The possible complications after parotidectomy are facial nerve palsy, Frey's syndrome, sialocele, salivary fistula, hematoma infection, and flap necrosis. Postoperative sialocele and salivary fistula are relatively uncommon and the management varies. We experienced a rare postoperative complication of excessive and prolonged drainage of saliva after superficial parotidectomy and removal of an necessary parotid gland tumor. The patient could be treated successfully with conservative management such as compressive dressing, restriction of oral intake of irritant food, maintenance of oral hygiene and the use of amitriptyline(antidepressant).

Cystadenomas are benign neoplasm in which the epithelium demonstrates adenomatous proliferation. Several morphologic variants of cystadenomas have bean described, including papillary-oncocytic, mucous, and seromucous subtypes. Cystadenomas arising from salivary glands are very rare and seromucous cystadenoma of parotid gland has not been reported in Korea so far. Cystadenomas of the parotid gland usually Present as asymptomatic slowly growing mass and those of the minor salivary glands produce smooth nodules that may be compressible. We performed a superficial parotidectomy in a patient with a seromucous cystadenoma. This may be the first report of seromucous cystadenoma of carotid gland in Korea.

PURPOSE: This study aimed at evaluating the therapeutic outcome, cost effectiveness and recurrence rate in children with H. pylori infection after the treatment using various medications. METHODS: Seventy five children (mean age 11.4+/-2.5 years) were given an endoscopy with biopsy and H. pylori status assessed by CLO test and histologic examination (Warthin Starry stain). Children were given one of following medications such as DA (Denol(R) and amoxycillin), OA (omeprazole and amoxycillin), DC (Denol(R) and clarithromycin) as primary treatment. And one of following medications such as DAM (Denol(R), amoxycillin and metronidazole), DC, OA, OC (omeprazole and clarithromycin) were used in children who failed the eradication of H. pylori. RESULTS: The endoscopic diagnoses were: nodular gastritis (46 cases), gastric ulcer (9), duodenal ulcer (6), superficial gastritis (6), and normal (8). H. pylori eradication rate was 91% (63 of 69 children) on 4 weeks course of DA, 50% (1 of 2 cases who had treatment failure on DA) on DAM, and 75% (3 of 4 cases who treated on DC primarily) and 50% (1 of 2 cases who had treatment failure on DA) on DC, and 100% on OA (all of 2) and on OC (all of 1 who failed on DA). In 3 of 7 children in whom H. pylori had not been eradicated by primary medications (DA 6 and DC 1 case), H. pylori was re-eradicated by secondary medications (DA 1, DAM 1 and DC 1 case). But in remaining 4 cases, H. pylori infection persisted. Reinfection of H. pylori was found in 4 of 75 children between 3 months and 3 years after completion of the treatment of DA, yielding recurrence rate of 5.3%. In 2 of 4 cases who had relapsed, H. pylori was re-eradicated by secondary medications (OC 1 and DA 1 case). But in remaining 2 cases, H. pylori infection persisted. CONCLUSION: These results suggest that dual therapy with Denol(R) and amoxycillin is the effective medications in treating H. pylori infection in children. Concerning the cost effectiveness, it can Austrabe recommended as first line treatment of choice as well.

We followed up the 100 patients (104 eyes) who had undergone cataract extraction and PCL implantation into the bag with 3M diffractive multifocal intraocular lensee for at least 2 month, in order to evaluate the function of this lens. 92 eyes (88.5%) had the far corrected vision more than 0.5 and 94 eyes (90.4%) had the near vision with far correction more than J3. We added additional adds (-0.25, +0.25, +0.50, +0.75D) in order to ascertain the adequacy of +3.50D add in this lens and it was found that the additional add of +0.75D was most preferred. This means that more add is necessary for near vision. There was, also, no more remarkable side effects or difficulties associated with this lenses. So, multifocal Intraocular lenses enable good near vision without any severe disadvatages but the amount of near add is cuntroversial.

We report a rare case of remote cerebellar hemorrhage after intradural disc surgery at the L1-2 level. Two days after the spine surgery, patient complained unexpected headache, dizziness, nausea and vomiting. From the urgently conducted brain CT, it was reported that the patient had cerebellar hemorrhage. Occipital craniotomy and hematoma evacuation was performed, and hemorrhagic lesion on the right cerebellum was effectively removed. After occipital craniotomy, the patient showed signs of improvement on headache, dizziness, nausea and vomiting. He was able to leave the hospital after two weeks of initial operation without any neurological deficit. Remote cerebellar hemorrhage following spinal surgery is extremely rare, but may occur from dural damage of spinal surgery, accompanied with cerebrospinal fluid leakage. Early diagnosis is particularly important for the optimal treatment of remote cerebellar hemorrhage.

Objectives: This research probes into the characteristics of modern type depression (MTD) in South Korea, drawing parallels to Japan’s hikikomori state. It aims to bridge cultural value gaps and highlight its significance in South Korea, given the observed reclusive youth behaviors. Methods: Participants undertook a semi-structured interview assessing hikikomori tendencies and MTD. Also, self-reports were done using the 25-item Hikikomori Questionnaire (HQ-25), 22-item Tarumi’s Modern-Type Depression Trait Scale: Avoidance of Social Roles, Complaint, and Low Self-Esteem (TACS-22), Patient Health Questionnaire-9 (PHQ-9), Beck Depression Inventory-II (BDI-II), Young’s Internet Addiction Test (YIAT), Smartphone Addiction ScaleShort form Version (SAS-SV), Revised UCLA Loneliness Scale (RULS), Lubben Social Network Scale (LSNS-6). Scores were analyzed by group (MTD, control) using and logistic regression was done. Additional analysis was done by group (modern type with hikikomori and modern type without hikikomori). Results: Of 59 participants, 29 had MTD. In both groups, age and sex were matched with differences in education years. The MTD group contained 14 hikikomori individuals. Logistic regression identified the HQ-25 as a significant predictor for MTD. Notably, specific components like “low self-esteem” in TACS-22 and “isolation” in the HQ-25 significantly influenced MTD status. HQ-25 and RULS were significantly different between modern type with hikikomori and modern type without hikikomori. Conclusions Using Japanese diagnostic criteria, this study illuminated MTD in Korean patients. The findings underscore isolation and low self-esteem predicted diagnosis of MTD. As MTD shares common pathological traits with hikikomori, the therapeutic interventions are important in Korea.
