Ligamentous injury of the knee which could be easily occur in sports or traffic accidents resulted in more and severe disability than that of other joints. Rupture of the anterior cruciate ligament which restrain hyperextension, excessive rotation and anterior motion of tibia on femur is the most frequent and significant injury of the knee. Its treatment and clinical significance is one of the most controversial issue and its incidence tends to increase. From 1983 to 1985, modified Scott operation were performed on 10 patients at the department of orthopedic surgery, Chung Ang university hospital. Following results are obtained. 1. No limitation of range of motion of the knee was developed. 2. Intra-articular fixation by use of pull-out wire was done and special consideration was done for the site of fixation on tibia plateau and method of dilatation of posterior capsule. 3. Of the ten patients, nine had negative anterior drawer test, postoperatively.

Local deposition of antibiotics has become increasing popular in the management of open fractures or osteomyelitis, and several substances have been employed as the vehicle for delivery. Although the elution characteristics of some substances have been documented, a comparative study of the characteristics of the commonly used substances were performed in order to establish the clinical indications for particular vehicles. Carriers were prepared, which were human iliac cancellous bone, bovine cancellous bone matrix(Lubboc?), absorbable gelatin sponge(Gelfoam?), fibrin glue(Beriplast? P) and polymethylmethacrylate(CMW?) for elution characteristics of vancomycin. The each carriers were immersed on the 20 ml of PBS and then obtained the samples for analysis of concentration of vancomycin at first, second, third, fourth, fifth, sixth, fourteenth, twenty-first, twenty-eighth day after immersion. The assay technique was fluorescent polarization immunoassay(Abbott, Dallas, Texas, U.S.A.). Nearly 50% of vancomycin was released from human iliac cancellous bone, Lubboc? and fibrin glue during the first 3 days, and negligible after first week. Gelatin sponge produced high local concentration of vancomycin during the first week. PMMA eluted the only 6% of vancomycin during the first day and trace amount detected as long as 4 weeks. There are significant statistical difference among carriers at second and fourth week(ANOVA test, P < 0.05). The authors considered that human iliac cancellous bone, Lubboc? ,Gelfoam? and fibrin glue may be best employed when brief antibiotic coverage is required, whereas PMMA may be better suited for long-term coverage.

Human sparganosis is one of the fhost common tissue parasitoses caused by helminthes in Korea. Human sparganosis is found most frequently in subcutaneous or adipose tissue of the abdominal wall, thoracic wall and inguinal region, it is rarely found in the orbital cavity, abdominal cavity, brain, breast, or lower extremities. We experienced one ease of sparganosis which developed into the thigh and popliteal area in combination with avascular necrosis of the femoral head. The case is a 59 years old male. The patient had the history of eating raw meat as a tonic food 3 years ago. The presenting symptom was left hip joint pain. We confirmed the sparganosis in microscopic findings of worms and by immunologic serum studies. The avascular necrosis of the femoral head was detected with plain roentgenography and magnetic resonance imaging. Many spargana were removed from the lesions by surgical excision. Two of them were alive, and others had degenerated or calcified. The life cycle and route of infestation of sparganosis were discussed briefly and related literatures were reviewed.

BACKGROUND: We investigated the effectiveness of fibular strut allograft augmentation of proximal humerus fractures to prevent varus deformity in patients over the age of 65 years with insufficient medial support. METHODS: We analyzed the clinical and radiological outcomes of locking plate fixation with adjunct fibular strut allograft augmentation in 21 patients with proximal humeral fractures. The inclusion criteria were age (65-year-old or older); presence of severe medial comminution; inadequate medial support; and those who could participate in at least a one year follow-up. The average age was 76.4 years. We analyzed each patient's Constant score, our indicator of clinical outcome. As radiological parameters, we analyzed time-to-bone union; restoration of the medial hinge; difference between the immediately postoperative and the last follow-up humeral neck-shaft angles;; and anatomical reduction status, which was assessed using the Paavolainen method. RESULTS: A successful bone union was achieved in all patients at an average of 11.4 weeks. We found that the average Constant score was 74.2, showing a satisfactory outcome. The average difference in the humeral neck-shaft angles between the immediately postoperative time-point and at the final follow-up was 3.09°. According to the Paavolainen method, the anatomical reduction was rated excellent. The medial hinge was restored in 14 of 21 patients. Although we did not find evidence for osteonecrosis, we found that a single patient had a postoperative complication of screw cut-out. CONCLUSIONS: Fibular strut allografting as an adjunct treatment of proximal humeral fractures may reduce varus deformity in patients with severe medial comminution.

BACKGROUND: Recently, clinical use of fresh frozen plasma(FFP) has increased in all blood transfusion activity. But inappropriate uses of FFP may be serious problem due to adverse transfusion reactions, viral transmission, and economic aspects. The aim of this study is to justify clinical indication of FFP by receiving the inappropriate use of them. METHODS: Analyzing hospital records of 115 patients transfused with FFP at Kyung Hee Medical Center from January to April 1998, retrospectively, we investigated the purpose of transfusions, the volume of transfused FFP, and the results of pre- and post-coagulation tests. RESULTS: The most common purpose of transfusions of FFP was plasma volume replacement after surgical operation. Coagulation tests of pre- and post-transfusion were performed 85% and 47%, respectively. Fifty-seven patients(88%) transfused FFP have shown normal results in pre-coagulation tests, and the volume replacement with FFP was done inadequately in 62 cases(54%). CONCLUSION: We conclude that, in order to eliminate side effects due to inappropriate use of FFP, and protect patients, it is necessary to make guidelines of FFP transfusion, educate hospital staffs regularly, and perform pre- and post-coagulation tests routinely for the patients who need FFP transfusion. The purpose of this study was to find the distribution and measurement of compressive and tensile stress when intrusi- on arch wire is forced engage with upper canine and to analysis stress at each section through FEM. And we compare compressive and tensile ratio at each section. The results were as follows. 1. At FA point and cemento-enamel junction of upper canine, compressive and tensile force ratio is about the same. 2. At apex, compressive force is the four times as tensile force. ; In intrusion, we show root resorption at apex. 3. At Cemento-enamel junction, the compressive and tensile force show the maximun value except FA point.

Rhabdomyosarcoma which was first described by Weber has generally been considered asan uncommon tumor. Pack and Eberhart were of the opinion that rhabdomyosarcoma represented 13.9 percent of soft tissue sarcomata. Recently it was classified as four types such as pleomorphic, embryonal, botryoid and alveolar type by many authors. We have experienced two cases of embryonal rhabdomyosarcoma, one arised within left leg and the other within right foot. The former(3 months old, female) showed tumor mass at birth, the latter(13 months old, male) showed at 1 month after birth.
