Authors reviewed mammographic ultrasonographic and MRI findings a patient with breast cysticerocosis, which was operated and pathologically proved. Mammography showed a 1 cm sized round radioopaque lesion with curvilinear calification, which was located near the pectoralis major muscle. Ultrasonograiphic findings showed heterogenous hypoechoic cystic lesion with internal hyperechoic nodule and posterior acoustic shadowing. T2Wl and proton density MR image showed low signal intensity with cresentic high signal intensity portion. 2D-FLASH dynamic MRI showed intermediate signal intensity and peripheral signal void area, which was not enhanced with Gd-DPTA. The possibility of cysticercosis can be considered be considered when a cytic lesion is discovered near the pectoralis muscle in a patient living in an endemic area.

Endometrial carcinoma usually occurs in postmenopausal women ; less than 5% occurs in women under the age of40. Up to one quarter of endometrial carcinoma patients below this age have PCO(polycystic ovary disease, Stein-Leventhal syndrome). The increased incidence of endometrial carcinoma in patients with PCO is related to chronic estrogenic stimulation. We report MR imaging in one case of endometrial carcinoma occuring in a 23 yearold woman with PCO and had complained of hypermenorrhea for about three years. On T2-weighted MR image the endometrial cavity was seen to be distended with protruded endometrial masses of intermediate signal intensity, and the junctional zone was disrupted beneath the masses. Both ovaries were best seen on T2-weighted MR imagingand showed multiple small peripheral cysts and low signal-intensity central stroma.

Ossification of the ligamentum flavum(OLF) is a well recognized cause of acquired spinal canal stenosis resulting in myelopathy, radiculopathy, or a combination of both. OLF is extremely rare in caucasian patients, but is one of the most common causes of compression of the posterior thoracic spinal cord in Japan. In most cases, it has been found in the thoracolumbar regions, and rarely in the cervical region. It is frequently but not alwalys found in association with ossification of the posterior longitudinal ligament(OPLL) and may cause cumulative damage, if comhined with OPLL. We experienced a case of ossification of the ligamentum flavum and posterior longitudinal ligament with sacral radiculopathy, which was treated hy posterior decompressive laminectomy and removal of the ossified ligamentum flavum and posterior longitudinal ligament. The day after operation, the radiating pain and paresthesia dramatically improved.

The goal of fracture treatment is to restore the patient to pre-injury function level. But the outcome assessment after orthopedic interventions has usually focused on physician-defined parameters of technical success, such as fracture union, alignment and range of motion rather than patient function and quality of life. However, the correlation among improvements in these parameters and functional status, psychosocial well being is inconsistent and weak in the elderly. Therefore authors assessed the functional status after surgical intervention of hip fractures in the elderly with functional recovery score(FRS). Independence in basic activities of daily living, in instrumental activities of daily living, in mobility, freedom from pain and intact memory were assessed in 133 hip fractures in the elderly. The hip fractures in the elderly resulted in 24.8% loss of function after the first year. The older the age, the more the loss of function after surgery of hip fractures. The more functional loss was observed in female patients and in patients with poor postoperative radiological results, but statistically insignificant (P>0.05). The more functional loss was observed in patients with underlying medical disease preoperatively and it was statistically significant (P<0.05). We recommend the evaluation of functional status in the elderly after hip fractures.

This paper reports a case of a twisted small bowel obstruction in a 74-year-old man that occurred after a double tract reconstruction (DTR) in a laparoscopic proximal gastrectomy (PG) for early gastric cancer. The patient had inadequate oral intake and reflux symptoms for 10 days after discharge. Imaging analysis revealed a narrowed small bowel with twists between the esophagojejunostomy and gastrojejunostomy sites. A fully covered stent was placed in the narrowed small bowel for 2 weeks. The patient was then discharged after stent removal without any dietary problems. The authors’ experience shows that twisted small bowel after a DTR in PG can be treated by endoscopy.

