1.Syndrome of nonsyphilitic Interstitial Keratitis and Vestibulcauditory symptoms: Cogan's Syndrome.

The syndrome of nonsyphilitic interstitial keratitis associated with vestibulcauditory symptoms (tinnitus, vertigo, nystagmus and progressive deafness) was described by Dr. Cogan in 1945. Since then, numerous cases of this clinical entity have been reported elsewhere in the world. Recently the author experienced one case of Cogan's syndrome in our ophthalmologic OPD for the first time in Korea. The 45 years old male patient, looking very well on gross appearance, was admitted to ENT ward with sudden onset of vertigo, tinnitus and progressive deafness under the diagnosis of Meniere's disease on May 14,1976. Two month later, on July 6, 1976, the visual acuity in his left eye had suddenly deteriorated from 1.2 to 0.6. Under the slit lamp microscopic examination, mild infiltration in the corneal stroma and almost total pcsterior synechia were detected, and a few inflammatory cells were found in anterior chamber but not observed agueous flares. Several fine keratic precipitates were adhered on the lower one half of the posterior corneal surface as a linear apperance. The patient was treated by local instillation of 1% Atropine, 10% Phenylephrine and 3%. Dexamethasone, and also 1% Depomedrol was injected subconjunctivally. Oral administration of Dexamethasone, Vitamine B complex, INH and Chloramphenicol were combined. Thereafter those ocular symptoms had gradually improved, and recently his visual acuity has recovered to normal level (OS 1.2). The vestibuloauditory symptoms just as tinnitus and vertigo were also disappeared, where as moderate deafness has been still remained.

2.Nasal Bone Fractures : Evaluation with Thin-section CP.

PURPOSE: To determine the value of thin-section CT in the diagnosis of nasal bone fractures. MATERIALS AND METHODS: We evaluated the thin-section CT scans of 40 patients with nasal bone fracture. CT scans were obtained with both axial and coronal planes, 1.5mm collimation with 2mm interval, and 9.6cm field-of-view. The axial scan plane was kept parallel to the orbitomeatal line from the nasion to the lower limit of the nose and the coronal plane was kept perpendicular to the axial plane. The data were reconstructed with bone algorithm. Nasal bone fracture was classified into 1 of 3 types on thin section CT:(I) simple fracture;(ll) simple fracture with displacement;(III) comminuted fracture. Associated facial bone injuries were also evaluated Simple radiographs of nasal bone were reviewed for comparison. RESULTS: Six patients had simple fracture, 10 patients had simple fracture with displacement, and 24 patients had comminuted fracture. Twenty-six patients had associated facial bone injuries which included fracture of nasal septum (n=15), fracture of frontal process of maxilla (n=9), fracture of ethmoid (n=6), widening of nasofrontal suture (n=5), and fracture of nasolacrimal duct (n=2). In 15 of 40 patients, CT could identify nasal bone fractures not detected on simple radiographs. CONCLUSION: Thin-section CT is a valuable aid in the evaluation of nasal bone fracture for accurate identification, nature, and combined facial injury.

4.The Study of the Oculocardiac Reflex.

Ophthalmologists now realize the importance of the oculocardiac reflex from ocular manipulation. The occurrence of cardiac disturbances has been recognized since 1908. Several authors have believed the mechanisms of the reflex affecting the pulse rate. the conduction system and myocardial contractibility of the heart. For the purpose of the recognition of this reflex, authors performed ocular compression and muscle traction after atropine injection or retrobulbar anesthesia in 135 persons. The results were noted that the reflex is blocked by intravenous atropine injection or retrobular anesthesia. while not by subconjunctival anesthesia or intramuscular atropine injection.

5.Three Cases of Leber's Hereditary Optic Atrophy in One Family.

6.A Case of Acute Optic Neuritis following Scarlet Fever.

Authors experienced a case of acute optic neuritis, 12 years old female patient, following scarlet fever which is rare among bacterial origin in ophthalmological aspects. On the first day of admission, the visual acuities of 0.04(N.C.) in right eye and 0.2 (N.C.) in left eye, blurring of disc margins (OU), central scotoma (OD), and high antistreptolysin O titer (A.S.T.O.) were noted. After bed rest and medical treatment with corticosteroid, penicillin, and vitamin B12 for two and a half months, the visual acuities and antistreptolysin O titer were recovered completely.

8.The Clinical Survey of the Ocular Injuries by Traffic Accidents.

Authors analysed clinically 44 patients (57 eyes) of the ocular injuries by traffic accidents among 333 In-patients who admitted to Han Gang Sacred Heart Hospital from Jan. 1972 to June 1974. 1. The incidence of ocular injuries by traffic accidens was 13.2% of all admitted patients. 2. Male (64%) was two times as much as female among the patients and they were m03tly belong to 2nd and 3rd decade. 3. The incidence of site was 41% in left eye. 4. The seasonal distribution showed that 48% of traffic accidents occurred in spring. 5. External ocular injuries; laceration or ecchymosis of lid, subconjunctival hemorrhage, and corneal laceration, were predominant (52%). 6. Ocular injuries which were combined with head trauma or facial laceration were 64% and 19% in single ocular injury. 7. The visual acuity of lower than 0.1 was 29.5% of all injured eyes.

9.Delayed intentional replantation of periodontally hopeless teeth: a retrospective study.

PURPOSE: The purpose of this study was to retrospectively evaluate the survival of periodontally hopeless teeth that were intentionally extracted and replanted after a delay and to compare the radiographic characteristics of the survival group with those of the failure group. METHODS: The clinical and radiographic data from patients who underwent delayed intentional replantation between March 2000 and July 2010 were reviewed. Twenty-seven periodontally hopeless teeth were extracted and preserved in medium supplemented with antibiotics for 10-14 days. The teeth were then repositioned in the partially healed extraction socket and followed for 3 to 21 months. The radiographic parameters were analyzed using a paired t test and the cumulative survival rate was analyzed using Kaplan-Meier analysis. RESULTS: Seven replanted teeth failed and the overall cumulative survival rate was 66.4%. In the survival group, the amount of bone loss was reduced from 68.45% to 34.66% three months after replantation. There was radiologic and clinical evidence of ankylosis with 5 teeth. However, no root resorption was found throughout the follow-up period. In the failure group, bone formation occurred from the bottom of the socket. However, a remarkable radiolucent line along the root of a replanted tooth existed. The line lengthened and thickened as time passed. Finally, in each case of failure, the tooth was extracted due to signs of inflammation and increased mobility. CONCLUSIONS: Delayed intentional replantation has many advantages compared to immediate intentional replantation and could serve as an alternative treatment for periodontally involved hopeless teeth. However, techniques for maintaining the vitality of periodontal structures on the tooth surface should be developed for improved and predictable results.
