2.Crural Ulcer After Accidental Application of 5 - Fluoroutacil Cream.

Topical fluorouracil therapy is an advanced treatment for actinic keratoses, Bowen's disease, multiple basal cell epithelioma and other precancerous skin lesions. Because of extensive and frequent use of 5-fluorouracil(5 F-U) cream, some untoward effects, mostly allergic contact dermatitis and irritation dermatitis, can be anticipated. We have experienced a case of crural ulcer after accidental application of 5 F-U cream for treating the tinea cruris at drug store. The patient was 21 year oid man who had suffered from erythema, erosion and shallow ulceration with oozing, serosanguinous discharge, buming and stinging sensation on the both groins and its adjacent area of the scrotum. The skin lesion was not well responded to topical corticosteroid but controlled by wet dressing and zinc oxide lotion within 10 days of hospitalization. At the 20th hospital day, the skin was healed with remained follicular hyperpigmentation. A few days later, the tinea cruris was confirmed by direct microsopic examination and culture. Generally, topical 5 F-U spares normal cell, while selectively destroying the more rapidly proliferating celI, but there are some possible factors to induce destruction of non-proliferating cell in the highly penetrating area The skin penetration can be different depending upon the anatomical location.

3.Quantity and Quality of White Cells and Bacteria in the Urine of Urinary Tract Infections.

In infections of the urinary tract resulting from bacterial invasions, pus cells are most commonly present in those urine, and bacteria and pus casts may also exist. Campbell considers that there are five pus cells per high power field are within normal limits in the uncentrifuged catheterized urine. On the other hand Helmholz says two to eight pus cells may exist in a normal specimen. It is of no moment if the leukocytes are single or clumped: The important thing is to demonstrate the polymorphic or pawnbroker's nucleus in the pus cells, and desquamated epithelial cells or other urinary debris should not be considered as pus cells. Alto most accurate counting of those should be made with a counting chamber in high power field. It is called bacteria urine if any kind of bacteria exists in the urine with or without leukocytes, and both bacteria urine and pyuria are most significant clinical signs for the diagnosis of urinary infection. According to Lancet (1964) when 100, 000 or more organisms are presentper milliliter of urine, true infection. are actually present and therefore one might assume if there were less than 100,000 organisms per milliliter that thereare no infections, but no one has yet defined what is the normal upper level of white cells in the urine of Korean people. The purpose of this paper is to study what is the normal upper level of white cells and bacteria in the urine of Koreanpeople. MATERIALS AND METHODS: Sixty patients with urinary tract symptoms were assessed. Forty of the subjects were women. A selected control group of twenty adults was composed of twelve female and eight male, none of whom bad ever had urinary infection. Urine specimens were obtained by midstream collection comparing with catheterization. White cell count of the urine was estimated by Thomas white cell counting chamber, while those specimens were not centrifuged, and bacteria count was performed by a standard drop plate method. Steroid provocation test wee based on the procedure described by Pears and Houghton, and 50 mg of prednisolone was given by mouth. RESULTS: 1. In the collection of urine specimens, the midstream collection was more convenient and harmless compared with the results of catheterization. 2. When there were less than five cells per c.mm. there was no evidence of urinary tract infection on those specimens. When there were more than 10 cells per c. mm. there were true infections in those subjects. 3. When there were more than 100, 000 organisms per milliliter there were true infections in those subjects. There were less than 100, 000 organisms per milliliter even in the control group. 4. In steroid provocation test, there were carried out in 20 controls and in 60 patients, the positive test was a rise in white cell count or bacteria count together of at least 100 percent in those 60 patients, but no response in those 20 control group.

5.A Case of Nevus Unius Lateris with Retroperitoneal Leiomyosatcoma.

Epidermal nevus syndrome, which used to combine congenital abnormalities of the central nervous and skeletal system and others, are not ingrequently encountered. Until now fer cases of epidermal nevus with intemal malignancy were reported in the literatures, Since the report on this epidermal nevus associated with intemal malignancy, the newer finding was interested by dermatologist. A case of 3 and half months old boy who had nevus unius lateris and retroperitoneal leiomyosarcoma since at birth was seen at St. Mart's Hospital. He did not mongolian and cafe au lait spots on the vack which seemed to be a characteristicaign of epidermal nevus syndrome, His developmenta percmtile was 25 of his matched age group. He was bome with spontaneous normal delibery in the hospital. His parents were relatively healthy and his morher had no history of any kind of medicine taken during the pregnancy. There were no family history of epidermal nevuys like skin lesions and hereditary disorders.

6.Clinical Experience with Frason Ointment.

Frason' a new topical steroid ointment containing 0.05% 17-desoxymethason and 0.5 % framycetin, was in trial for several kinds of dermatoses. Total 40 patients including 8 cases of contact dermatitis, 6 seborrheic dermatitis, 4 atopic dermatitis, 4 pityriasis rosea, 4 acne vulgaris, 3 nummular eczema, 3 psoriasis vulgaris, 3 pustulosis palmaris et plantaris and 1 folliculitis were treated with Frason', and the results were as follows. 1) 29 patients revealed excellent or good therapeutic effects (72.5%) with unoccluded application of Frason ointment, 2 or 3 times daily, among the 40 patients. 2) In comparison with the 0.05% Frason' ointment and the 0.05% betamethasone- gentamycin cream, the 0.05% Frason' ointment was found to be slightly more effeetive in the aspect of anti-inflammatory and anti-bacterial action. 3) W/0 base of Frason' ointment had slightly irritating reaction compare to O/W base of betamethasone cream, There was no side effect except 1 patient who complained of buming sensation after initial application of Frason ointment.

