2.Animal Experiments of Cutaneous Depigmentation by 4-Isoprophlcatechol and Hydroquinone.

The depigmenting activity of 4-isopropylcatechol and hydroquinone, and the behaviour of salicylic acid affecting to the activity were studied. To the hydrophilic ointment base, 4-isopropylcatechol and hydroquinone were incorperated respectively in concentrations of l% and 3%, and to these creams, salicylic acid was also added in concentration of 3% and 5% respectively. These creams were applied topically to the epilated skins of the back of black guinea pigs once a day for 3 weeks. Followings were the results: 1) The depigmenting activity was found to be more potent in 4-isopropylcatechol than in hydroquinone. 2) The depigmenting activity of hydroquinone was found to be remarkably accelerated by the addition of salicylic acid. 3) The skins of guinea pigs were found to be mildly irritated by the application of both 4-isopropylcatechol and hydroquinone.

3.Computed tomographic findings of malignant ovarian germ cell tumors.

Diagnosis of malignant germ cell tumors of the ovary is usually difficult because many ovarian neoplasms have similar clinical and radiologic manifestations. We reviewed retrospectively 12 cases of ovarian malignant germ cell tumor and evaluated with respect to size, mass characteristics on computed tomography for differential diagnosis. Endodermal sinus tumors were mainly cystic mass with some solid component and septations. Immature teratomas showed typical manifestations, such as fatty tissue, calcifications, cysts, and irregular shaped soft tissue densities. Dysgerminoma was mainly solid mass without cystic component, and mixed germ cell tumor showed nonspecific manifestations. All cases were relatively large in size, and young in age. In conclusion, CT findings of malignant germ cell tumors are helpful in differential disgnosis.

4.Solitary Trichopeithelioma Treated by 5-Fluorouracil Ointment Application.

Trichoepithelioma, a tumor of the hair follicle, is usually seen as multiple lesions, but occasionally appears as a single tumor. Multiple trichoepithelioma is a dominant hereditary skin conditions whereas solitary trichoepithelioma is not hereditary and is growing tumor, usually seen in adults. A case of 56 year old housewife with solitary trichoepithelioma was reported. The skin lesion appeased as a firm, skin colored papule and the size was 0.5cm in diameter on right side of nose. Shaving biopsy was done and showed a high degree of differentiation toward hair structure and horn cyst that the keratinization is abrupt and complete. The methood of treatment is a surgical excision and electrodesiccation but remain a scar due to surgical trauma. And so, we used a 5-fluorouracil ointment instead of surgical excision and electrodesiccation, The skin lesion on nose was cured completely as a result of 5-FU ointment application without any scar formation cosmetically.

5.Range of Active Motion and Axial Angles of the Wrist Joint in Normal Adult Korean

With a ordinary goniometer, we measured the range of active motion of the wrist flexion, extension, ulnar and radial deviation in 120 normal adult subjects ranging in age from 20 to 49 years old. Normal limits were determined for the male and female groups, for the age groups, and for the right and left sides, respectively. The data were compared with the axial angles in roentgenograms that described by T.E. Keats in 1966. And we have undertaken a study of the correlation between the range of motion and the axial angles. The results were as following: 1. Range of active motion (degrees±standard deviation) Rexion; 73.9 ±6.386. extension; 65.9 ±5.164 unlar deviation; 28.3 ±5.124 radial deviation; 18.6±2.556 2. Axial angles (degrees±standard deviation) P-A view;73.8±3.985 lateral view;79.7±4.777 3. Males had a smaller range of motion than female in wrist joint. 4. The right side showed a smaller range of motion than the left. 5. There were no correlations between the range of motion and the axial angles.

6.A Comparative Clinical analysis of Arthroscopic Meniscectomy and Arthroscopic Meniscus Repair

There have been controversies on the rationale, surgicl indication, technique and results of meniscus repair.. Authors comparatively studied 100 cases of meniscus injury in 88 patients (arthroscopic total menisectomy: 16 cases, arthroscopic partial menisectomy: 68 cases, arthroscopic meniscus suturing: 16 cases) from February 1985 to March 1992. The results were as follows: 1. Male (61.4%) was much more affected than female. 2. The third decade group was most commonly affected and the most common causes was sports injury (43.2%) 3. The lateral meniscus was more injured (71%), than the medial (29%) 4. In arthroscopic finding, longitudinal tear (37%) was the most common. 5. According to the Tapper and Hoover's criteria, satisfactory result was 56.3% in arthroscopic meniscus repair group.

7.Clinical Observation on the Allergic Contact Dermatities in Korea.

