1.A Clinical Study on Gestational Trophoblastic Disease.

This study is designed to evaluate the clinical characteristics and the outcomes of the management of gestational trophoblastic disease diagnosed at our hospital. With a retrospective review of hospital record from 1989 to 1998, we analysed 54 cases of gestational trophoblastic disease by the clinical characteristics and the outcomes of management. The results are as follows 1. The incidence of the gestational trophoblastic disease compared with delivery is one per 341 deliveries and is decreasing with time(from one per 252 deliveries at 1989 to one per 694 deliveries at 1998). 2. The most frequent age is 20-29 years old(61,1%). 3, The most frequent gravity and parity is 0-2 times(68.5%) and 0-1 time(79.6%) respectively. 4. The symptoms and signs are amenorrhea(94.4%), vaginal bleeding(74,1%), abdominal pain( 33.3%), hyperemesis gravidarum(25.9%), excessive uterine enlargement(24.1%), anemia(18.5%), hyperthyroidism(7.4%), theca lutein ovarian cyst(3.7%), and preeclampsia (1.9%). 5. We divided the patients by the uterine size for gestational age; large for date 50%, normal for date 44.4%, and small for date 5.6%. 6. The antecedent pregnancies of patients with persistent gestational trophoblastic tumor are hydatidiform mole 87.5%, abortion 12.5% in middle risk group and hydatidiform mole 66.7%, term pregnancy 22.2%, abortion 11,1% in high risk group. 7. We divided the patients with persistent gestational trophoblastic tumor by the FIGO staging system; stage I 70.6%, stage II 5.9%, stage III 11.8%, stage IV 11,8%. 8. The regimens of treatment are consisted of D & E only(59.3%), D & E with prophylactic chemotherapy(9.3%), D & E with chemotherapy(25.9%), and D & E with chemotherapy and hysterectomy(5.6%). Complete remissions are shown in 100% of D & E only and D & E with prophylactic chemotherapy, in 85.7% of D & E with chemotherapy, and in 33.3% of D & E with chemotherapy and hysterectomy. Dividing the patients with persistent gestational trophoblastic tumor by the WHO prognostic scoring system, complete remissions are shown in 75% of middle risk group and in 77.8% of high risk group. 9. The duration of chemotherapy for complete remission is 2.5 cycles & 77.5 days in middle risk group and 4 cycles & 130.1 days in high risk group. 10. The subsequent pregnancies after complete remission of gestational trophoblastic disease are term delivery 81%, spontaneous abortion 9.5%, induced abortion 4.8%, and preterm delivery 4.8%.

3.Total Cystectomy for Bladder Cancer -102 Cases-.

Between Jan. 1971 and Dec. 1985, 102 consecutive patients underwent total cystectomy and urinary diversion for primary carcinoma of the bladder. Among the patients, planned radiation therapy (2,000 rad. for 5 days) followed by operation was done in 18 patients, therapeutic dose of radiation was given postoperatively in 8 patients and 76 patients underwent operation only The postoperative mortality rate was 3.9%. Early complications occurred in 30% of the 102 patients and included wound infection, urine leak, bowel obstruction and medical complication. An increased late complication incidence was noted among 26 patients receiving pre- or postoperative radiation (50%) compared with those not receiving radiation (32%). Staging errors by CT scanning occurred in 37% of patients, overstaging in 8 (33%) and understaging in 1 (4%). Survival was evaluated in 45 patients and the overall 5-yesr survival rate for patients with transitional cell carcinoma was 38%.

4.Indomethacin Protects from UVA and UVB Irradiation.

The sunlight protecting effect of indomethacin (IM) was compared with topical sun screening agents of sun protecting factor(SPF) 2 and SPF 4, and it was found that IM stood in between SPF 2 and SPF 4. (vehicle of IM) painted skin was 4.8 J/cm and MED skin was 37.5 m J/cm. with UVB of PED painted was compared with topical 2 and SPF 4, and it was MED with UVA of PED with UVB of PED painted skin was 37 5 m J/cm.

5.Neurilemmoma of the Tongue.

A 54-year-old man had a 1.0 *0.8cm sized, reddish, slightly firm tumor mass on the right side of his tongue. Some 30 years ago, he bit his tongue with considerable bleeding. Since then, he has experienced irritation at the previous bit site, particularly after eating irritaible foods or smoking. About 5 years ago, he felt a pea sized mass exactly at the bit site which gradually increased in size. The histopathologic findings of skin biopsy specimen show typical features of neurilemmoma consisted of Antony type A and type B tissue.

6.The Relationship of Psychosocial Factors to Blood Pressure.