7.bcl-2 and MIB-1 Expression in Basal Cell Carcinoma.

BACKGROUND: bcl-2 is a newly characterized proto-oncogen that has been shown to suppress programmed cell death(apoptosis), which is involved in tumorigenesis, and its expression has been demonstrated within tumor cells in a variety of neoplasms. OBJECTIVE: We examined whether there are differences in bcl-2 expression in basal cell carcinoma and several epidermal keratinocytic tumors. In addition, we evaluated bcl-2 expression according to histological types of basal cell carcinoma as well as the interaction of expression of bcl-2 and MIB-1 protein in BCC subtypes. METHODS: Routine paraffin sections of formalin-fixed sixty tissues(20 BCC, 10 Bowen disease, 10 keratoacanthoma, and 20 SCC) were labelled with anti-bcl-2 monoclonal antibody and MIB-1 using a labelled streptavidin-biotin complex. RESULTS: 1. Twenty cases of basal cell carcinoma were examined and all expressed cytoplasmic bcl-2. Three out of 20 cases of squamous cell carcinoma were focally positive. None of the 10 Bowen disease cases and the 10 keratoacanthoma cases expressed bcl-2. 2. In BCC, bcl-2 immunoreactivity was great in the superficical subtypes which had an indolent growth variant, moderate in the circumscribed types, and weak in the infiltrative types which had aggressive growth variants. 3. Evaluation of the distribution of bcl-2 immunoreactive categories and MIB-1 grades revealed a negative correlation tendency, but no statistical significance. CONCLUSION: Our results suggest the presence of a protection mechanism from apoptosis mediated by bcl-2 protein involved in the neoplastic growth mechanism of BCC. In addition, the observed findings in the expression pattern of bcl-2 and MIB-1 in the BCC subtypes may be due to interaction between bcl-2 and other apoptotic-related oncogens.

8.Psychiatric Evaluations of Chronic Urticaria and Alopecia Areata by Cornell Medical Index.

This st;udy was to obtain psychosomatic factors influencing cause of chronic ureicaria and alopecia areata with comparison of dermatophytosis. This survey was carried out by Cornell Medical Index from March 1975 to- September 1976 on total 93 persons which constitute 22 chronic urticaria,38 alopecia areata and 33 dermatophytosis at; the out patient clinic of dermatology department of St. Mary hospital Findings were as follow: 1. The average numbr of complaints of over-all section was 47.36+26.89, the somatic section (31. 73+16. l4) and the psychic (l5. 64+-12. 64) in the chranic urticaria. The average number of complaints of over-all section was R8.50+20.89, the somatic section (22. 63+-13. 10) and psychic section (15. 87+-9. 90) in the alopecia areata, For the control, over-all section was 25. 18+18. 87, the somatic section (16. 30+l2. 38) and the psychic(8. 88+8. 96) in the dermatophytosis. Consquently, there was an increased number of complaints in the group of chronic urticaria and alopecia areata than the control group. 2. There was increased complaint of both the somatic and psychic section in the chronic urticaria. 3. There was increased complaint of psychic section than the somatic section in the alopecia areata.

9.Closed Intramedullary Nailing of Femoral Shaft Fractures

Since May 1981, the standard method of treatment of the femoral shaft fractures at the Daejeon EuIji Deneral Hospital has been the closed intramedullary nailing technique of Kuntscher. We performed intramedullary nailing on thirty-five fractures of femoral shaft in 35 patients. Closed intramedullary nailing was used in twenty-eight femora and open intramedullary nailing with cerclage wiring in seven fractures, in which large butterfly fragment was present, precluding control of rotation or length, or both. The overall rate of union was 97 percent. The range of motion of the knee at follow up was good in 80%, fair in 14%, poor in 6% (Good Full extension; loss of flexion less than 10 degrees. Fair Any loss of extension;loss of flexion of 10 to 50 degrees, Poor Loss of more than 10 degrees of extension; range of flexion-extension less than 90 degrees). The advantages of this technique include the negligible risk of infection, the rapid stabilization of fracture, thus facilitating management of other associated injuries, the rapid return of function of the knee, and early union of fracture and return to work. It is authors' opinion that when proper equipment and expertise are available, than closed intramedullary nailing is the treatment of choice not only for simple uncomminuted fracture of the femoral shaft but also for open comminuted fractures.

10.A Case of Primary Cutaneous CD30(Ki-1)-positive Large Cell Lymphoma Showing Repetitive Spontaneous Regression and Recurrences.

Primary cutaneous CD30+ large cell lymphoma(LCL) is a rare cutaneous peripheral T cell lymphoma with a favorable prognosis. This lymphoma characteristically presents itself as a solitary or localized skin tumor with frequent cutaneous relapses and partial or complete spontaneous remission. Recently we saw a sixty-three year old male who had developed primary cutaneous CD30+ large cell lymphoma that waxed and waned. He presented with localized multiple nodules that had shown repetitive spontaneous regression and recurrences with the same morphology in the same area for several years. There was no evidence of nodal and visceral involvement. The immunohistochemical studies demonstrated that most neoplastic cells in the tumor were positive for CD30 and pan-T cells and negative for pan-B cells, S-100 proteins, EMA and monocyte-macrophage related antigen(CD68).
