Although ventricular pacing alone initially had deemed adequate for most clinical situations, some patients did not do well after ventricular pacing was initiated, and developed various symptoms attributed to this mode of pacing. The pacemaker syndrome is complex of clinical signs and symptoms related to the adverse hemodynamic and electrophysiologic consequences of ventricular pacing in the absence of other causes. Neurologic symptoms or those congestive heart failure predominated. We recently experienced a case of pacemaker syndrome in a 44-year-old female who had suffered sick sinus syndrome and was implanted with dual chamber pacing system being programmed to VVI pacing. She complained of chest discomfort, dyspnea, and near-fainting in a day after being programmed to VVI. Blood pressure was decreased to 9/60mmHg. Electrocardiography showed toPwave onT wave, representing retrograde ventriculoatrial conduction. The symptoms and signs were disappeared immediately after the pacing system was programmed to DDD pacing.

4.Analysis of 32 cases of Krukenberg Tumors.

OBJECTIVE: The Krukenberg tumor is a metastatic or primary malignant ovarian tumor of the signet-ring cell type. The incidence of this tumor is higher in Korea than in western, among relatively young age group with poor prognosis. We evaluated the clinical characteristics of the Krukenberg tumor (metastasized from stomach cancer) and the relationship between the survival and clinical characteristics (including treatment modalities). METHODS: We analyzed 32 cases of the Krukenberg tumor metastasized from stomach cancer at the Department of Obstetrics and Gynecology, Kosin University Gospel Hospital. RESULTS: Among the 32 cases of the Krukenberg tumor, the age distribution ranged from 22 to 58 years, and the mean age was 38.8 years. The most common chief complaint was lower abdominal pain(11 cases). The majority of the stomach cancer showed serosal invasion and was signet-ring cell type in histology. The mean survival was 20 months. In patients with ascites(22 cases), the mean survival was 16 months in contrast to 25 months in ascites-free patients(10 cases). In the chemotherpy group, the mean survival was 23 months in contrast to 16 months in chemotherpy-free group. Also, longer mean survival(25months) was shown in cases with optimal surgery group than non-optimal surgery group. CONCLUSIONS: The survival of krukenberg tumor was shown to be related to the surigcal resectability and chemotherpy modalities.

5.MR Imaging Findings of Avulsion Fracture of the Tibial Spine of the Knee, Focusing on Cruciate Ligament Tear.

PURPOSE: To determine the presence of cruciate ligament tears following avulsion injuries involving the ACL and PCL, and to correlate the findings with those of surgery. MATERIALS AND METHODS: Between March 1997 and May 2002, avulsion injury involving the ACL or PCL was diagnosed in 19 patients. Ten of these [8 males and 2 females aged 10-51 (avergae, 27.7) years] were included in this study. We assessed the presence of cruciate ligament tears at MR imaging, correlating the findings with those of surgery. Associated intra-articular injuries, treatment methods and follow-up results were also evaluated. RESULTS: Among Seven patients with ACL avulsion injury, this was assessed at MR imaging as complete tear (n=1), partial tear (n=5), or intact (n=1), while all MR images of PCL avulsion injury (n=3) showed that this was partial tear. All imaging findings corresponded with the surgical findings. In four patients there was associated knee injury involving, respectively, tears of the medial meniscus, lateral meniscus, PCL and MCL, and popliteal ligament. CONCLUSION: Our findings showed that with one exception, patients with avulsion injury of the ACL or PCL had suffered either a partial or complete tear. MR imaging may be useful in the diagnosis of tears of the cruciate ligament which have not been noticed at surgery or arthroscopy in avulsion injuries involving the ACL and PCL.

6.A Case of Asymmetric Septal Hypertrophy Combined with Conn's Syndrome.

We report a case of a 44 year old femele with unilateral aldosterone-proudcing adrenal adenoma characterized by hypertension, plasma aldosterone excess, and low plasma renin, commonly but not invariably with hypokalemia. She also had asymmetric septal hypertrophy of left ventricle established with two-dimensional echocardiography. The electrocardiogram showed inverted T wave and prominent U wave with high QRS voltage on precordial leads. In the case of this patient, we are not sure whether asymmetric septal hypertrophy was caused by secondary hypertension and chronic aldosterone excess of primary aldosteronism, or hypertrophic cardiomyopathy per se, so further long=term follow-up is required to determine it. Following the successful unilateral adrenalectomy, however, the systemic pressure fell down to the normal level and electrolyte abnormalities were corrected immediaterly within a few days and the modest regression in septal hypertrophy was noted in one year, suggesting that the promary aldosteronism contributes to the development or porgression of asymmetric septal hepertrophy.

8.Chemical Analysis of Glucose Concentration in Aqueous Humor in Diabetic Cataract Patients.

It is difficult to analyze the chemical compositions of aqueous humor because of the impossibility of collection of aqueous humor in normal situation, lack of analyzing technique, limited amount of aqueous humor. We measured the glucose concentration of aqueous humor both in cataractous patients with diabetes mellitus and in cataractous patients without diabetes mellitus, thus we compared and analyzed glucose concentration in aqueous humor and correlation between blood glucose con centration and aqueous humor glucose concentration in cataractous patients with diabetes. The mean concentration of glucose in aqueous houmor in diabetics was 92.81 +/- 35.58mg/dl and that of non-diabetics was 53.62 +/- 34.42mg/dl. Aqueous glucose concentration in diabetics was statistically significantly higher than that in non-diabetics.

To evaluate the cause of intraocular pressure elevation and the effect of indomethacin after Nd:YAG laser anterior capsulotomy in rabbit authors measured the intraocular pressure, prostaglandin E2 and thromboxane B2 concentrations of aquous humor, and observed the morphological change of trabecular meshwork using scanning electron microscope in indomethacin pretreatment (group I) and non-treatment (group II) group. The intraocular pressure and the concentrations of prostaglanding E2 and thromboxane B2 in group II were higher than in group I. And the scanning electron microscopy showed inflammatory reaction in group II. These results support that the causes of intraocular pressure elevation are elevation of prostaglandin E2. thromboxane B2 concentrations in aqueous humor and intraocular inflammation secondary to it.
