PURPOSE: The chemoembolization with Lipiodol and doxorubicin hydrochloride is used in patients with hepatocellular carcinoma. What condition is the ideal emulsion of Lipiodol and doxorubicin for excellent anticancer effect? METHOD AND MATERIALS: Microscopic evaluation was performed on the emulsions, which were varied with different specific gravities of doxorubicin solutions, degrees in mixing of the emulsion, and amount of Lipiodol. RESULT: 1. Maximal amount of doxorubicin solution was contained in Lipiodol droplets and the release of doxorubicin from the droplets were delayed, when specific gravity of doxorubicin was equal to that of Lipiodol (SG, 1.28). 2. The optimal therapertic ratio of Lipiodol and doxorubicin was 3:2 at least, as in the emulsion less than 3:2, unmixed free forms of doxorubicin solution were increased. 3. The emulsion mixed by pumping 50--100 times had smaller Lipiodol droplets and contained larger amount of doxorubicin solution in the droplets than by pumping 20 times. CONCLUSION: We recommend the emulsion with specific gravity of doxorubicin equal to Lipiodol (SG. 1.28), the ratio of Lipiodol and doxorubicin closo to 3:2, and the mixture prepased with puming 50--100 times.

The purpose of this study was to elucidate the prevalence rate of tbe concomitant; atopic diseases in the patients with allergic rhinitis and, concurrently, the incidence of the atopic diseases in the families of the patients. A total of 40 patients who were referred to the department of dermatology, Kang Nam St. Marys Hospital, for the confirmative examinations of allergic srhiniti were taken for this study. The results were as follows, 1. The prevalence rate of concomitant atopic dermatitis in the sample patients was 22.5% and that of concomitant bronchial asthma was 12. 5% The prevalence rate of concomitancy with both diseases was 2.5% and that of concomitancy with atopic dermatitis and/or bronchial asthma was 37.5%. 2. The frequency of the associated manifestations, in descending order, was allergic conjunctivitis (30%), urticaria or angioedema (25%), insect sting (22.5%), oral symptoms (5%), migrane or headache (5%), immediate drug reactions (2.5%) and gastrointestinal symptoms (2.5%).

The goal of fracture treatment is to restore the patient to pre-injury function level. But the outcome assessment after orthopedic interventions has usually focused on physician-defined parameters of technical success, such as fracture union, alignment and range of motion rather than patient function and quality of life. However, the correlation among improvements in these parameters and functional status, psychosocial well being is inconsistent and weak in the elderly. Therefore authors assessed the functional status after surgical intervention of hip fractures in the elderly with functional recovery score(FRS). Independence in basic activities of daily living, in instrumental activities of daily living, in mobility, freedom from pain and intact memory were assessed in 133 hip fractures in the elderly. The hip fractures in the elderly resulted in 24.8% loss of function after the first year. The older the age, the more the loss of function after surgery of hip fractures. The more functional loss was observed in female patients and in patients with poor postoperative radiological results, but statistically insignificant (P>0.05). The more functional loss was observed in patients with underlying medical disease preoperatively and it was statistically significant (P<0.05). We recommend the evaluation of functional status in the elderly after hip fractures.

The 35 patients of femur neck fracture treated with bipolar hemiarthroplasty were followed up for an average of 18 months with the range of 12 months to 5 years after bipolar hemiarthroplasty from Jan. 1986 to Dec. 1992. 1. Mean age of the patient was 68 years old(25-93) and mean hospitalization period was 55 days. 2. Associated medical problems were diabetes mellitus in 6 cases, cerebrovascular disease in 5 cases, rectal cancer in 1 case and pulmonary tuberculosis in 1 case. 3. An anterior dislocation, one of the early complication occured in 1 case, and superficial infection in 3 cases(8.5%), a deep infection, one of the late complication occured in 1 case. 4. The clinical result was focus on the functional hip assessment of bipolar hemiarthroplasty after femur neck fracture, and the functional results evaluated by Lunceford's Method were excellent in 22 cases(63%), good in 5 cases(14.3%), fair in 5 cases(14.3%), poor in 3 cases(8.6%). 5. The bipolar hemiarthroplasty was satisfactory method for the displaced femur neck fracture in the elderly patients, but to return the patient's preinjury activity level, more active postoperative rehabilitation should be required.

