3.Analysis of Angiographic Outcome by Thrombolysis in Myocardial Infarction(TIMI) Frame Count for Primary Stenting in Patients with Acute Myocardial Infarction.

BACKGROUND AND OBJECTIVES: Primary intervention by stent implantation during acute myocardial infarction is a novel strategy to provide better myocardial perfusion compared to thrombolysis or baloon angioplasty. We aimed to assess the reperfusion achieved by primary stenting, employing TIMI frame count for more objective and quantitative measurement. MATERIALS AND METHOD: Measurements for number of frames required to opacify standardized angiographic landmark branch(TIMI frame count) were determined for the coronary arteries of 77 normal controls and 65 patients with acute myocardial infarction who underwent primary stenting within 12 hours of symptom onset. RESULTS: In normal subjects, TIMI frame count for left anterior descending artery(LAD) was 1.3 times of mean count of right coronary artery(RCA) and left circumflex artery(LCx), and significant less than that of TIMI study(22.3+/-4.9 vs 36.2+/-2.6, p<0.05). TIMI frame count for RCA and LCx was similar to results of TIMI study. In infarct related arteries(IRA), corrected TIMI frame count(CTFC) after primary stenting was similar to those of normal control. Frame counts of RCA was larger compared to that of normal control, but statistically insignificant(23.0+/-7.5 vs 17.6+/-3.5, p>0.05). There was no difference of CTFC of non-infarct related arteries between patients and normal controls. CONCLUSION: The TIMI frame count of LAD artery in normal Korean subjects was significantly less than that of American counterpart. In patients with acute myocardial infarction, primary stenting appeared to provide improved coronary flow similar to that observed in normal subjects, as measured by TIMI frame counting.

5.W-Shaped Triple V-Y Advancement Flaps for the Correction of Secondary Cleft Lip Nasal Deformity.

A secondary cleft lip nasal deformity can be corrected by many procedures including central lip tissue transfer, forked flap, nasal floor and alar base advancement, nasal skin flaps and composite graft. These procedures have been applied for many years, but each one has some conspicuous residual problems. We propose triple V-Y advancement flaps, which we have performed on 13 secondary cleft lip and nasal deformity patients over the past 6 years. The W-shaped triple reverse V-shaped incision easily exposed the alar cartilages. We made the advancement and rotation of the nasal skin flap to lengthen the columella and project the nasal tip. Postoperatively, the columella length and nasolabial angle were found to be increased by an average of 3.7 mm and 42 degrees, respectively. In lateral view, a relatively natural nasal line was achieved. The scar in the nasal tip posed few problems. This procedure should be useful for the correction of the secondary cleft lip nasal deformity.

6.Prevalence and Risk Factors of Symptom-Giving Pelvic Girdle Relaxation in Pregnant Women.

OBJECTIVE: This study was done to evaluate the risk factors and prevalence of symptom-giving pelvic girdle relaxation in pregnant women. METHODS: Three hundreds thirty-two postpartum women were asked to fill out a questionnaire within one week after their parturition. The diagnostic criteria of this lesion were adopted from the Norwegian Medical Association's and Larsen's criteria. The questionnaire were included age, parity, BMI(kg/cm2), weight gain during the pregnancy, history of symptom giving pelvic girdle relaxation in previous pregnancy, level of exercise and vocational status during and before the pregnancy and baby's birth weight. The answers were evaluated to determine the risk factors. The correlation was tested by student t-test and logistic regression. RESULTS: The prevalence of symptom-giving pelvic girdle relaxation during pregnancy was 18.9%. The history of symptom-giving pelvic girdle relaxation in previous pregnancy, multiparity and the absence of regular exercise before and during pregnancy showed correlation with symptom-giving pelvic girdle relaxation in pregnancy (p<0.05). CONCLUSION: It is suggested that women who experienced symptom-giving pelvic girdle relaxation in previous pregnancy should be checked for this lesion before pregnancy. Regular exercise is required to lower the incidence and lessen the severity of this lesion in pregnancy.

7.A Case of Cryoglobulinemia Associated with Multiple Myeloma.

Cryoglobulinernia occurs in about 5% of the cases of multiple myeloma. The most common finding in patient with cryoglobulinemia is ulceraticn that oceurs about ankle, hands, and occasionally the ears, upon prolonged exposue to cold. A 59-year old male had had 5 years of pain in his ears. There were black or dark brown colored ischemic ulcerations on his both helix. He also had mottled purpuric patches on his both ankles. A test for cryoglobulinema was positive. X-ray examination of the skull showed multiple punched but lesions. The bone marrow study revealed myeloma cell infiltration.

