19 years old female had untreated Veau classification class II cleft palate with ectopic eruption of upper right lateral incisor and congenital missing of lower lateral incisors. Upper left lateral incisor, left first molar aid lower left first molar were root restswithperiapicalpathologiclesions. So all root rests were extracted and prosthodontic rehabilitation after orthodontic treatment was planned. She was treated by means of multibanded system with face bow. After 23 months all orthodontic correction were achieved and, as soos as debanding procedure was done she was referred to oral surgeon and prosthodontist for surgical operation and bridge construction.

A 23 year-old female with skeleto-dentoalveolar protrusion of maxilla, minor broken contact points between anterior teeth, and missing of lower 1st molars, has been treated with multibanded edgewise technique. After treatment of 14 months, she has gained functional overbite-overjet relationship and facial harmony due to the retraction of upper anterior teeth. Root resorption was slight. Especially, us ing the space of missed lower 1st molars instead of extracting lower premolars, expected and favorable results were obtained.

Solitary bone cyst are usually found in the juxta-epiphyseal region of metaphysis of long bones of children. They are rarely seen in non-tubular bones. The following case describes what we believe to be a solitary cyst of the capitate. Cyst was treated by radical curettage and packing with autogenous iliac bone. The most recent evaluation 7 months after surgery showed no discomfort.

Supracondylar Fractures of the femur are not as cammon as femoral neck or intertrochanteric fractures, but their treatment may be just as difficult. Few surgeons have had extensive experience with this complex fractures of uncommon frequency. Until a few years ago, conservative treatment was considered superior to internal fixation of supracondylar fractures of the femur in adults. However, the development of new fixation devices and techniques has, according to several investigations, improved the results of the treatment of these fractures. The following clinical results were shown by analysis of 27 cases of femoral supracondylar fractures experienced in the Department of Orthopedic Surgery, Capital Armed Forces General Hospital in the past 3 years from Jan. 1977 to Oec. 1979. 1. The most common cause of these fractures were traffic accidents. 2. Of 27 cases, 11 cases were treated by conservative method, 16 cases were treated by open reduction and internal fixation. 3. Anatomical reduction and rigid internal fixation led to 80% of good to excellent results whereas only 33.4% of good to excellent results were obtained in patients in whom the fixation was not absolutely rigid. 4. In the treatment of supracondylar fractures of the femur, anatomical reduction and rigid internal fixation (80% of good to excellent results) resulted in better functional recovery after union of fractures than conservative treatment (45.5%).

From 1963 to 198I, we have performed Colonna's capsular arthroplasty on 7 children. Three of the hips had congenital dislocations and four hips had pathologic dislocation. The results on these cases, including some long term follow-up were satisfactory. Since Colonna first published his technique in 1932 ideal indications for the classical arthroplasty have largely been replaced by those of more recently developed procedures such as Salter's innominate osteotomy, Chiaris osteotomy and etc. We feel, however, that there still seems today a narrower, but distinct domain of usefulness left for the capsular arthroplasty, and that new indications need to be redefined. We propose the following criteria for such purpose. First, congenital dislocations of the hip in children beyond 6 years of age, which is the upper age limit of Salter's innominate osteotomy, and below 8 years of age, which is the upper age limit o original Colonna's capsular arthroplasty. Second, teratogenic congenital dislocations in which the shallow and slanted acetabulum renders otherwise stable, deeply seated acetabulum with a prospect for a concentric remodelling unlikely. Third, pathologic dislocations secondary to septic hip in children 3 to 8 years of age in which a pain-free, stable and reasonably mobile hip often betrays rentgenographic imaginations and renders reconstructive procedures in adult life feasible.

Tooth movement by orthodontic force is based upon alveolar bone resorption at compression site and bone formation at tension site of tooth. The function of cyclic AMP is to participate not only in initial action of bone cells by mechanical forces but also in the continuous cellular response leading, to bone remodeling. This experiment was performed to clarify the role of cyclic AMP in bone remodeling by mechanical forces. The orthodontic forces of about 80 gm and 100 gm were applied to the right canines of maxillary and mandibular bone, respectively, in cats, treated for periods of time ranging from one hour to 28 days. Alveolar bones were obtained from compression and tension sites . surrounding tipping maxillary and mandibular canines as well as from contralateral control sites. The samples were extracted, boiled and homogenized, and the supernatants were assayed for cyclic AMP by a radioimmunoassay mothod. The results were as follows: 1. The orthodontic movement of canines was increased to the end of experimental period and the action of orthodontic forces on tooth movement was more effective in maxillary canine. 2. The cyclic AMP levels of alveolar bones in compression and tension sites initially decreased, then increased and remained elevated to the end of experiment. The differences of the cyclic AMP levels between treated sites and non-treated sites were gradually increased. 3. The cyclic AMP levels in treated sites of mandibular alveolar bone was higher than that of maxillary alveolar bone.

