Myositis Ossificans progressiva is a very rare disease and not more than five cases have been reported in this country. The symptoms usually appear before the age of six and no case has ever been reported in medical literature in which patient is older than the age of twenty. This author reports here that a 52 years old male was diagnoed as a myositis ossificans progressiva. The patient developed his symptoms when he was lynched by a club during the Korean conflict at the Korean conflict at the age of thirty. Another interesting fact is that is that his daughter has also developed similar symptoms which appeared following an accident when she was run over by an angry cow at the age of six.

3.Effect of Serotonin on Gastric Secretion in the Dog.

Heidenhain pouch secretion in response to small dose of serotonin was studied in conscious dogs. A single subcutaneous injection of 0.5 to 2.0 mg of serotonin produced no changes in spontaneous fasting secretion; however, the milk-induced secretion was greatly inhibited by the same dose. This inhibition was abolished by treatment of dibenzyline or LSD(d-lysergic acid die- thylamide). LSD alone enhanced the response of gastric secretion to milk. Constant intravenous infusion of serotonin, at levels of 3 to 10 microgram/kg/min was associated with a significant increase in the volume of gastric juice aspirated from three anesthetized dogs, but the acidity of juice varied very slightly. However, when histamine is given as much as 0.8 to 3 microgram/kg/min, a marked increase in both the volume and acidity was observed. A significant elevation of mucin content in the juice obtained from the Heidenhain pouch was seen in dogs receiving a single subcutaneous injection of 1.0 mg of serotonin. In case of histamine, the mucin content of pouch juice was not relatively increased and merely an increase in the total amount of mucin secondary to the volume increase was seen. The observed increase in mucin by serotonin was inhibited by LSD, BOL (2-bromo-d-lysergic acid diethylamide) or dibenzyline, and mildly by morphine. Atropine or hexamethonium did not block the response of mucin production to serotonin. The gastrointestinal motility elicited by serotonin was not affected by these agents. It is felt that the receptor(s) responsible for the mucin production in the dog belongs to the D-receptor types postulated by Gaddum and Picarelli.

4.The Management and Evaluation for the Urological Complications following Meningomyelocele.

With the advent of improved neurosurgical techniques and ventricular shunt, urological complications advanced to become the major obstacles to health and life quality for individuals with meningomyelocele. Herein, we reviewed 34 patients with neurogenic bladder secondary to meningomyelocele. The most common clinical presentation was urinary incontinence (70.6%). The most common location wag lumbosacral spine (53%). Uroradiological study showed hydronephrosis in 19 patients (55.8%), chronic pyelonephritis in 7 (20.6%), and vesicoureteral reflux in 11 of 26 (42.3%). According to the results of urodynamic study 24 patients were classified as follow , Group 1 ; low compliance and low pressure bladder with storage failure (16.7 %), Group II ;low compliance and high bladder pressure with emptying failure (58.3%), Group III, detrusor external sphincter dyssynergia with emptying failure (12.5 %),Group IV; atonic bladder (12.5%), Group II and III showed high incidence of upper tract dilatation and vesicoureteral reflux which seemed to be caused by high bladder pressure. The Group I and IV showed very low incidence of upper tract dilatation and vesicoureteral reflux. The most commonly applied treatment modality was urinary diversion. Recently 6 patients were treated by intermittent catheterization, which was effective in resolving upper tract dilatation and preventing febrile urinary tract infection. One patient underwent successful bilateral ureteral reimplantation. One patient underwent cutaneous vesicostomy. It seems that we should try to diagnose and treat high pressure group as soon as possible, and apply intermittent catheterization and antireflux surgery more frequently in patients with meningomyelocele.

6.Complications of Endovascular Detachable Coil Treatment in Cerebral Aneurysms.

OBJECTIVE: The purpose of this study is to evaluate complications in 70 patients who had underwent endovascular treatment to occlude cerebral aneurysms. METHODS: From May 1999 to December 2002, we treated 70 patients by endovascular treatment or by combination of endovascular treatment and surgery. Complications have been developed in fifteen patients. Twelve patients had anterior circulation aneurysms: 4 posterior commmuncating artery ; 2 anterior communicating artery ; 2 paraclinoid artery ; 2 anterior choroidal artery ; 1 distal internal carotid artery ; 1 middle cerebral artery ; 1 pericallosal artery and three patients had posterior circulation aneurysms: 3 basilar artery tip. RESULTS: The complications related to the coil embolization were the thromboembolic event in 9 cases, rupture of the aneurysm in 3 cases, coil prolapse in 3 cases and coil migration in 1 case. CONCLUSION: The coil embolization can be a alternative good modality in the treatment of cerebral aneurysms. But careful attention should be required to reduce the various complications of procedures and to improve the prognosis of aneurysmal treatment. Effort to overcome the technical problem and to develop more comfortable device are needed for the better results of endovascular treatment.

7.Differentiation of Tuberculous Enteritis and Crohn's Disease with Barium Study Using Bayes Theory.

