Allopurinol is widely used for chronic tophaceous gout as a uric acid lowering agent. Hypersensitivity to allopurinol occurrs in about 10% of patients, which limits the usage of allopurinol. The successful oral and intravenous desensitization of allopurinol has been reported worldwide since 1976. We recently experienced a 51-year-old male patient with gouty arthritis and hyperuricemia, who had previously experienced skin rash after allopurinol treatment. When allopurinol was retried, erythematous and foliative skin rash developed on entire body. Because allopurinol was essential in controlling hyperuricemia, the oral desensitization of allopurinol was tried. We report successful rapid oral allopurinol desensitization in the patient with chronic tophaceous gout, who exhibited exfoliative dermatitis as allopurinol hypersensitivity.

A statistical analysis of the diagnostic value for 244 aspiration biopsy cytology(ABC) among a total 1,043 cases from various sites was performed. ABC, using diagnostic terminology similar to that of a surgical pathology reports, was compared to the final tissue diagnosis. For the entire series, a sensitivity of 91.8%, a specificity of 99.3%, a positive predictive value of 98.9%, a negative predictive value of 94.8%, and an efficacy of the test of 96.3% were shown. There were 8 false negative and 1 false positive diagnosis. The diagnostic accuracy was 89.8%. Those results indicate that the ABC is a considerably highly accurate procedure that should be routinely employed.

Clinical study was carried out on the 64 hemodialysis patients(HD) with chronic renal failure who had been treated from December 1992 to July 1993 in Yeungnam University Hospital. The following results were obitained. In hematologic parameters, MCH was 28.8±2.0pg, and MCV was 92.4±4.7fl. Result revealed normochromic and normocytic anemia. Mean values of serum ferritin were 657.4±292.0ng/ml in men and 511.5±370g in women. Mean values of serum iron were 145.5±63.7µg/dl. Mean values of transferrin saturation was 61.6±28.4%. Serum frerritin, serum iron and transferrin saturation were higher in HD group than normal reference. In erythropoeitin treatment group, Hb and Hct were significantly higher than non-erythropoietin treatment group. Amount of transfusion was significantly higher in non-erythropoietin treatment group than erythropoeitin treatment group(p<0.05). Values of iron, transferrin saturation were significantly higher in abnormal liver function test(LFT0 hemodialysis group than normal LFT group(p<0.05). Transfusion amounts revealed positive correlation with ferritin(r=0.4675), transferrin satruation (r=0.3823) and iron(r=0.3386) (p<0.05).

The effect of topical steroid application on clinical results of 215 eyes(126 patients) performed with excimer laser photorefractive keratectomy (PRK) from July 1993 to July 1994 was evaluated, postopertively. The range of myopia was between -2.5 and -11.5 diopter and followed for 9 months after PRK with Summit Omnimed(TM). Topical corticosteroid or non-steroidal antiinflammatory drug (NSAID) was not applied for one month after operation in all cases. After one month, in cases of 115 eyes showing myopic regression of more than -1 diopter or stromal haziness of more than grade 1(delayed steroid group; DSG), 0.1% fluorometholone was applied 4 times daily for 4 weeks, then tapered according to the planned schedule. 46.5% (100 eyes, non-steroid group;NSG) did not show the myopic regression nor stromal haziness without treatment of fluorometholone throughout the follow-up period. Uncorrected visual acuity, spherical equivalent and corneal haziness were evaluated 1, 2, 4, 8, 12, 24, 36 week after operation. 73%(73 eyes) of NSG and 27%(30 eyes) of DSG showed a myopia less than 6.0 diopters preoperatively.