Radiological manifestations on plain films of the abdomen in 75 cases with proven typhoid perforation of distal ileum, in Kyungpook National University Hospital between Oct. 1972 and Sep. 1981, were analyzed and discussed. Some clinical and laboratory findings which may be helpful in diagnosis of this condition were also analyzed. Typhoid perforation was occurred most frequently in those between 2nd and 4th decades, and the male to female ratio was 4:1. Typhoid perforation was occurred most frequently in summer, but was occurred in all seasons. Radiological findings were free intraperitoneal gas (64%), loss of preperitoneal fat line (62.7%), free abdominal fluid (53.3%), distention of gas-filled bowel (94.7%), elevation of diaphragm (8%) and fixation of small bowel segments in right lower abdomen (43.7%). We concluded that a large amount of intraperitoneal gas with gas-filled distention of small bowel and findings of peritonitis are highly suggestive of typhoid perforation when the patient has acute abdomen.

The effect of triamcinolone acetonide(TA) on the expression of Gl related genes was investigated the cultured keloid fibroblast. The addition of TA to the culture medium resulted in growth inhibition of keloid fibroblast. TA reduced the expression of cyclin A, B, E and cyclin dependent kinase(CDK) 2 mRNA, but unexpectedly, the expression of cyclin C, Dl and CDK4 mRAN was not affected significantly as compared with those of normal fibroblast. Expressions of p16, p21 and p27, the wellestabilished CDK-inhibitors, were also investigated. The level of p16 was not detected in both normal and keloid fibroblasts and the expression of p27 was significantly decreased in keloid fibroblast. The expression of p21 was dramatically increased in keloid fibroblast but not significantly changed in normal fibroblast. Also the expressions of p53 and pRb, the well known tumor suppressor genes, were increased by the addition of TA. These data suggested that the observed growth inhibitory effect of TA may be related to transcriptional inactivation of cyclin A, B, E and CDK2 and to the transcriptional activation of p21, but the mechanisms of unchanged expression of cyclin C, Dl and CDK4 mRNA remain to be elucidated.

Authors retrospectively analyzed the CT findings of 102 cases of histologically proven bronchogenic carcinomaduring last 4 years from January 1980 to July 1984 at Kosin Medical College. The results were as follows; 1. Thesex ratio was 86 males to 16 females and the greatest number (66.7%) of cases were seen in fourth and fifthdecades. 2. The distribution of histoligic types of primary lung cancer as follows: squamous cell carcinoma 66cases, Adenocarcinoma 10 cases, small cell carcinoma 7 cases, Large cell carcinoma 5 cases, bronchiloalveolar cellcarcinoma 1 case, Unclassifed 13 cases. 3. Location of primary lesions as follows: Right lung 61 cases, Left lung40 cases. In both lungs, the greatest number of cases were found in the upper lobes. Ratio between central andperippheral mass was 2.5:1, except adenocarcinoma(6:4). 4. CT findings were as follows; Hilar or central mass(75cases), Peipheral mass(26), Bronchial abnormalities such as narrowing, obstruction, or displacement (60),Thickening of the posterior wall of the right upper lobe bronchus, bronchus intermedius, or left mainbronchus(17), Post-obstructive changes; Atelectasis, Pneumonitis, Emphysema(34, 17, 1 respectively), Hilaradenopathy(21), Mediastinal lymph node enlargement(50). Mediastinal invasion(51), Pericardial thickening(5), SVCsyndrom with collateral vessels(3), Pleural effusion (27), Pleural thickening or invasion(14), Chest wallinvasion(2), distant metastasis(26). 5. In most of patients(92 cases), the size of mass above 3cm, but in 9 casesbelow 3cm. Margins of the masses were serrated or lobulated in most cases. In 5 cases, cavitary formations werenoted, walls of which were thick and irregular, and air-fluid level was noted in 1 case. In 2 cases, eccentriccalcification were noted within mass. 6. Among 51 cases of whom direct mediastinal invasion was suspected, 8 caseswere operated upon, and this revealed that the masses were not resectable. Among the patients in whom no direct mediastinal invasion was suggested, 12 cases were operated uppon, and this revealed that the masses wereresectable in all cases. 7. Staging was as follows: stage I, 2 cases, Stage II, 13 cases, stage III, 86 cases.

