1.Pattern of Adenomas after Colorectal Cancer Surgery.

Postoperative colonoscopy is an effective tool for management of colonic adenomas. Authors analyzed the pattern of colonic adenomas detected during follow-up colonoscopy after colorectal cancer surgery and evaluated the characteristics of adenornas, risk groups, and effective fo1low-up schedule. Study group were 222 patients and colonoscopy was performed 389 times. Patterns of adenornas were analyzed by variables as age, sex, preoperative serum CEA level, location of primary colorectal cancer, Borrmann type, Duke's stage, histologic differentiation, DNA ploidy, recurrence and histology. Metachronous adenomas were detected in 79 patients(35.6%) and both metachronous and syachronous adenomas were observed in 29 cases(13.1%). High risk variables for adenomas were male, old age and presence of synchronous adenoma. There were 2 patients with maligant change of adenomas. Yearly follow-up by complete colonoscopy over 3 years or more is recommended and follow-up interval should be shortened in the high risk groups.

2.Treatment of the Tibial Plafond Fractures Using the Ilizarov Method.

An intra-articular fracture of the distal tibia or a fracture of the tibial plafond is a relatively uncommon but the most difficult fracture to manage. There are many treatment options and many authors have reported good results after open reduction and rigid internal fixation to restore the length with articular surface and bone-graft followed by early motion and prolonged non-weight bearing. But in the fractures with severe comminution or with severe soft tissue injury, tries for internal fixation with plate and screws in the distal tibia will result in extensive soft tissue stripping of the wound with precarious blood supply, high rates of wound breakdown, superficial and deep infection, and osteomyelitis. The purpose of this study is to evaluate the results of treatment of severely open (open type III-A and more by Gustilo and Anderson) or comminuted plafond fractures (fracture type III and more by Ovadia and Beals) with an Ilizarov method with a minimum follow up of one year (average: 27.9 months) and we have reviewed 12 cases from December 1990 to October 1995. The results were as follows. 1. The average time of bone union were 17.5 weeks (range: 6-42weeks). 2. The radiological results of the treatment were excellent in 4 patients (33.3%), good in 4 patients (33.3%) and poor in 4 patients (33.3%) by radiological assessment of Crutchfield and the clinical results of the treatment were excellent in 4 patients (33.3%), good in 5 patients (41.7%) and poor in 3 patients (25.0%) by functional rating scaie of Crutchfield. 3. It seems that prognosis depends on severity of comminution at the time of injury. 4. The complications were bony collapse of the distal tibia in 2 patients, nonunion in 1 patient, degenerative arthritis of the ankle joint in 2 patients and chronic osteomyelitis in 2 patients. 5. Ilizarov technique is an useful method in the management of severely open or comminuted plafond fracture of the tibia and it would be preferable to apply buttress plate or bone graft after improvement of skin condition to prevent the late collapse.

4.A Clinical Study of Panoxyl 2.5(R) in Acne Vulgaris.

Bezoyl peroxide, a powerful oxidizing agent, has been uaed topically in the treatment of acne vulgaris as a cream or lotion and has been shown to he effective for this codition. In 1972 Fulton emphaaized the importance of the vehicles used to obtain. maximum potential of benzoyl peroxide and for this, the gel formulation has been examined in many clinical trials. The present study was undertaken to evaluate the therapeutic effect of Fanoxyl 2.5, a 2.5% . benzayl peraxide gel, in patients with acne vulgaris. A total of 31 patients entered this study at the Departrnent of Dermatology, Jeoul National University Hospital rluring 3 rnanths period, from Decemher, 1978 through February, 1979, All patienta were instructed to apply Fanoxyl 2.5' on their affected areas once or twice a day and the nurnber of lesions on two or three t fixed areas were counted before and onee weekly after initation of the treatment. The reaults observed in the 3I patients were as follaws; 1. The percentage decrease in total nurnber of lesions in 4 weeks after treatment was 56.9%. 2. The authors could follow up for 6 weeks in 6 cases. In 6 case, the mean percentage in total number of lesions in 6 weeks after treatment was 81.7%. 3. Clesed and open eomedones were gradually eliminated; and their numbers were decreased in 4 weeks after treatment by 46.5% and 56.8% respeetiveIy. The papules were rapidly eliminated and their numbers were decreased in 4 weeka, after treatment by 79.8%. 4. Although most patients felt a mild burning sensation and tightness at the begining of the treatment, only a minority of the patients complained of discomfort due to pain(l case), erytherna(l case), tightness(2 cases),burning sensation (1 case), scaling(1 case). But all were tolerable without taking any specific measures or discontinuance of application. In one case, contact dermatitis developed after 4 weeks treatment. The authors concluded through this experiment that Panoxyl 2.5' (2.5% benzoyl peroxide gel) is a very effective local therapeutic agent in the treatment of acne vulgaris patients.