PURPOSE: To differentiate tuberculous enteritis and Crohn's disease with barium study using Bayes theory. MATERIALS AND METHODS: The study group consisted of 34 patients with tuberculous enteritis (age range 16-86 years, mean age 43.3 years, M:F=19:15) and 36 patients with Crohn's disease (age range 19-78 years, mean age 35.2 years, M:F=18:18). These diagnoses were confirmed by therapeutic tests (tuberculous enteritis: 15, Crohn's disease:16) or histopathological examinations (tuberculous enteritis: 19, Crohn's disease: 20) conducted from January 1993 to May 2003. Three radiologists (two abdominal specialists and one trainee) analyzed each radiological finding of tuberculous enteritis and Crohn's disease by means of a barium enema and/or small bowel series. We used Fisher's exact test to verify the statistical significance of each radiological finding and p-values less than 0.05 were considered to be significant. We calculated the likelihood ratio (LR) of tuberculous enteritis versus Crohn's disease for each finding by employing Bayes theory. RESULTS: The radiological findings associated with a high likelihood ratio for tuberculous enteritis were the involvement of the cecum (LR=2.65) and ascending colon (LR=1.99), rigid narrowing (LR=1.94), shortening of the bowel (LR=1.99), haustral loss (LR=1.97) and sacculation (LR=3.88). The radiological findings associated with a high LR for Crohn's disease (low LR for tuberculous enteritis) were age between 20 and 29 years (LR=0.53), the involvement of the jejunum (LR=0.12), terminal ileum (LR=0.19), sigmoid colon (LR=0.30) or rectum (LR=0.17), and the presence of skip lesions (LR=0.19) or strictures (LR=0.21). With these LRs, the probability of the subject having tuberculous enteritis versus Crohn's disease could be calculated using Bayes theory. CONCLUSION: The analysis of a barium study using Bayes theory could provide an objective, easy and fast method of differentiating tuberculous enteritis and Crohn's disease.

8.Surgical Treatment of Instability of Upper Cervical Spine Associated with Trauma.

With the aim of determining the outcome of surgery, a review of patients undergoing trauma-associated atrantoaxial arthrodesis. Between 1993 and 1997, 16 patients underwent 19 proceedures, with a follow-up period of between six and 29 months. The most common reasons for surgery were odontoid fracture(n=12), os odontoideum(n=1) procedure, and neurofibrimatosis type I(n=1). Ten posterior wiring, four Halifax clamp application, one anterior screw fixation of dens, two transarticular screw fixations, and one staged operation(anterior odontoiddectomy and posterior occipitocervical fusion) were performed. All patients has been surgically managed for about three months with a Halo-vest or rigid cervical neck collar, during which time three complications associated with operative procedures arose : Halifax clamp dislodgement, malunion and subluxation kyposis. In 15 of 16 patients, fusion was successful. In caes involving complicated atlantoaxial dislocation, the authors recommend postoperative Halo-vest immobilization for sucessful fusion after posterior C1-C2 wiring or Halifax clamping

9.Metastatic lung cancer mimicking anaplastic thyroid cancer.

Anaplastic thyroid cancer is the most aggressive form of cancer found in humans. Usually it is easily diagnosed; however at times other diseases are mistaken for anaplastic thyroid cancer. We present a case of primary lung adenocarcinoma presenting with features that appeared as anaplastic thyroid cancer. The 43-year-old female patient was diagnosed with anaplastic thyroid cancer at a local clinic just before presenting to our hospital. At the clinic she had a neck node excisional biopsy and was informed of the diagnosis of anaplastic cancer. On admission to our hospital, very large bilateral thyroid masses, and lymphadenopathy involving multiple cervical lymph nodes was observed; therefore, we started chemoradiotherapy. The patient showed dramatic improvement and we began to think of other potential etiologies. A FDG-PET study showed increased uptake at the left lower lung area corresponding to a pneumonic consolidation; a TBLB was performed, and reported as poorly differentiated adenocarcinoma. We referred the patient to the oncology department. The patient died after two cycles of systemic chemotherapy.

10.Reliability study of 6-axis model surgery simulator for orthognathic surgery.

The purpose of this study was to evaluate the reliability of 6-axis model surgery simulator (6AMSS) for orthognathic surgery. A rectangular parallelepiped plastic block was assembled to model-mounting plate of 6AMSS. Left-right (X), anterior-posterior (Y), up-down (Z) translation and pitching (empty set X), rolling (empty set Y) and yawing (empty set Z) rotation was planned and performed using 6AMSS. The actual translation and rotation were measured with dial gauge and precisional protractor, respectively. Comparison between the planned and actual movements of plastic block for each variable were made using paired t- test. Statistical analysis for X, Y, Z, empty set X, empty set Y and empty set Z movement have shown no significant differences between planned and actual movement (P > 0.05). This indicate that model surgery performed with the aid of the 6AMSS is accurate in 3D translation and rotation. The 6AMSS is practically useful for accurate fabrication of surgical splint for orthognathic surgery.
