2.Two Cases of Bullous Dermatoses in Childhood.

We presented two cases of bullous dermatoses in childhood. The first case was a 13-year-old boy who had numerous tense bullae on the scalp, both forearms and dorsa of hands, trunk and both thighs in symmetrical distribution, but he had not any oral lesion. On histologic examination, subepidermal bullae containing many neutrophils and only a few eosinophils were demonstrated. The lesions began to subside prornptly since 2 days after ornl administration of prednisolone and there was no evidence of recurrence during the follow-up studies of 4 months. The second case was a 14-year-old boy who had erythematous macules, vesicles, bullae and crusts scattered irregularly on almost entire body surface and had some oral lesions. Histologically erythematous macular lesions revealed many microabscesses composed almost entirely oi neutrophils on the tip of dermal papillae but vesicular lesions showed intraepidermal bulla containing numerous eosinophils exclusively. His skin lesions were well suppressed by DDS but reappeared one or two days after cessation of DDS. We thought these two cases were not the typical cases of bullous pemphigoid or dermatitis herpetiforrnis. We did not perform immunologic studies, so we cannot assure that these two cases were belong to either bullous pemphigoid or dermatitis herpetiformis and the so-called benign chronic bullous dermatosis of childhood.

3.An Extensive Cutaneous Erythema associated with Vasculitis in a Patient with Systemic Lupus Erythematosus.

A 42-year-old woman with systemic lupus erythematosus (SLE) has had wide-spread erythemas with some purpuric patches on her trunk and upper extremities during exacerbation of her disease. Biopsy findings from erythematous lesions and purpuric areas revealed the pathology of lupus erythematosus and leukocytoclastic vasculitis (LCV) with immune deposits, respectively. A feature of wide-spread symmetrical erythemas with foci of LCV occurring in this patient is considered to be an unusual presentation.

6.Doppler Echocardiographic Measurement of Cardiac Output.

A noninvasive method for assessing cardiac output was evaluated by comparing it with thermodilution determinations in 25 patients who admitted to Pusan national university hospital from March, 1985 to December, 1986. This method used M-mode & two dimensional echocardiography to measure the internal diameter of aortic valve anulus & pulmonary valve anulus and pulsed doppler echocardiography to obtain aortic & pulmonary blood velocity. Good correlations were observed between thermodilution and doppler echocardiographic measurements of cardiac output from aortic flow (r=0.98, p<0.05) & pulmonary flow (r=0.86, p<0.05). Linear regression analysis yielded y=0.91x0.14 for aortic flow and y=0.77x0.84 for pulmonary flow. These results indicate that accurate cardiac output can be measured by noninvasive & simple doppler echocardiography.

7.Doppler Flow Patterns of Constrictive Pericarditis.

To recognize the hemodynamic change in the constrictive pericarditis, we have reviewed the Doppler echocardiography, cardiac catheterization, and pathology of 6 patients who were admitted to Pusan National University Hospital due to right-side heart failure, diagnosed as constrictive pericarditis and were undertaken pericardiectomy. Doppler echocardiographic findings showed that acceleration and deceleration of early diastolic rapid filling were increased, followed by shortening of duration but there was a tendency to decrease in velocity-time integral of early diastolic rapid filling compared to that of atrial contraction filling in the left ventricle. On the contrary, there was decrease in acceleration of rapid filling in right ventricle, but other indices were comparable to that of left ventricle. The integral of D wave increased relatively compared to that of S wave in superior vena canal flow. Also, there was decrease in peak flow velocity, acceleration and velocity-time integral of aortic and pulmonary arterial flow velocity. One patient who had increased fraction of integral of early diastolic rapid filling compared to that of atrial contraction filling in right ventricle showed that he had higher central venous pressure and D wave was dominant in superior vena caval flow. In conclusion, Doppler flow patterns showed characteristic diastolic filling inpairment and systolic dysfunction in constrictive pericarditis, so that Doppler echocardiography is believed to be a useful method of noninvasive diagnosis and follow-up of hemodynamic change in constrictive pericarditis.

8.Dyspeptic Symptom Severity Related to Helicobacter pylori and Stress in Functional Dyspepsia.

BACKGROUND: It is well-known that dyspeptic symptoms in functional dyspepsia are related to psychological factors such as stress and mood disorder. But it is still controversial whether H. pylori infection is a important factor for functional dyspepsia. We conducted this study to investigate whether H. pylori infection or stress are related to dyspeptic symptom severity in functional dyspepsia. METHODS: From Mar. 1 1999 to Apr. 30 1999, attendees to a Health Screening Center and a Family Practice Center were eligible study subjects. The subjects were responded to questionnaire including dyspeptic symptom severity items and BEPSI-K(stress scale). And among eligible subjects, one hundred and thirty two diagnosed with functional dyspepsia by gastrofibroscopy were final study subjects. H. pylori infection was confirmed by antral biopsies & smear (Giemsa stain). RESULTS: In functional dyspepsia, high BEPSI-K score group(upper tertile) complained more severe dyspeptic symptom severity than low BEPSI-K score group(lower tertile) after adjusted for other variables(adjusted OR=8.389, 95% C.I.=2.232-31.231). But the dyspeptic symptom severity was not associated with H. pylori infection status (adjusted OR=1.514, 95% C.I.=0.567-4.046). CONCLUSIONS: The dyspeptic symptom severity in functional dyspepsia was associated with stress but not with H. pylori infection status.

9.Health-related quality of life in people with overweigh and large waist circumference.

BACKGROUND: The relationship between abnormal body fat distribution and increased risks of morbidity and mortality is well established. Less is known about the impact on functional health status and subjective well-being. This study quantified the association between body fatness and health-related quality of life. METHODS: Health-related quality of life was measured by Catholic Medical Center Health Survey(CMCHS) V1.0. Tertiles of body mass index(BMI) and waist circumference in 484 adult persons visiting a university hospital-based Health Screening Center were compared. To estimate the relative risks of poor health in those with high BMI and large waist circumference, odds ratio and 95% confidence interval(CI) were determined by logistic regression analysis. RESULTS: Compared with the reference group (subjects with waist circumference in the lowest tertile), the odds ratios for men with waist circumference in tertile 3 were 2.24 (95% CI:1.15-4.36) for social function, and 2.5(95% CI:1.02-6.11) for change in health. Women in tertile 3 had odds ratio of 3.23(95% CI:1.05-9.97) for bodily pain. Statistical significance was not shown in other domains. The odds ratios of subjects with the largest BMI, compared with those in the lowest tertile, were not statistically significant in 10 domains. CONCLUSION: Large waist circumferences affects quality of life in the domains of social function and change in health for men, and bodily pain for women.

10.A Case of Allergic Contact Dermatitis to Cephalosporins.

Cephalosporins are B-lactam antibiotics. They are usually bactericidal in action and act by inhibiting mucopeptide synthesis in the bacterial cell wall. Cephalosporins have been used widely in Korea. However, allergic contact dermatitis to cephalosporins has not been reported in the Korean dermatologiy literature. We report a case of allergic contact dermatitis due to cephalosporins in a 23-year-old nurse who suffered from itchy, erythematous patches and plaques with numerous fissures on both hands. A patch test with ceftriaxone and a prick test with cefotiam were positive.
