2.A Case of Metastatic Carcinoma of the Skin.

We report a case of metastatic carcinoma of the skin in 62-year-old male who showed multiple nodules on the anterior chest, scalp, face and left axilla for 2 months duration. He suffered from lower abdominal discomfortness for 6 months. Histopathologic findings revealed atypical cell nests and some tubular or glandular formation in the dermis and subcutaneous fat tissue. Material in the lumen showed diastase resistant and PAS positive, and alcian blue positive at pH 2.4, but negative pH 0.4. So it represented sialomucin. We did not find primary site of malignant tumor. In view of clinical and histopathological findings, it was considered to be originated from the large intestine.

3.Intervention Model Development of Health Promotion for Women Workers in Traditional Marketplaces: Using Community based Participatory Action Research.

PURPOSE: The purpose of this study was to describe the intervention model development of health promotion for women workers working in the traditional marketplace, using community capacity building framework. METHODS: Need assessment of the target population, work-related environment investigation, social network group building, and setting health management in the marketplace were performed. Then the interventions including cardiovascular health, musculoskeletal health, and psychological health were conducted. The results were evaluated using NCEP-ATP III(National Cholesterol Education Program's Adult Treatment Panel III), OWAS, and CES-D. RESULTS: It was found that the intervention program for the vulnerable group in marketplace was appropriate, promoting the improvement of metabolic syndrome and the reduction of pain complaints. Therefore, the intervention framework for health promotion of women workers in the marketplace was developed. Work-related environment assessment also was included in the framework development. Several community capacity building strategies, including developing community resources and promoting partnership, making small social network group, and promoting program participation. CONCLUSION: It is suggested that occupational health nurses and professionals consider the appropriateness of intervention framework development after identifying the needs of women workers' work-related environmental problems.

5.The Effects of Nutritional Education Program on Nausea and Vomiting, Anorexia, Food Intake, and Nutritional Status of GI Cancer Patients Undergoing Chemotherapy.

PURPOSE: The purpose of this study was to identify the effects of nutritional education program (NEP) on nausea and vomiting, anorexia, food intake, and nutritional status among GI cancer patients undergoing chemotherapy. METHODS: A nonequivalent, non-synchronized posttest design was used. Study subjects were conveniently selected 30 patients (15 subjects for each group) who were undergoing chemotherapy. The subjects of the experimental group received NEP which consisted of 2 educations and 1 counselling while they were hospitalized, and telephone counselling with nutritional supports after discharge for four times. RESULTS: The score of anorexia was lower and the amount of food intake was higher in the experimental group than those of the control group. However, there were no significant differences in levels of nausea and vomiting, weight, hemoglobin, albumin, and total protein level between the experimental and the control group. CONCLUSION: The findings demonstrate that the NEP was effective in alleviating anorexia and improving subjective food intake of GI cancer patients undergoing chemotherapy. These findings suggest that NEP can be a useful nursing intervention for preventing nutritional disorders for patients with GI cancer after chemotherapy.

7.Health status and behavior of homeless in Seoul.

BACKGROUND: Since recent economic crisis(IMF) in Korea, the number of new homeless is increasing(that is, 'IMF style homeless'). There are only few studies on the health problem of this uniquely formed population. We have attempted to estimate the problem of health behavior and health status of this population. METHODS: Some homeless who agreed to enter the asylum during the winter were taken for medical check-up in Sungbuk Community Health Center from November to December, 1998. The participants filled up a self-reported questionnaire and physical examination, laboratory test, and chest x-ray were performed. RESULTS: Among 'IMF style homeless', the prevalence of hypertension(30.9% in primary screening, 7.3% in secondary screening), diabetes mellitus(9.4% in primary screening, 3.5% in secondary screening), syphilis(7.3%), and hepatitis B antigen carrier(5.3%) was higher than among the general population. But we could not find statistical difference according to the duration of homelessness. We found higher smoking rate and higher alcohol drinking amount per week in homeless than in the general population. The amount of alcohol drinking per week was larger among those who had been homeless for more than 3 months(p< 0.01). CONCLUSION: The prevalence of chronic diseases in homeless was higher than in the general population, but statistical difference according to the duration of homelessness could not be found. The longer the duration of homelessness, the larger the amount of alcohol drinkin was seeng.

8.Comparison of the Optimal Depth of the Internal Jugular Venous Catheterization between Right and Left.

BACKGROUND: The purpose of this study was to measure and compare the optimum depth of the internal jugular venous catheterization between the right and left side. METHODS: Forty-four patients were enrolled for this study and divided into two groups (22 patients each). The optimum depth of the catheterization was calculated using the sum of two component (A and B); the advanced length of the catheter from the level of the cricoid cartilage (A) and the distance from the catheter tip to the junction of the superier vena cava and right atrium (B). RESULT: The optimum depths of the internal jugular venous catheterization were 16.0 1.0 cm (right) and 18.4 1.5 cm (left) respectively. Left side was significantly longer than right side (p<0.05). In this study, we experienced some complications; arterial punctures (5 cases) and migration of the catheter to the opposite subclavian vein (1 case). Five complications were associated with left internal jugular venous cannulation and one was associated with the right side cannulation. CONCLUSION: We concluded that the optimum depth of the internal jugular venous catheterization was longer in the left side than in the right side.
