To evaluate the appearance rate of typhoid granuloma, a clinical and histological study was made on bone marrow smears or biopsies of 60 patients with proven typhoid fever. The results were as follws: 1) Sixty cases consisted of 34 cases of bacteriologically proved group, 10 cases of serologically proved group, and 16 cases of clinically suspected group. The positive culture rate from bone marrow fluid was the highest (70.6%) among specimens although it was obtained 1.6 days in average after the administration of antibiotics. 2) Fourth-eight cases by bone marrow smear revealed the M:E ratio of 3.93:1 in average, mild myeloid hyperplasia and 50.5% of appearance rate of typhoid cells. 3) Fourth-nine cases of bone marrow biopsy revealed 8 cases of typhoid granuloma, all of which belonged to the bacteriologically proved group. All these have no pathognomonic morphologic features. 4) Fourty cases of bone marrow examination of patients with FUO except typhoid fever revealed no typhoid cells in smear and 3 cases of granulomatous lesion from two patients with miliary tuberculosis and one patient with Hodgkin's lymphoma.

A case of a clinical syndrome of selective hereditary complete IgA deficiency with partial T-lymphocyte inactivation is reported. A 14 year old boy was admitted with chjef complaint of persistent wide shallow ulcerated non-vesicular patches on the skin folds of whole body, chronic upper respiatory infections and gastointestinal upsets. General appearance showed us as if the undemourished or malabsorption status had been accompanied. The duration of illness was about 10 years under the incorrect or mistaken diagnosis and skin lesions have incompletely responded to the treatment. Peripheral T-lymphocyte determination was carried out by the method of Wybran to be tumed out active T-cell 14%, total T-cell 48%. Tuberculin test showed negative with 5 TU. Immunoglobulin values were respectively as follows: IgG 325 mg%, IgM 60 mg%, and IgA trace. And we diagnosed this case as selective IgA defiency with partial T-lymphocyte inactivation. We started the immunochemotherapy with tuberculo-protein complex, Tubercin-3 and antibictics for 2 months. After 2 months with above regimen. the skin lesions were markedly improved. T-cell count also retumed to the normal limit(active 35%, total 59%) and levels of TgG and IgM raised(IgG 617.5 mg%, IgM 75mg%), but IgA still showed trace amount, Tuberculin test tumed out positive.

Korean Journal of Pathology 1995;29(5):584-589
The origin of the stromal cell of cerebellar hemangioblastoma has long been studied electron microscopically and immunohistochemically. The results and theories about the stromal cell origin are variable and plentiful. However, the exact origin of the stromal cell remains controversial. The present study is aimed to elucidate the nature of the stromal cell of cerebellar hemangioblastoma. Ten cases of hemangioblastoma in Severance Hospital were used for immunohistochemical analysis of the stromal cell. The immunohistochemical staining of GFAP, S-100 protein, NSE, alpha-l-antichymottypsin, cytokeratin, CD 68, factor VIII related antigen, and synaptophysin were performed. The results were as follows; GFAP and S-100 protein were stained mainly but weakly in bellar capillary spindle cell and cellular process. NSE was stained mainly in foam cells, and 6 cases among them revealed strong reaction. ct-l-antichymotrypsin was stained in a few foam cells of 5 cases. Cytokeratin, CD 68, factor VIII related antigen, and synaptophysin showed negative reaction. Based on these results, it is considered that the origin of the stromal cell is histiocytic or neurogenic rather than glial. The weak positivity of GFAP and S-100 protein may support the neurogeriic origin but ct-l-antichymotrypsin positivity does not support the possibility. The positivity of GAP and S-100 protein supports the phagocytic action of histiocytic cell and suggests histiocytic origin rather than neurogenic.