Questionnaires and blood pressure measurements were administered to 279 medical school undergraduates in 1987 to investigate the relationship between psychosocial factors and blood pressure as well as reliability and validity of the Framingham Type A Behavior Scale(FTA). The reliability coefficients of SCL-90-R and FTA measured by Spearman-Brown haves split test were 0.57-0.91, The factors of FTA extracted by principal component analysis were hard-driving competitiveness factor and impatience factor(2-factor solution). The total score of FTA was positively correlated with relative weight and place raised but the correlations were insignificant, and had significantly positive but weak correlations with depression, anxiety hostility, paranoid, and psychoticism subscales of SCL-90-R. In the univariate analysis of blood pressures, relative weight and family history were significant in systolic pressure in males and economic status was significant in blood pressures in both sexes. For diastolic pressure, relative weight and frequency of alcohol intake were significant in males and relative weight was in females. After controlling relative weight, the frequency of alcohol intake for diastolic pressure and economic status for systolic pressure were significant in males. The important variables selected by stepwise regression analysis were relative weight and economic status for systolic pressure of males and relative weight and the frequency of alcohol to the model, changing coefficient of determination 0.206 to 0.217. In females, economic status and relative weight were selected for systolic pressure and for diastolic pressure body mass index alone, but the model of blood pressure for females was considered to be unstable due to small sample size(56). FTA was unrelated to the blood pressures in both sexes.

7.A Case of Generalized Familial Benign Pemphigus.

Familial benign pemphigus(Hailey-Hailey disease) is a rare hereditary dermatosis that begins in the 2nd or 3rd decade of life. The skin lesion is characterized by a localized, recurrent eruption of small vesicles on an erythematous base. During its course there are remissions and exacerbations. It seldom begins in early childhood or after the age of 50. The main treatment modalities are conservative ones. A 59-year-old woman with familial benign chronic pemphigus presented with a 10 year history of generalized pruritic recurrent skin lesions on her neck, axilla, inguinal, antecubital, and trunk area. Physical examination showed moist, macerated, fissured and scaly patches on an erythematous base in the axillae, groins, neck, antecubital, and trunk. A biopsy specimen showed extensive suprabasal separation containing acantholytic cells.

9.Studies on Serum Lipids Level in Normal Korean Adults and Vegetarians.

The serum lipids level of healthy Koreans, including 306 normal adults, 15 overweight persons, 25 faculty members and 40 vegetarians (monks) were studied in 1973. The following results were obtained. 1. The mean serum liplids values of normal Korean adults were 188.7+/-33.7mg% for total cholesterol, 176.9+/-47.0mg% for phospholipid and 78.0+/-28.9mg% for triglyceride. 2. It was observed that the serum level of normal Koreans were influenced only by age and were not influenced statistically by sex, weight and socioeconomic state. 3. The mean serum lipids level of Korean vegetarians (monks) were 188.0+/-44.0mg% for total cholesterol, 158.5+/-35.6mg% for phospholipid and 64.8+/-21.4mg% for triglyceride and not influenced by sex and duration of altered eating habit. 4. The serum lipids level of Korean vegetarian (monks) were significantly low in phospholipid and triglyceride but not in cholesterol. 5. No significant difference in serum total cholesterol level between normal and vegetarian were observed all the role of diet of general population were discussed.

10.A Survey on Menarche of School Girls in Seoul and Taeru Area.

It is clearly defined through a number of authoritative studies that the age of menarche influenced by the various combined factors such as nutrition status, physical status, physical growth and development status, socio-economic status, locality, culture, education level, climate, rece heredity etc. In order to obtain statistical data regarding the menarche of Korean school girls, anthors investigated on 4207 middle school girls and 703 woman college students in Seoul and Taegu during the period of September 10 to 30, 1973 and the result are summarized as followings; 1. The rates of menses experience by years were 18.2% in the age group of 12 years girls, 31.9% in 13 years, 64.6% in 14 years, 89.8% in 15 years, 98.1% in 16 years respectively. 2. The average age of menarche for the 2504 school girls who were born during the year of 1957-1961 is 13.4 years with the rang of 9 years to 16 years. And the most frequent age of menarche is 13 years. 3. The average age of menarche for the 703 woman college students who were born during the year of 1950-1954 is 14.3 years with the rang of 9 years to 18 years. And the most frequent age of menarche is 13 years. 4. The apparence of menarche is most common in August (20.7%) for the group who were born during the year of 1957-1961 and most rear in November (4.2%). And it is also most common in August (19.9%) for the group who were born puring the year of 1950-1954 but most rear in June (3.4%).
