1.A Comparison of UVB-induced Minimal Erythema Dose ( MED ) s to the Skin of the Back and Extremities in Young Adult Koreans.

BACKGROUND: The erythemal response of the skin to UVB radiation is used as a diagnostic phototest and guideline to phototherapy. OBJECTIVE: The purpose of this study was to compare the UVB-induced MEDs to the back, arm, and thigh skin METHODS: A sunlight fluorescenct lamp(Waldmann UV 7001K) was used as a UVB radiation source. The back, arm, and thigh skin were irradiated with the dose, from 40mJ/cm2 to 180mJ/cm2. The minimal doses for erythema responses to the skin were assessed visually at 24 hours after irradiation. RESULTS: MEDs of the back, arm and thigh skin were 92.6 +/- 17.3mJ/cm2(mean +/- S.D.), 123.0 +/- 24.2mJ/cm2, and 126.6+/- 28.3mJ/cm2, respectively. The most frequent MED was 100mJ/cm2 for the back skin and 120mJ/cm' for the arm skin and thigh skin. CONCLUSION: In this study, UVB-induced MEDs to the back, arm, and thigh skin in young adult Koreans were assessed. A significant difference in the MED was found between the back and extremities skin, with a lower value for the back skin(92.6 +/- 17.3mJ/cm2) than for the arm skin(123.0 +/- 24.2mJ/cm2) or for the thigh skin(126.6 +/- 28.3mJ/cm2).

2.Clinical analysis of dissecting aortic aneurysm in emergency department.

Dissecting aortic aneurysm is an emergency condition which requires prompt diagnosis and management. From January 1992 to December 1996, 54 patients were admitted to our department. Patients were categorized according to the Stanford classification of aortic dissection and investigation of the clinical records utilized the retrospective method. Male constituted the majority, according for 38(70.4%) of the total number of patients, whereas females accounted for 16(29.6%). Mean age was 52.1+/-2.2 years old (range:25~82). Clinical findings included chest pain in 46 cases(85.2%), renal failure in 11(20.4%), aortic insufficiency in 10(18.5%) and stroke in 9(16.7%). Predisposing factors were hypertension in 48 cases(88.9%), Marfan's syndrome in 5(9.3%) and diabetes mellitus in 1(1.8%). 21 patients(type A:12, type B:9) underwent surgical treatment. There were three(2 in type A and 1 in type B) deaths in the surgical treatment group and nine(5 in type A and 4 in type B) deaths in the medical treatment group. These results reflect our current policy in the management of dissecting aortic aneurysm.

3.Pitalls in Interpretation of Physical Tests of Knee Ligament Injury

The significance of the physical examination is controversial since Slocum and Larson first described the concept of rotatory instability of the knee. The findings of the physical examination of the 101 knees of 95 patients from 1982 to 1985 were compared with the operative findings. The results were as follows. 1. Valgus stress test a) The valgus stress test c the knee in extension was positive in 1) combined tear of medial collateral ligament, posterior oblique ligament, either anterior or posterior cruciate ligament, 2) avulsion fracture of medial collateral ligament and 3) extensive tear of medial capsular ligament. But the test was negative when the posterior oblique ligament was intact although the medial collateral ligament and the anterior cruciate ligament were torn. b) The valgus stress test c the knee in 30° flexion was positive in all cases of torn medial collateral ligament, But it was severer in cases of positive test in full extension. 2. The varus stress test in extension was positive when the anterior or posterior cruciate ligament was torn in addition to lateral collateral ligament and posterolateral capsule. 3. The Slocum test was negative, or positive only in neutral rotation in isolated tear of the anterior cruciate ligament. 4. The Ritchey-Lachman test was positive in all cases of torn anterior cruciate ligament. 5. The posterior drawer test could be positive in cases of intact posterior cruciate ligament. 6. The pivot shift test was negative in cases of torn medial collateral ligament and posterior oblique ligament although there was associated tear of the anterior cruciate ligament. 7. The flexion rotation drawer test was positive in all cases of torn anterior cruciate ligament although there was associated tear of the medial collateral ligament. The positive test didn't always mean anterolateral rotatory instability, 8. The reversed pivot shift test was positive in cases of posterolateral rotatory instability but it was negative in cases of associated tear of posteromedial capsule. 9. The drawback of external rotation drawer test was that it depended on subjective evaluation. 10. The rotation test was very good for the rotatory instability but it could not be used in bilateral injury and the anterolateral rotatory instability of isolated tear of anterior cruciate ligament.