8.Radiologic study on measurement of lumbosacral angle in backache patients

The radiologic findings of lumbosacral spine and measurement of lumbosacral angle were analysed in 238 with backache and 102 without backache which were visited at Chungnam National University from March 1980 to July 1981.The measurement of lumbosacral angle was based on a method of Fergson. The results obtained were as follows; 1. The age group of 18 to 29 years was most common in backache group and the male was affected more frequently than the female with the ratio of 1.9 to 1. 2. In patients with backache, the overall mean lumbosacral angles were 44.2±2.6 degrees in male and 35.8±2.0 degrees in female. In patients without backache, in control group, the overall mean lumbosacral angles were 32.6±0.7 degrees in male and 33.4±1.4degrees in female. 3. In control group, difference of means between male and female was about 1degrees in patients with backache, the overall mean lumbosacral angles were increased about 12degrees in male and 2degrees in female than control group. In patients with and without backache, no significant difference of lumbosacral angle between the 4 age groups was present. 4. In backache group, increased lordosis was more common and increased lumbosacral angle than the decreased lordosis. 5. In backache group, lumbosacral angle of abnormal radiologic findings in lumbosacral spine was significantly increased than control group. 6. In patients with backache, radiologic findings and its lumboscral angles were alumbosacral anomaly 56 cases (23.5%): 46.9 degrees, increased lumbar lordosis 46 cases (19.2%): 48.1 degrees, osteoarthritis 44 cases(18.5%) : 40.8 degrees, decreased lumbar lordosis 30 cases (12.6%): 29.9 degrees, in order, And these radiologic findings were similar with many other authors.

9.THE EFFECT OF PLASMA-TREATED POLYETHYLENE FIBER ON THE FLEXURAL STRENGTH OF COMPOSITE RESIN IN VARIOUS APPLIED PORTIONS.

There has been many researches aimed at reinforcing the strength of resin, and these have led to the development and use of numerous materials in recent years. A case in point, is the recent development of plasma-treated polyyethylene fiber which has been used mainly in fixed provisional restoration to reduce the incidence of fractures. This study aims at assessing whether plasma-treated polyethylene fiber as applied to composite resin is effective in increasing the flexural strength and how applied portions affect this. Twenty-four applied and eight unapplied composite resin bars were fabricated. Twenty-four applied specimens were divided into three groups. Plasma treated polyethylene fiber was applied to the groups each with different portions of composite resin. In the first group, plasma-treated polyethylene fiber was not applied. In the second group, fiber was applied to the compression side of composite resin. Fiber was applied to the tension side in the third group, while fiber was embedded in the tension side of the compositeresin in the fourth group. Each specimen was tested by use of a three-point bending strength test with an instron testing machine, and the flexural strength was calculated. The following results were obtained. : 1. Under the conditions of this study, the third and fourth groups demonstrated a statistically greater flexural strength compared to the first and second groups. 2. But there was no statiscally significant difference, not only between the first group and the second group, but also between the third group and the fourth group. Taken together, it can be concluded that plasma-treated polyethylene fiber applied to composite resin is an effective method in increasing flexural strength, and the best way of increasing the flexural strength is by application of plasma-treated polyethylene fiber to the tension side, or the embedding of same in composite resin. It must be mentioned however that this test used a static single-load test method. This method determined the maximum stresses that could be tolerated, but this might not be valid where the prediction of clinical failure is concerned. In order therfore to clinically utilize plasma-treated polyethylene fiber to reinforce the composite resin, it is suggested that a further study which considers the various loads be undertaken.

10.Comparison of ANG II-mediated Ca2+i, IP3 Production and ATPi in Isolated Renal Proximal Convoluted Tubules of Adult SHR and WKY.

BACKGROUND: The renin angiotensin syaimstem plays an important role in hypertension. Therefore, the purpose of this study was to investigate the comparison of responsiveness to angiotensin II (ANG II) in isolated renal proximal convoluted tubules of spontaneously hypertensive (SHR) and Wistar Kyoto (WKY) rats. METHODS: Intracellualr calcium concentration ([Ca2+i) was measured using Fura- 2/AM, inositol trisphosphate (IP3) accumulation was determined by radioimmuno assay and cellular ATP content measured using the microchemilunescene method in renal proximal tubule suspension or isolated renal proximal tubules. RESULTS: When measured the ANG II-induced [Ca2+i, the young rats showed a greater response to ANG II than adult rats in both strains. The ANG II (10-7 M)-induced [Ca2+i transient in the cortical tubule suspension from adult SHR was significantly lower than that in age-matched WKY. In isolated proximal tubule segments, ANG II-induced [Ca2+i increment was only observed in S1 segments. Comparing responsiveness to ANG II in SHR and WKY, similar phenomenon was observed as experiment using tubule suspension. IP3 accumulation by ANG II also attenuated in adult SHR. The 20-minutes incubation without any exogenous substrate in proximal convoluted tubule (S1) significantly decreased cellular ATP content and ANG II (10-7 M) inhibited decrement of cellular ATP level. The effect of ANG II on cellular ATP restoration was disappeared by the treatment with losartan. CONCLUSION: From these results, the responsiveness of ANG II to AT1A receptor is attenuated in the proximal convoluted tubules of adult SHR comparing the age- mached WKY.
