1.Functional endoscopic sinus surgery for odontogenic maxillary sinusitis; A case report.

The chronic paranasal sinusitis are caused by various orign such as dental, general and anatomic problem. Among them, the odontogenic maxillary sinusitis which concerned by cral & maxillofacial surgeon have been treated by medicine, conservative treatment such as endodontic treatment, and Caldwell-Luc operation. But, New technique, so called Functional Endoscopic Sinus Surgery, have recently been performed in the ENT dept. This technique have the mechanism that widening maxillary ostium through removal of involved nasal mucosa and adjacent tissue accelerates adequate drainage and regeneration of normal mucosa and have merits such as minimum morbidity and complication. So, we would report one case, 22 years old female, of 3 patents who were diagnosed as odontogenic maxillary sinusitis and treated by Functional Endoscopic Sinus Surgery and Apicoectomy, and had good results.

2.Therapeutic result of lavage and manipulation technique for closed lock cases of the TMJ.

The Pumping technique generally has been known as a diagnostic method that predicts intracapsular status and selects required evaluation and treatment. But, in addition to this indication, when it is followed by Lavage and Manipulation technique, this may be used as therapeutic method that is effective on Closed Lock with limited mouth opening and pain. Therefore, we examined results of this technique abut Closed Lock and would like to report therapeutic effects of Lavage and Manipulation technique. We selected 40 patients of 200 patients diagnosed as Closed Lock over 53 months from SEP. 1991 to PEB. 1996, who were possible follow-up and performed by Lavage and Manipulation technique. Appraisal between pre-procedure and post-procedure was performed just after procedure, in 1 month and in 3 months, and therapeutic effects were divided by marked, useful, a little useful and ineffective. As a result, Lavage and Manipulation technique had remarkable increase of maximum mouth opening(>5mm) in 39 persons(98%) just after proceure, 36 persons(90%) in 1 month, and 39 persons(98%) in 3 months, and subsided pain completely just after procedure and had remarkable decrease of pain in 30 persons(75%) in 1 month, 32 persons(80%) in 3 months. So we concluded that the Lavage and Manipulation technique is effective therapy of Closed Lock which can be used with ease.

6.MANDIBULAR CONTOURING SURGERY BY MULTIPLE STEP SURGICAL CORRECTION WITH ANGLE-SPLITTING OSTECTOMY.

The mandibular contour determines the shape of the lower part of the face and thus influences the appearance of the face. A patient with a large, squarish, or broad face who desires a small, round, or slender face can undergo mandibular contouring surgery to reduce the width of the lower face. The successful correction of a prominent mandibular angle by conventional angle ostectomy has been reported. But, in the majority of patients with a widened facial appearance, both the mandibular angle and part of the mandibular body anterior to it are protuberant laterally, so both must be resected. The purpose of this study is to introduce a new method of performing mandibular contouring surgery, more effectively and easily, and to reduce postoperative complication and evaluate its results. We treated 6 patients who has prominent mandibular angle using multiple step osteotomy with angle-splitting ostectomy. The advantages of this new method are as following. (1) easily performable (2) effective mandibular contouring surgery by reducing the width of lower face (3) producing a natural relief of the mandibular angle (4) low risk of soft tissue damage and complications (5) shortening of the operation time. etc.

9.Evaluation of the Feasibility of Applying Metabolic Target Volume in 4D RT Using PET/CT Image.

In this study, we evaluated feasibility of applying MTV (Metabolic Target Volume) to respiratory gated radiotherapy for more accurate treatment using various SUV (Standard Uptake Value) from PET images. We compared VOI (Volume of Interest) images from 50%, 30% and 5% SUV (standard uptake volume) from PET scan of an artificial target with GTV (Gross Tumor Volume) images defined by percentage of respiratory phase from 4D-CT scan for respiratory gated radiotherapy. It is found that the difference of VOI of 30% SUV is reduced noticeably comparing with that of 50% SUV in longitudinal direction with respect to total GTV of 4D-CT image. Difference of VOI of 30% SUV from 4D-PET image defined by respiratory phase from 25% inhalation to 25% exhalation, and GTV from 4D-CT with the same phase is shown below 0.6 cm in maximum. Thus, it is better to use 4D-PET images than conventional PET images for applying MTV to gated RT. From the result that VOI of 5% SUV from 4D-PET agrees well with reference image of 4D-CT in all direction, and the recommendation from department of nuclear medicine that 30% SUV be advised for defining tumor range, it is found that using less than 30% SUV will be more accurate and practical to apply MTV for respiratory gated radiotherapy.

10.Histological Evaluation of Gore-Tex(R) used in Nasal Augmentation of Human.

BACKGROUND AND OBJECTIVES: Although Gore-Tex is popularly used in nasal augmentation in human, studies about the histological changes have been carried out only in animal models. The purpose of this study is to investigate histological changes of Gore-Tex used in nasal augmentation in human. SUBJECTS AND METHODS: Gore-Tex of 24 patients who underwent nasal augmentation using Gore-Tex (non-reinforced sheets) 2-36 months ago and had re-operated for the sole cosmetic problems was obtained. Histologic examination was performed from specimens including Gore-Tex and the surrounding soft tissue with light microscope and electromicroscope. RESULTS: Histologically, connective tissue ingrowth was observed in all specimen. But the degree of foreign body reaction and collagen deposit varied from specimen to specimen and had no relationship with the duration. Neovascularization was observed from specimen whose duration was longer than 12 months and degenerative calcification was observed from 36 month old specimen. CONCLUSION: Unlike the findings of previous reports, individual differences and partial degenerative lesions in Gore-Tex were observed. Additional studies are required to confirm the stability of Gore-Tex as an implant to be used in human.