PURPOSE: Whereas cyclosporine is increasingly used not only in transplantation but also in autoimmune disorders, it may be associated with several side effects. Gingival hyperpalsia is one of the most frequent side effects and has been estimated to occur in 21-30% of all patients receiving the drug and may require surgical correction. Azithromycin is a macrolide antibiotics reported to coincidentally reduce gingival hyperplasia in renal transplant recipients treated for respiratory infections. To confirm the effect of azithromycin in cyclosporine induced gingival hyperplasia we tried clinical use of azithromycin in renal transplant patients with severe gingival hyperplasia. METHODS: Patients (n=9) with cyclosporine induced gingival hyperplasia were selected and took azithromycin for 5 days. Follow-up visits were conducted at week 4 and week 8. Changes in gingival hyperplasia were evaluated by measuring the ratio of clinical crown height and width in each of the four central incisors. RESULTS: Significant improvements were observed in periodontal measurement. The pre-treatment ratio of clinical crown height and width was 1.04+/-0.14 and it was increased to 1.17+/-0.16 in 4 weeks and 1.22+/-0.15 in 8 weeks. 88.9% (8/9) of patients reported an improvement in clinical symptoms. Azithromycin was tolerated and 77.8% (7/9) of patients reported that the treatment was at least somewhat useful. CONCLUSION: Azithromycin treatment for cyclosporine induced gingival hyperplasia is efficacious, cost effective, and has less morbidity compared with gingivectomy.

PURPOSE: The incidence of adenocarcinoma of the uterine cervix is low. Traditionally, Low Dose R ate (LDR) brachytherapy has been used as a standard modality in the treatment for patients with carcinoma of the uterine cervix. The PURPOSE of this report is to evaluate the effects of the High dose rate (HL)R) brachytherapy in the patients with adenocarcinoma of the uterine cervix compared with the LDR. MATERIALS AND METHODS: From January 1971 to December 1992, 106 patients of adenocarcinoma of uterine cervix were treated with radiation therapy in the Department of Radiation Oncology, Yonsei University with curative intent. LDR brachytherapy was carried out on 35 patients and 7 1 patients w -re treated with HDR brachytherapy. In LDR Group, 8 patients were in stage I, 18 in stage II and 9 in st;ge III. External radiation therapy was delivered with 10 MV X-ray, daily 2 Gy fractionation, total dose 40-46 Gy (median 43 Gy), And LDR Radium intracavitary irradiation was performed with Henschke applicator, 22-59 Gy to point A (median 43 Gy). In HDR Group, there were 16 patients in stage I, 38 in stag <, II and 17 in stage III. The total dose of external radiation was 40-61 Gy(median 45 Gy), daily 1.8-2.0 3y. HDR Co-60 intracavitary irradiation was performed with RALS (Remote Afterloading System), 30-57 Gy (median 39 Gy) to point A, 3 times a week, 3 Gy per fraction. RESULTS: The 5-year overall survival rate in LDR Group was 72.9%, 6 1.9%, 45.0% in stage I, II, III, respectively and corresponding figures for HDR were 87.1%, 58.3%, 41.2%, respectively (p>0.05). There was no statistical difference in terms of the 5-year overall survival rate between HDR Group and LDR Group in a denocarcinoma of the uterine cervix. There was 1 1% of late complication rates in LDR Group and 27% in HDR Group. There were no prognostic factors compared HDR with LDR group. The incidence of the late complication rate in HDR Group stage II, III was higher than that in LDR Group ( 16.7% vs. 31.6% in stage II, 11. 1% vs. 35.3% in stage III, p>0.05). Although the incidence of radiation induced late complication rate was higher in HDR Group stage II and III patients than that in the LDR Group, statistical significance was not detected and within acceptable level. CONCLUSION: There was no difference in terms of 5-year survival rate and failure pattern in the patients with adenocarcinoma of the uterine cervix treated with HDR and LDR brachytherapy. Even late complication rates were higher in the HDR group it was an acceptable range. This retrospective study suggests that HDR brachytherapy seems to replace the LDR brachytherapy in the adenocarcinoma of the uterine cervix. However, further studies will be required to refine the dose rate effects.
