The author developed a computer program for use in registration, monthly statistics, printing of reports, data storage and retrieval in the Radiology department, This program has been used in the Department of Radiology, MoonHwa Hospital since November 1990. This program was written in FoxBASE language, consisted of two independent subprograms, one installed in a registration computer without a printer and another in a reporting computer with a printer. The subprograms were designed to link their data by floppy disk. Each computer's hard disk contained permanent files for retrieval and temporary files for data input. All permanent files were indexed on several keywords including the patient's identification data. 1. Registration was performed easily and rapidly. 2. A monthly statistic was obtained simply. 3. Retrieval of the results of previous radiologic studies, printing of report, storage and indexing of data were achieved automatically. This program had merits of simple operation, large storage capacity, rapid retrieval speed, relative low price, easy adjustment for other hospitals. Therefore this program is considered to be an economic substitute for computerization of radiology departmental management in medium sized hospitals.

The authors tried to do the setup of a database management system of congenital heart diseases. We used a personal computer (IBM PC compatible, 486DX2, 50MHz, 256 Cache memory, 250 MB Hard disk) and developed database program by using Foxpro for Windows(version 2.6, Microsoft, USA). For classification and compatibility of terms in congenital heart diseases, we used code system which was modified from Boston Children's Hospital's codes. The program's menu consisted of input, retrieval, statistic and other utility menus. Used data were summary report, echo-cardiographic report, catheterization laboratory report, surgery report, fetal echo-cardiographic report, electrophyiologic study report and reply report. These all data was related each other. The essential code number of each datum was transferred to summary data automatically and simultaneously on entry of data. Because many portions of input fields of each data were to be selected by menu and popup style, special education for use this program was not necessary. All stored data could be retrieved by example-showing method rapidly and easily. Statistics of diagnosis and surgical results could be gotten by several key strokes, simply. On networking, multiple users could use this program without difficulty. We could consummate the database program which was very effective in data management for congenital heart diseases.

OBJECTIVES: The analysis of heart rate variability (HRV) is a useful non-invasive tool to investigate the autonomic nerve function. Previous studies on the relationship between HRV and depression have been reported controversial results. Similarly, the correlation between the serum lipids and depression is debatable. The purpose of this study is to examine the relationship between heart rate variability, lipid profile and depression. METHODS: A total of 42 patients with major depressive disorder (MDD) and 32 age and sex-matched normal subjects who had no previous history of major medical and mental illnesses were recruited for this study. A structured-interview was used to assess the general characteristics and psychiatric illness. HRV measures were assessed by time-domain and frequency-domain analyses. Psychological symptoms were measured using the Hamilton rating scale for anxiety (HAM-A), Hamilton rating scale for depression (HAM-D). In addition, the evaluation for lipid profile was performed by blood test. RESULTS: In serum lipid profile test, MDD group showed higher cholesterol (197.68+/-42.94 mg/dL vs. 176.85+/-34.68 mg/dL, p=0.044), TG (139.45+/-92.54 mg/dL vs. 91.4+/-65.68 mg/dL, p=0.018), LDL (130.03+/-33.18 vs. 106.62+/-27.08, p=0.004) level than normal control group. In HRV time domain analyses, the standard deviation of the NN interval (SDNN) was decreased in MDD group than normal control group, but was not significant (32.82+/-14.33 ms vs. 40.36+/-21.40ms, p=0.078). ApEn (Approximate Entrophy) was significantly increased in MDD group than normal control group (1.13+/-0.11 vs. 0.91+/-0.18, p<0.001). ApEn was correlated with LDL level (r=0.277, p=0.028), HAM-D scores (r=0.534, p<0.001) and HAM-A scores (r=0.470, p<0.001). CONCLUSION: MDD patients showed increased ApEn, one of the HRV measurement. And this ApEn was correlated with LDL, HAM-D and HAM-A scores. In this study, the analysis of ApEn would be a useful test of MDD.

Ipratropium bromide (IP) is a new anticholinergic bronchodilator. To evaluate its effect on bronchial asthma which is still unknown in Korea, a double blind and randomized study was done on all patients of bronchial asthma who visit out-patients clinic of out department from June to September 197 and showed 75 to 100% of FEV1/FVC ratio on prebronchodilator spirometry (pre BD). The selected patients were given 2 puffs of Fenoterol (FE) or Ipratropium inhalator blindly and Spirometry. The repeated results are: 1. In both FE and IP groups, there was a significant bronchodilatory effect on 5 and 60 minutes after administration. 2. One 5 minutes, effect of FE was significantly greater than IP (FVC p<0.05, FEV1 p<0.01). 3. One 60 minutes, effect of IP was slightly less than FE but statistically non-significant. On the basis of above results, we concluded that onset of effect of IP is slower than FE, but its effect is significant and nearly comparable to FE.