The author investigated the allergic contact dermatitis patients which visited our out-patient skin dispensary during 1969 and 1970. The patients were evaluated arcording to age, sex and cause of the disease. Furthermore, the patients were analysed as to the causative allergic contact factors by clinical inspection and history. The causes were confirmed by the patch test which was done on 88 of the allergic contact dermatitis patients. It is believed that patch test is clinically a most reliable procedure for the determination of responsible antigenic substance in allergic contact dermatitis and the positive reactions may be labelled as latent or potential hypersensitivity(Hjorth et al, 1968; Maibach, 1965). There is a group of chemicals used on the so-called routine screening patch test tray, which have proved to be frcquent causes of allergic contact dermatitis. Many diagnostic groups of chemicals and therapeutic trays, shoe trays, metal trays, etc., in addition to the standard routine screening patch test tray are used. Certain allergens seem to cause dermatitis in all countries hut many sensitizers vary from one country to anothcr (Calnan, 1955), The author has tried to establish a standard routine screening patch test tray fitted to our country according to the basic consideration used with standard routine patch test trays of the Skin and Cancer Unit, New York University Hospital. Paraphenylene diamine, ammoniated mercury, mercury bichloride, formalin, crude coal tar, potassium dichromate and mercaptobenzothiazole were the most potent sensitizers. These chemicals must be included in our standard screening routine patch test tray. However, the patch test results and the present clinical dermatitis (i.e. cosmetic dermatitis and rubber or leather dermatitis) did not coincide. Further investigation for the establishment of a proper stanard patch test tray for Korea should be carried out.

8.The cervical spinal fractures : comparison of the sites and incidences according to the causes and the types of the injuries.

The fractures of the cervical spine are relatively uncommon, but they may cause serious neurologic deficits temporarily or permanently. So, it is very important to treat the patients early by way of exact evaluation for the sites and the mechanisms of the injuries. The authors reviewed retrospectively 188 cervical spinal fractures in 100 patients from Sep. 1984 to Aug. 1990. Commonly involed levels were C5 and C6 in lower cervical level and C2 in upper cervical level and the sites in each spine were body; lamina and odontoid process. The hyperflexion injury was the most common type of the cervical spinal fractures occupying 53% of all cervical fractures and cause more multipe fractures(2.26 fractures/patient) than in hyperextension (1. 68 fractures/patient). In hyperflexion injuries, body, transverse and spinous process were commonly involved but lamina fracture was relatively common in hyperextension injury. The dislocations associated with fractures were developed most commonly in hyperflexion injury and 70% of these were anterior dislocation and the most commonly involved levels were C5-6 and C6-7. In conclusion, hyperflexion injury needs more close examination for the entire spinal levels than injuries of other mechanisms because it results in more severe fractures with or without dislocation and relatively frequent multiple fractures in different levels.

9.The frequency of the cutaneous problems and the influence of hemodialysis in patients with chronic renal failure.

The purpose of this study was to examine the frequency of eutaneous disorders in patients with chronie renal failure presently on hemodialysis(HD). The results were as follows. 1. Cutaneous lesions were present in all 78(100% ) patients with chronic renal failure. 2. The frequency of msjor cutaneous problems associated with chronic renal failure was as follows: xerosis(82.1%), pruritus(74.4%), nail changes(74.4%), hyperpigmentation(70.5 % ), xerostomia(42.3'Yo), poor wound healing(37.2%), easy bruisability(30,8%), hypotrichosis(21.896), and purpura(14.1 % ). 3. Cutaneous problems which appeared after the initiation of HD were. appearance of new pigmented nevi or lentigines(9 patients), appearance or aggravation of acne(6), contact dermatitis at AV fistula site(2), gynecomastia(1), extensive flat warts(1), extensive tinea versicolor(1), and extensive vitiligo(1). 4. Among 55 patients with hyperpigmentation, 11 patients reported decreased pigmentation following HD. In 15 patients, hyperpigmentation worsened following HD, and in 9 patients hyperpigmentation first appeared after HD was initiated. 5. Cutaneous disorders favorably affected by HD were as follows: easy bruisability(3/ 24), xerostomia(3/33), gingival friability(2/7), and hypotrichosis(5/17). 6. Poor wound healing and xerosis were not improved by HD. 7. Among 58 pruritic patients, 7 patients were improved after HD, 7 patients became worse during each HD, and 3 patients were not pruritic only for 2 to 3 days after each HD. Four patients experinced pruritus only during each HD. 8. There was no significant statistical difference between the frequency of pruritus and xerosis and the levels of blood urea nitrogen, calcium, phosphorus, and parathyroid hormone.