7.Clinical Observation on Chronic Prostatitis.

A Clinical observation was made on 220 cases of chronic prostatitis during the period from January 1980 to March 1981. The following results were obtained: 1. Age distribution was between 20 and 70 years old, showing the highest incidence in 20 to 29 years old (44. 6%) and the next in 30 to 39 years old (36.8%). 2. Subjective symptoms were in order of urethral pain or tingling, morning drops or urethral discharge, urinary frequency and perineal discomfort. 33 cases (14.3%) were free of symptoms. 3. Previous diseases were acute or chronic urethritis on 110 case (50.0%), previous prostatitis on 25 cases (11.4%), urinary stone on 7 cases (3.2%), S.P.H. on 7 cases (3.2%). 15 cases (6.7%) were free of previous diseases. 4. In urine examination, 94 cases (43.2%) showed W.B.C. more than 5/H.P.F 126 cases (56.8%) were within normal limit. 5. In 137 cases in which prostatic gram stain was done, microorganisms were gram (+) cocci on 28 cases (20.5%). gram (-) bacilli on 13 cases (9.4%), gram (+) bacilli on 8 cases (5.9%). No microorganism were on 88 cases (64.2%).

8.A Case of Bilateral Renal Agenesis Diagnosed by Prenatal Ultrasonography.

Fetal bilateral renal agenesis is a lethal congenital anomaly. An early and reliable prenatal diagnosis is extremely important as it may offer options for pregnancy termination as early as possible. The criteria for the ultrasonographic diagnosis of bilateral renal agenesis are severe oligohydramnios, nonvisualization of the bladder, and the empty renal fossae. However, severe oligohydramnios makes it difficult to diagnose the disease because of poor sonographic resolution. We present a case of fetal bilateral renal agenesis diagnosed by ultrasonography after amnioinfusion at 19 weeks gestation.

9.Changes in nitric oxide production in preeclampsia.

OBJECTIVE: To determine the changes of nitric oxide production in preeclampsia, the concentration of nitric oxide metabolite, nitrite, was measured in umbilical vein after perfusing plasma from normal pregnant women and preeclamptic pregnant women. MATERIAL AND METHOD: 15 normal and 15 preeclamptic umbilical cords were obtained at the time of cesarean section. Two pieces of umbilical cord in equal length(20 cm in length) were prepared from each umbilical cord. Two pieces of umbilical cord were connected in parallel in a perfusion chamber. One piece of umbilical cord was perfused sequentially for 20-minutes' interval with the perfusates in the order of cord buffer, cord buffer including 15% normal pregnant serum, 15% normal pregnant serum with histamine(10-5mol/L), 15% normal pregnant serum with calcium ionophore A23187(5 mol/L) and the other one was perfused exactly same way using 15% preeclamptic serum instead of 15% normal pregnant serum. All the perfusates used were gassed with 95% O2 and 5% CO2 and warmed to 37degree C. Perfusates were collected in eppendorf tube and freezed at -70degree C until assayed. NO was measured by means of Greiss reaction. one way ANOVA and paired t-test were used where appropriate and p-value < 0.05 was considered significant. RESULTS: NO production in normal umbilical cords was not different regardless of perfusate. Although adding histamine and calcium ionophore, the NO production was slightly increased but statistically not significant in both groups. NO production in preeclamtic umbilical cords was significantly increased with 15% preclamptic serum(15% normal serum vs. 15% preeclamptic serum; 0.060+/-0.016microgram/ml/min vs 0.075+/-0.014microgram/ml/min, p<0.05). CONCLUSION: The preeclamptic sera may not affect the production of NO in the human umbilical vein endothelial cells. The biologic significance of increased NO production in preeclamptic umbilical cord with perfusing preeclamptic serum is unknown, but it might be compensation for the vasoconstriction of preeclampsia.

10.A Case of Werniche's Encephalopathy in Hyperemesis Grevidarum.

Werniche's encephalopathy is clinically characterized by the acute onset of global confusion, ataxia, gaze paresis, and nystagmus. It result from a deficiency in thiamine, an essential coenzyme in intermediate carbohydrate metabolism. The prompt use of thiamine prevents progression of the disease and reverses those lesions that have not yet progressed to the point of fixed structural change. We experienced a case of Wemiches encephalopathy associated with hyperemesis gravidarum, which happens to the patient who are injected only dextrose without thiamine. Therefore, we emphasize the need of thiamine replacement in hyperemesis gravidarum.
