1.Metastatic Large Cell Neuroendocrine Carcinoma of the Lung Mimicking a Merkel Cell Carcinoma.

Large cell neuroendocrine carcinoma (LCNEC) of the lung is a newly recognized entity of pulmonary neuroendocrine carcinoma. Histologically, it is very difficult to differentiate LCNEC from other pulmonary carcinomas and the prognosis is significantly poor. The cutaneous metastasis of LCNEC of the lung shares some features with Merkel cell carcinoma of the skin in light microscopy and yet it is negatively stained with cytokeratin 20. We report a case of cutaneous metastasis of LCNEC of the lung, previously misdiagnosed as squamous cell carcinoma. Our patient showed a poor response to the chemotherapy and also revealed a brain metastasis on follow-up brain CT scan.

6.A Case Report of Human Thelaziasis.

The authors preEented the 6th human thelaziasis case in Korea with the review of the literatures. The patient was 18 years old high-school girl who had lived in Seoul since birth. She experienced epiphora and moving foreign body Eensation of the right eye since one week prior to examination, and extracted 2 living worms from the right eye by herEelf on 1 and 2 days prior to examination. Morphological features of an extracted worm are as follows; Gross appearance of the worm is slender milky, white nematode, and microscopic measurements are 12.51mm in length. 0.34mm in maximum width, 15.5micro in depth of oral cavity, 0.667mm in length of the esophagus, 180~220 cuticular striations per mm, anal-opening on 0.088 mm from posterior end, and vulvar-opening on 0.544mm from anterior end. From the above findings it was confirmed as female worm of Thelazia callipaeda Railliet and Henry 1910.

Spontaneous coronary artery dissection is a rarely identified entity whose exact incidence, etiology, pathogenesis, medium-term evolution and optimal treatment have not yet been firmly estabilished. The cause of spontaneous disection remains unclear but theories of etiology include a medial eosinophilic angiitis, pregnancy induced degeneration of collagen and rupture of the vasovasoum. Most paients die suddenly, but a clinical spectrum is seen including and unstable angina, myocardial infarction and cardiogenic shock. We experienced 4 cases with spontaneous coronary artery dissection found angiographically which caused myocardial infarction and unstable angina. Our patients were treated medically.
