1.Heart Diseases in Korea.

Statistical observation was made on 2,639 cases with heart disease which has been seen in the Deparment of Internal Medicine, National Medical Center, in Korea, during the past 15 years ranging from January 1959 to December 1973. The results were as follows; 1. During the last 15 years, 2,639 cases with heart disease were observed, which represents about 12.9% of total 20,434 medical In-patients, and variable incidence for the consecutive years was noted. 2. The disease occurred most frequently in group aged under thirty and its incidence decreased in the fifth and sixth decades. 3. The incidence in male was 49.8% and in female 50.2%, the ratio of which was almost same. 4. The most common etiologic form of heart disease was hypertensive heart disease which occurred in 43.2% of all cardiac patients, corresponding to 5.57% of all In-patients. The incidence was most frequent in the sixth decade and decreased in the fifth and fourth decades with the male preponderance. 5. The second common heart disease was rheumatic heart disease, 31.6% of all cardiac patients, corresponding to 4.08% of all In-patients. It affected the mitral valve in most of the cases, whereas involvement of the aortic valve was rare. 6. Coronary heart disease, the third common heart disease, occurred in 5.1% of all cardiac patients, and most frequently in sixth and seventh decades. it affected male 2.5 times more frequently than female. 7. Postpartum heart failure and beriberi heart disease showed 3.8% and 3.1% respectively. They occurred more frequently in younger female, but the frequency was much decreased recently. 8. Pericarditis occured in 2.6% of all cardiac patients and tuberculosis was still a common cause of pericarditis. 9. Congenital heart disease was seen in 1.7% of the cases and patent ductus arteriosus, tetralogy of Fallot, atrial septal defect and ventricular septal defect occupied 76% of all congenital heart disease. 10. Cor pulmonale was present in 1.6%, arrhythmia 1.3%, cardiomyopathy 1.3%, thyroid heart disease 0.8%, endocarditis 0.7% and syphilitic heart disease 0.2%.

3.Dislocation of Peroneal Tendons Two Cases Report

Dislocation of peroneal tendons are caused by forceful domifiexion of the foot accompanied by a powerful contraction of the peroneal muscles. This mechanism of injury tears the superior pemneal retinaculum and allows the tendons to snap anteriorly. It is an important, although infrequent, cause of disability of the ankle. The acute injury can be treated by immobilization in a plaster cast, but when there is chronic recurring dislocation, surgical reconstruction is necessary. Authors experienced two cases of dislocation of pemneal tendons which were treated by use of Sarmiento's and Platzgummer's(modified) operative procedures with good results.

4.Antiproliforative effect and HLA-DR induction of recombinant gamma interferon on cultured human keratinocytes.

The authors investigated the antiproliferative effect and expression of HLA-DR an- tigen by recombinant gamma-interferon (r-IFN-y) on cultured human keratinocytes (KC). The results were as follows, 1. From 10l.J/ml of r-1FN-p exposure, the proliferation of KC decreased in a concentration dependent fashion. But there was little difference of antiproliferative effect above 30U/ml of r-IFN-y exposure. 2. The expression of HLA-DR antigen on KC increased in a concentration and time dependent fashion of r-IFN-p exposure. E3ut t,here was little difference of HLA-DR antigen expression on KC above 30tJ/ml and most of HLA-DR antigen were expressed within 48hr. 3. The opt,imal condition for HLA-DR antigen induction on KC by r-IFN-p was likely t,hat HLA-DR KC was observed at 48hr under the our exposure of 30U/ml of r-IFN p. 4. After 4hr exposure of 30U/ml of r-IFN-p, KC expresed HLA-BR. antigen, reaching a maximum intensity at 3 days. At, 7 days, the loss of HI A-DR KC showed over 90% of maximum intensity.

5.Clinical Trials of Esperson on Various Dermatoses.

Esperson, a new topieal steroid ointment containing 0. 25% 17-desoxymethasone, w as used in several kinds of dermatoses. Total 32 patients including 8 cases of contact dermatitis, 2 of atopic dermatitis, 5 of seborrheicdennatitis, 3 of neurodermatitis, 4 of nummular eczema, 2 of hand eczema, 1 of chronic eczema, 1 of perioral dermatitis, 5 of psoriasis and 1 of pityriasis rosea were treated with Esperson. Among these 32 patients, we observed excellent effects in 11 patients (34.4%), good effects in 16 patients (50%) and fair effects in 4 patients (12. 5%). Namely, among these 32 patients, 27 patients (84. 4%) revealed excellent to good therapeutic effects on their skin conditions with Esperson. In some details, 22 out of 26 patients (84.6%) with eczema and 4 out of 5 patients (80%) with psoriasis revealed excellent to good therapeutic effects respectively. No untoward side effect was noted in all subjects treated with Esperson.

6.A case of pemphigus erythematosus.

We present here, a case of pemphigus erythematosus developed in a 31-year-old woman, who had been suffering from repeated episode, of remission and exacerbation with several erythematous, and somewhat oozing patche accompanying a mild pruritus or her face, upper part of the back and anterior chest. In serological studies, she had antinuclear antiboc@lies at. a titer of 1:160. Histopathological finding showed subcorneal bullae. Immunofluorescent study revealed IgG and C, deposition in the intercellular space of the epidermis and along the de rmoepidermal junction.

9.Anthralin Therapy for Psoriasis.

Our Study was performed to evaluate the effect of anthralin for the treatment of psoriasis. Thirty seven patients with mild form of psoriasis were trested with topical anthralin ointment. 1. Twenty patients(54%) showed clearing of lesion within 8 weeks. 2. Treatment with anthalin was failed in 15 patients(40.5%). 3. Side reaction such as staining, pruritus and erythems was noted in 18 patients (48.6%), among them, 8 patients(21.6%) terminated the therapy with anthralin due to marked side reaction.

10.A Clinical Investigation of Pustulosis Palmaris et Plantaris.

A clinical investigation of the patients with pustulosis palmaris et plantaris (PPP) was made on 36 patients who were seen at the Department of Dermatology, Seoul National University Hospital, from January 1986 through June 1987. The results were summarized as follows : 1. In this series of 36 patients, 14 patients(38.9%) were male and 22 patients (61.1%) were female. Females are more subject to the affection than males. 2. The mean age of onset in male was 30.6 years and in female it was 38.4 years, indicating a earlier onset in male. The peak age of incidence in male was between 20 and 29 years and in female, between 40 and 49 years. 3. The sites of involvement were mostly bilateral hands and feet. 4. the season of the year did not influence the severity of the skin lesions in 68.9%. 5. Nine patients(28.1%) had psoriasis-like skin lesions and 14 patients(43.1%) had nail changes out of 32 patients with PPP. 6. No direct relationship could be found between pustular eruptions and focal infections, such as tonsilitis, otitis, dental abscess and sinusitis. 7. Remmission of the pustular eruptions could be achieved in 5 out of 10 patients treated with etretinate and in 4 out of 11 patients treated with ketoconazole for more than 4 weeks' treatment