For the study on the effect of retinoic acid on the formation of palatal rugae and the cleft palate, retinoic acid was administered orally 150mg/kg of body weight by gastric tube at GD 10.5 to Sprague-Dawley rats. The pregnant rats were sacrificed on GD 17.5 under ether anesthesia, and laparatomized. After removal of uterus, the number of pregnant sacs and fetuses were counted. The fetuses weighed, the MEE (medial edge epithelium) thickness measured and the mitotic figures counted after routine processing and H·E stain. All the palates were photographed, and the number of rugae & the rugal pattern analysed. TEM photographs of MEE cells were observed after routine processing. The results were as follows ; 1. Rat fetus body weight after retinoic acid treatment increased significantly compared with the control group. 2. Mitotic figures in the retinoic acid treated group increased significantly compared with control group. 3. In the retinoic acid treated group, 79.3% of fetuses had cleft palates. Among fetuses with cleft palates, complete cleft palates were 10.6%, incomplete cleft palate 89.4%. Incomplete clefts were of two types ; median type (cleft palate at the intermolar region) and soft palate type (cleft posterior to the 8th rugae). Median type was 64.6% and the soft palate type 35.4%. 4. 2.3% of the fetuses had the numerical anomaly of the palatal rugae in the control group, but that of retinoic acid treated group 87.7%. 5. 17.4% of palatal rugae of the control group was disrupted, but 100% of the retinoic acid treated group disrupted. 6. Rugal papillae were observed in the 15.1% of fetuses of the control group and 63.1% of fetuses of the retinoic acid treated group. 7. Longitudinal rugae were observed in 19% of fetuses of the retinoic acid treated group, but not in the control group. 8. In TEM photographs, cytoplasmic processes, intercellular space, and desmosomes decreased. Swelling of mitochondria & ER were also found in the retinoic acid treated groups. According to the above results, it appears that there is close relationship between palatal rugae and cleft palates, and that excess retinoic acid induces disruption of pattern and numerical variations of rat fetus palate rugae. Also retinoic acid has an inhibitory effect on the proliferation of medial edge epithelial cells of palatal shelves. The cleft palates may be induced by the above mentioned retinoic acid effects. But, the exact mechanisms of retinoic acid on cleft palate formation is not thoroughly known and should be further studied.

BACKGROUND: External ventricular drainage (EVD) is an important procedure for draining excessive cerebrospinal fluid (CSF) and monitoring intracranial pressure. Generally, EVD is performed in the operating room (OR) under aseptic conditions. However, in emergency circumstances, the operation may be performed in the intensive care unit (ICU) to save neuro-critical time and to avoid the unnecessary transfer of patients. In this study, we retrospectively analyzed the risk of EVD-induced CNS infections and their outcomes according to the operating place (ICU versus OR). In addition, we compared mortalities as well as hospital and ICU days between the CNS infection and non-CNS infection groups. METHODS: We reviewed medical records, laboratory data and radiographic images of patients who had received EVD operations between January, 2013 and March, 2015. RESULTS: A total of 75 patients (45 men and 30 women, mean age: 58.7 +/- 15.6 years) were enrolled in this study. An average of 1.4 catheters were used for each patient and the mean period of the indwelling catheter was 7.5 +/- 5.0 days. Twenty-six patients were included in the ICU group, and EVD-induced CNS infection had occurred in 3 (11.5%) patients. For the OR group, forty-nine patients were included and EVD-induced CNS infection had occurred in 7 (14.3%) patients. The EVD-induced CNS infection of the ICU group did not increase above that of the OR group. The ICU days and mortality rate were higher in the CNS infection group compared to the non-CNS infection group. The period of the indwelling EVD catheter and the number of inserted EVD catheters were both higher in the CNS infection group. CONCLUSIONS: If the aseptic protocols and barrier precautions are strictly kept, EVD in the ICU does not have a higher risk of CNS infections compared to the OR. In addition, EVD in the ICU can decrease the hospital and ICU days by saving neuro-critical time and avoiding the unnecessary transfer of patients. Therefore, when neurosurgeons decide upon the operating place for EVD, they should consider the benefits of ICU operation and be cautious of EVD-induced CNS infection.
