1.A Report of the Body Composition and Life Styles in Incheon area : Data Source from the Public Service Event of the Incheon Dietetic Association.

Among the diverse factors causing obesity, the life styles and the food habits are very important. This study has carried out to evaluate the association between the habit of living and the Body Composition in 132 male and 313 female of 20-75 years old people in Incheon. The research tools are questionnaires of 6 items of life styles(Smoking, Drinking, Frequency of exercises, Normal Activity Degree, Favorite food, Degree of salt ingestion) and a height and Body Composition. The statistical results are as follows : 38% of male and 23% of female were over the normal range(Korean Society of Obesity 2004). Although the tendency of average BMI are increasing, there was no significant difference in comparison of the average BMI according to the age in male. But, the percentage of Body Fat, the Visceral Fat Area and the WHR, there was statistical significance according to age in male. The middle aged group(between 40~50 years) and older group(over 60 years) were over the normal range. In female, there were significant difference in comparison of the average BMI, the percentage of Body Fat, the WHR, the Visceral Fat Area and the Fitness Score. Especially, in the old female group, the BMI and the WHR score were over the normal range. The BMI: There was positive correlation with the Percent Body Fat(r=0.796, r=0.839, p=0.000), the Body Fat Mass(r= 0.891, r=0.932, p=0.000) and the WHR(r=0.783, r=0.902, p=0.000). Frequency of Exercises, Normal activity Degree, Favorite food, Degree of salt ingestion : There was significant difference in comparison of sex(P<0.05). The results of this study hope to provide an opportunity to further examine the relationship of the Healthy Behavior to Body Composition and obesity.

2.Result of Total Knee Arthroplasty in Pre-operative Adreanal Insufficiency Patients.

PURPOSE: The purpose of this study is to investigate to effect of adrenal insufficiency on the results of TKA. MATERIALS AND METHODS: A total of 89 patients (89 knees) treated with TKA from March, 2008 to October, 2008 were enrolled in this study. Levels of serum cortisol and adrenocorticotropic hormone (ACTH) were checked preoperatively. Hydrocortisone 50~75 mg was injected to adrenal insufficient group at 7:00 AM and 4:00 PM on operative day and the following day. We evaluated the range of motion, the knee society knee score and function score at preoperatively and 2 years follow up, and compared the results between non-adrenal insufficiency group (NAI) and adrenal insufficiency group (AI). RESULTS: Cortisol and ACTH levels were reduced in 36 of 89 patients. All of 36 patients of low cortisol level do not stimulated in ACTH stimulation test. In the adrenal insufficiency group the knee society score (KSS) improved from 49.8 to 86.8 and the knee society functional score (KSFS) from 42.6 to 89.5 at 2 years follow-up. In the control group KSS rose from 51.9 to 84.3 and KSFS from 49.4 to 88.6 during the same period. In adrenal insufficient patients, there were no postoperative complication to include mortality, infection, periprosthetic fracture except skin lesions during operation or postoperatively. There was a case of rupture of quadriceps tendon on the 10th postoperative day which was treated with primary repair. CONCLUSION: Based on our study, there was no increased operative and postoperative complications except skin lesions and 1 case quadricepse tendon rupture in patients with adrenal insufficiency group.

3.Comparison of Blood Pressure Measured at the Aortic Cannula Sideport and within the Ascending Aorta after Cardiopulmonary Bypass.

BACKGROUND: Arterial pressure measured in a peripheral artery sometimes underestimates central arterial pressure after discontinuation of cardiopulmonary bypass (CPB) for reasons that are still unclear. Therefore, the sideport of the aortic cannula is often used for measurement of central pressure because it is accessible and is near the ascending aorta. This study was designed to assess whether blood pressure measured at the sideport of the aortic cannula mirrors blood pressure measured within the ascending aorta. METHODS: Blood pressure was compared in 21 patients 5 minute after discontinuation of CPB at two sites: at the tip of the 23-gauge needle that points distally at a 25degrees to 30degrees angle to the blood flow within the ascending aorta and at the sideport of the aortic cannula. The effects on the sideport-to-aorta pressure differences and wave contour were tested by placing a clamp at the proximal and distal end of the arterial line (aortic cannula sideport-oxygenator tube) of the CPB circuit. RESULTS: There were no statistical differences between systolic, diastolic and mean blood pressure measured at the sideport and ascending aorta when the clamp was applied to the proximal end. The average sideport-to-aorta systolic pressure differences (0.3+/-3.1 mmHg ranged from -5 to 6 mmHg), diastolic pressure differences (-0.9+/-2.1 mmHg ranged from -4 to 4 mmHg) and mean pressure differences (-0.5+/-1.0 mmHg ranged from -2 to 2 mmHg) were not significantly different. No statistical differences were noted between pressures measured at the two sites when the clamp was applied to the distal end of the arterial line. CONCLUSIONS: It is concluded that the pressure measured at the sideport of the aortic cannula reflects the pressure in the ascending aorta after discontinuation of CPB.

PURPOSE: To evaluate the incidence and clinical features of age-related macular degeneration (AMD) in Korea. METHODS: Web-based (www.armd-nova.or.kr) registration was conducted for AMD patients aged 50 or more who were newly diagnosed by retinal specialists in Korea from August 20, 2005 to August 20, 2006. Patient data including ophthalmologic examination, fundus photography, fluorescein angiogram and/or indocyanin green angiogram (ICG), past medical history, behavioral habit, combined systemic diseases were up-loaded. RESULTS: Among finally enrolled 1,141 newly diagnosed AMD patients, 690 patients (60.5%) were male and 451 patients (39.5%) were female. The average age of AMD patients was 69.7+/-8.0. Early AMD was observed in 190 patients and 951 patients had late AMD. Classic choroidal neovascular membrane (CNVM) was observed in 18.6% of exudative AMD patients and 63.4 % had occult CNVM. Subfoveal CNVM was observed in 80.4% of the patients with CNVM. Among the 580 exudative AMD eyes that performed indocyanin green angiography (ICG), 184 eyes (31.7%) had polypoidal choroidal vasculopathy (PCV) and 36 eyes (6.2%) showed retinal angiomatous proliferation (RAP). Age, male gender, smoking, diabetes and hypertension significantly increased the risk of the AMD among Koreans. CONCLUSIONS: Because of the low rate of participation by retinal specialists, definite incidence of AMD was not obtainable. However, the estimated 1-year AMD incidence in the Pusan area of Korea is at least 0.4%. In contrast to Western people, 31.7% of exudative AMD cases were revealed to be PCV and 6.2% were revealed to be RAP. This discrepancy between ethnic groups should be considered in the diagnosis and treatment modality selection of Korean AMD patients.
