (IIa), with compression screw in displaced head fracture (IIb), with miniplate and/or compression screw in displaced head and neck fracture (IIc), with compression screw and miniplate in comminution fracture (III) or excision of head in irreducible state. The clinical results were evaluated by An and Morrey's functional rating index. RESULTS: Functional rate score averaged 92.7 in type IIa, 88.4 in IIb, 86.4 in IIc, 83.5 in type III with reduced fracture, 75.0 in type III with excised head, and 75.5 in type IV. Complications included heterotopic ossification (2 cases), metal loosening (1 case), malunion (1 case), partial ankylosis of elbow (3 cases), posttraumatic arthritis (1 case). CONCLUSION: These results supported the recommendation for internal fixation with compression screw in isolated radial head fracture (IIb) and with miniplate in fracuture combined with displaced neck (IIa, IIc, indicated some III). We concluded that sub-classification is useful for dicision making in radial head or neck fracture's treatment.

6.Effect of Cooling and Rewarming on Muc Gene and Interleukin 8 Expression in Respiratory Epithelial Cells.

PURPOSE: Cooling and rewarming have been described to contribute to the pathogenesis of exercise induced asthma. However, little is known about the cellular response to cooling and rewarming of respiratory epithelial cells. Hypersecretion of mucus and allergic inflammation are important pathologic finding of patients who suffered from asthma. We investigated whether cooling and rewarming of respiratory epithelial cells induce mucin gene (MUC5AC, MUC5B) expression and IL-8 production. METHODS: NCI-H292 (human lung mucoepidermoid carcinoma cell line) cells were cultured in 6 well plates. Experimental groups were preserved at 1degree C, 4degrees C, 18degrees C and control groups at 37degrees C for 2 hours. And then both group were kept at 37degrees C. MUC5AC, MUC5B and IL-8 mRNA expressions were examined by RT-PCR. IL-8 concentration in the cell culture medium after rewarming was measured by ELISA. RESULTS: Cooling and cooling-rewarming stimuli did not increase MUC5AC and MUC5B expression. IL-8 concentration was remarkably decreased in experimental groups after cooling and then markedly increased during first 6 hours. IL-8 concentration of 1degrees C, 4degrees Cgroups were significantly increased compared to control group at 6 hour, of 18degrees C group at 12 hour and then persisted until 24 hour. CONCLUSION: Cooling and rewarming stimuli to respiratory epithelial cells did not increase MUC gene expression. However, increased IL-8 production provides evidence of cooling and rewarming induced airway inflammation. Further investigation will be needed to support this result.

Patients who have repaired cleft lip and palate generally undergo restriction of maxillary growth. Concave facial profile is often exhibited with relatively normalized mandible. Horizontal and sagittal deficiency of the maxilla could cause anterior and posterior crossbites. In growing patients, orthodontic and orthopedic treatment is acceptable with maxillary expansion and protraction. However, surgical approach has to be accompanied with orthodontic treatment in skeletally matured patients. We used SARPE and BSSRO to expand the constricted maxilla and retract the mandible in a patient who had cleft palate repaired in infancy. Through SARPE, orthodontic treatment and BSSRO, we sufficiently expanded the maxillla and improved facial profile.

Hypertensive encephalopathy is a medical disorder, which occurs with sudden increase of blood pressure (BP). The MRI findings of hypertensive encephalopathy are diffuse hyperintensity on T2-weighted images, predominantly within the cortex and subcortical white matter of the parieto-occipital lobe. The brainstem is rarely involved. Diffusion-weighted images do not show any abnormalities. We report two patients with hypertensive encephalopathy whose MRI showed exclusive brainstem involvement. They improved rapidly after BP control.