Gefitinib (Iressa(R), AstraZeneca) is an oral form of an anticancer drug called epidermal growth factor receptor-tyrosine kinase inhibitor. It is widely used for various solid cancers, including lung cancer. Cutaneous adverse reactions induced by gefitinib have recently been reported with an incidence ranging from 49% to 100%, and they include acneiform skin rash, hyperpigmentation, xerotic skin, pruritus, skin fissures, nail change and disorders of the mucous membranes, eyes and hair. To the best of our knowledge, no cases of leukocytoclastic vasculitis associated with gefitinib have ever been published in the Korean medical literature, and there have been only four such reported cases in other countries. Herein, we report on a case of leukocytoclastic vasulitis induced by gefitinib in a patient with lung cancer.

Malassezia yeasts are lipophilic fungi that are regarded as normal flora of the skin. They are associated with a number of diseases affecting human skin, such as pityriasis versicolor, Malassezia folliculitis, and seborrheic dermatitis, and less commonly with other dermatologic disorders, such as confluent and reticulated papillomatosis and transient acantholytic dermatosis. Clinically, Malassezia folliculitis shows erythematous papules, sometimes pustules. Here, mutiple erythematous papules and pustules over the trunk and upper arms developed in a 37-year-old man. The skin lesions became enlarged and confluent, resulting in the formation of an annular configuration. They spread gradually to form a large plaque with central hyperpigmentation, with the edge of the lesions showing multiple papules and pustules. Microspores were found in potassium hydroxide preparations of these lesions, and large-sized circular, cream colored, raised, smooth, and dentate colonies were found in culture media. Histologically, sectioning showed mild acanthosis of the epidermis and perivascularinflammation in the superficial dermis. PAS-stained sections showed spheric to oval yeast organisms in the follicularepithelium. In PCR analysis, Malassezia sympodialis was identified. The patient was treated with oral itraconazle, resulting in complete clearing of the lesions. Here, we report an interesting case of Malassezia follicultis that showed clinical configuration of erythema annulare centrifugum.

Cryoglobulinemia, first described by Wintrobe and Buell in 1933, is characterized by the presence of serum immunoglobulins that precipitate below 37degrees C and that dissolve on rewarming. Although the actual mechanism of cryoprecipitation and the etiology of cryoglobulinemia remains obscure, it is frequently associated with other systemic diseases, such as hepatitis C virus (HCV) infection, multiple myeloma, Sjogren's syndrome, B cell lymphoma, and systemic lupus erythematosus. No cases of cryoglobulinemia associated with B cell lymphoma have ever been published in the Korean medical literature, and few such cases have been reported in other countries. Herein, we report on a rare and interesting case of cryoglobulinemia developed in a patient with gastric diffuse large B cell lymphoma.

CD20 positive T cell lymphoma is a rare condition that is associated with the coexpressions of CD20 and T cell markers, such as, CD3, CD5, or UCHL-1. Positivity for CD20 in this tumor represents an aberrant immunophenotype, but the presence of monoclonal T cell receptor (TCR) gene rearrangements and negativity for immunoglobulin heavy chain gene rearrangement indicate that this tumor is a T cell lymphoma. The majority of cases of CD20 positive T cell lymphoma have been reported as immature peripheral T cell lymphoma not otherwise specified. However, we believe that this disease is likely to be re-listed as a new disease entity after its pathogenesis has been elucidated and more cases have been evaluated. Here, we present a case of peripheral T cell lymphoma coexpressing CD20 and T cell markers with a demonstrable TCR gene rearrangement, in a patient who had been misdiagnosed as having B cell type lymphoma 4 years previously. We hypothesize that in this case initially circulating normal CD20+ T cell subsets underwent neoplastic transformation and CD20 positive T cell lymphoma subsequently developed in the lymph node, and then recurred in the skin due to systemic disease or metastasized from the nodal disease.