4.The Treatment of Infected Nonunited Fractures of Long Bone

Many difficult problems are encountered in the treatrnent of infected nonunion of long bone. To solve these difficult fractures, authors adopted an active approach-thorough debridement of the infected and necrotic soft tissue and bone, rigid internal fixation with autogenous cancellous bone graft and opening of the wound to ensure continuous drainage until union of the fracture. Authors treated 28 cases of infected nonunions with this method from Mar. 1980 to Feb. 1989, and the results are as follows ;1. Femur(54%) and tibia(43%) were most frequently involved sites. 2. Most of the cases(86%) were treated by rigid internal fixation and cancellous bone graft. 3. In most cases(64%), infection was controlled spontaneously by the time of bony union, but in 5 cases which showed persistent drainage, implant removal controlled the problem, and residual soft tissue and bony defect were solved by Papineau's technique or musculocutaneous flap. 4. Revision was required in 5 cases(18%) because of insercure fixation. 5. Union could be obtained in average 4.8 months, and infection could be controlled in all except 1 case.

5.A Clinical Study of Arthroscopic Meniscectomy

Ninety cases of meniscus injury were treated by arthroscopic meniscetomy between March, 1988 and March, 1992. Eighty four cases were treated by arthroscopic partial, 5 cases by subtotal, and 1 case by total meniscectomy. All cases were followed up for more than 1 year. The results were as follows. 1. The ratio of male to female was 57 to 31, and that of right to left was 52 to 38. 2. There were 41 cases of medial meniscus and 49 cases of lateral meniscus. Longitudinal tears were 35 cases (39%), oblique tears were 20 cases (22%), horizontal tears were 15 cases (17%), and radial tears were 9 cases (10%). There was no difference in the shape of tear between medial and lateral meniscus. 4. The most common associated injury was anterior cruciate ligament tear. 5. The Tapper and Hoover scale was used to record the clinical results, and satisfatory results (exellent and good)were in 86% of cases.

8.Vacuum Arthrography of the Hip Joint

It is possible to create a diastasis in the hip by traction. The space thus created between articular surfaces can be visualized radiographically and is known as “a vacuum phenomenon”. Twenty one cases of unilateral hip joint disease were studied with vacuum arthrography from Oct. 1983 to July 1984 in Catholic Medical College and Center. The patients ages ranged from a month to 53 years of age but most of them(18 out of 21 patients) were below 10. There were 5 female patient's and 16 male patients in this series. The results were as follows: 1. In the absence of an effusion in all normal hip joints the vacuum phenomenon was demonstrated. 2. In the absence of an effusion in the hips of a transient synovitis, 4 Legg-Calve-Perthes disease and 3 sprain the size of vacuum was normal. 3. The size of vacuum of 3 transient synovitis and a pyohip of healing stage which had only mild effusion was smaller than those of normal hips. 4. In the presence of large amount of effusion in the hips of 5 transient synovitis, 2 pyogenic arthritis and 2 tuberculous arthritis it has not been possible to produce the vacuum phenomenon. 5. This study, in summary, shows that the size of vacuum represents the amount of effusion. 6. Non-ossified femoral head in the new born, articular surface of acetabulum and thickness & local changes in the cartilage of the femoral head could be demonstrated using the vacuum arthrography. 7. It has not been possible to demonstrate the articular surface lateral to the limbus, ligamentum teres and limbus in the vacuum arthrogram.