This study was investigated to compare the staining resistance of soft denture liners. Specimens wee made of Coe-soft. Coe-Comfort, Soft-liner, Visco-gel, and were stored in 1% methyleneblue solution for 24 hours. The amounts of color change before and after treatment with mono-poly and thermocycling were measured by colorimeter(TC-6FX, Tokyo Denshoku Co. Ltd, Japan) for evaluation of staining resistance. The following conclusions were drawn from this study. 1. The staining resistance of Visco-gel was increased, but there was no changer of staining resistance in Coe-soft, Coe-comfort, and Soft-liner after treatment with monopoly. 2. The staining resistance of the Coe-comfort was the least in all soft denture liners. 3. The staining resistance of Visco-gel and Soft-liner were decreased after thermocycling.

Deliberate hypotension reduces bleeding in a wound, there by providing the surgeon with both better visibility and technical freedom for a more definitive dissection and minimizes the need for blood replacement, thereby reducing the risks of transfusion reactions, hepatitis, and acquired immune deficiency syndrome (AlDS). anesthesia With major development in surgery for spine problems in this century there are many reports and studies about the pathophysiology of spine, cord hemodynamics and functional monitoring about spinal cord activity during induced hypotension. Although by the comparative studies of vasodilator deliberate hypotension with nitroprusside was found to induce a reliable and rapid drop in blood pressure, we have found some different responses to the chosen vasodilators between the older, less active groups and the healthy, robust, and physically active groups. In this study we evaluated the hypotensive anesthesia for 42 cases of spinal surgery-posterior stabilization, spinal fusion, laminectomy, osteotomies, and discectomy-performed at Kwangju Christian Hoispital from May 1987 to May 1988. The result of our study was as follows: 1) Halothane and enflurane were used as primary anesthetics in 31 and ll cases respectively. Nitroprusside, hydralazine and both of this two vasodilatora were supplementarily used for inducing hypotensin in 14, 16 and 5 cases, respectively. 2) Although in many paient particularly in the older and less active groups, the combination of halogenated anesthetics and hydralazine are sufficient to produce appropriate operating conditions, in healthy, robust and physicaly active groups nitroprusside or even the combination of two vasodilators are required. 3) The mean arterial pressure of the lowest blood pressure in the induced hypotension was 60.5+/-11.6 mmHg. 4)The mean blood loss and blood replacement was 1242.2+/-769 ml and 925.4+/-854 ml respectively. 5) In the intraoperative period gas analysis arterial CO2 tension was 34.2+/-6.6mmHg and other values was within normal limit. 6) There was no significant perioperative and postoperative complication due to induced hypotension itself. In conclusion, the technique of using induced hypotension with inhalation anesthetics supplemented by nitroprusside, hydralazine, or both is safe and useful one to use in performing surgery for spinal problems.

Hand, foot and mouth disease, a distinctive clinical syndrome caused by a coxsackie virus, is clinically characterized by vesicles appearing on the hands, feet and in the mouth. The infection begins with a fever and mouth lesions consisting of small vesicles surrounded by red areolae on the buccal mucosa, tongue, soft palate and gingiva. The disease usually lasts spontaneously between 7 to 10 days after onset. We observed 20 cases of hand, foot, and mouth disease from July, 12th to September, 4th, 1979 and examined histopathologically and virologically. We have isolated only one viral strain showing cytopathic effect on HeLa cell among the five cases of acute stage and also observed that viral particle in the electronmicroscope.

Korean Journal of Anesthesiology 1988;21(4):658-662
A 51 year old female patient underwent resection of pheochromocytoma under general anesthesia. Thiopental sodium 250mg and succinylcholine 50 mg were used for endotracheal intubation, followed by N2O, O2 and ethrane for induction and maintenance. For the management of blood pressure during surgical amnipulation, we used sodium nitroprusside dripping. After tomor resection, blood pressure was controlled by dopamine dripping and intravenous lactated Ringer's solution. The patient tolerated anesthesia and surgery well despite the episodic hemodynamic changes. Although tolerated anesthesia and surgery well despite the episodic hemodynamic changes. Although specific anesthetic drugs have been recommended, we believe that optimal preoperative preparation, a gentle induction of anesthesia, and good communication between surgeon and anesthesiologist are most important for anesthetic management.
