1.Experimental Cyclocryothermy in Rabbits.

The effects of Cryosurgery of the ciliary body on the aqueous humor dynamics of ten albino rabbits were studied by perfusion experiments. The cryo-probe (tip diameter, 3 mm) was frozen by dry ice and alcohol mixture and applied over the full circumference of the eye 2 mm from the limbus for 30 seconds. Eight applications were made in each eye. Intraocular pressure fell during the first 10 days after surgery, the amount of fall averaged 2.5 mmHg (12.6% of the pre-treatment level). Thereafter the intraocular pressure rose gradually in all eyes and reached almost to the original level 3 weeks after surgery. Facility of outflow changed inconsistently, showing no definite pattern. Aqueous flow showed significant reductions during the 2 to 3 weeks after treatment, then gradually recovered. There was no significant reduction in flow after the end of 3 weeks. No complications were encountered except mild iridocyclitis during the first week. The unsatisfactory outcome of this cryotherapy is considered to be due to the insufficient freezing of the ciliary body, and more applications is recommended in order to produce more sustained reduction of intraocular pressure.

2.Electroretinogram in Experimental Ocular Siderosis.

Experimental ocular siderosis was induced in fourteen albino rabbits by the insertion of iron fragments through the scleral incision made at equatorial region. ERGs were recorded up to 30 days after the insertion of iron, both in scotopic and photopic conditions. Light stimulus used was electronic photoflash of 2 msec duration. The changes in the scotopic b-wave were classified into 3 types. The first type (4 eyes) showed gradual decrease finally attaining 54.6% of the original amplitude. The second type (3 eyes) showed supernormal b-wave during whole period of study, the increase reached 43.0% after 30 days. In the third type (6 eyes) the b-potential remained almost stationary. The scotopic a-wave changed in a similar way as the b-wave. Sometimes negative (-) type ERGs were recorded, especially during the falling stage of the b-potential. The photopic a- and b-waves changed almost parallel to the scotopic components. By slitlamp biomicroscopy and ophthalmoscopy, fibrous connective tissue proliferated around the iron fragments, never extending deep into the vitreous body. Fundi were well visible except the insertion site. Histological section revealed moderate scar tissue formation around the insertion site of iron, and mild visual cell and ganglion cell destructions.

3.Experimental Studies on Vitreous Implantation.

Effects of implantation of fresh and stored vitreous were studied on albino rabbits. Auto-and bomo- implantations of fresh vitreous were performed in sixty eyes, and course was followed up to one month after implantation operation. Of these, thirteen eyes were discarded because of bleeding into vitreous cavities and contamination of vitreous by blood. In remaining forty-seven eyes, vitreous became clear after ten days. Intraocular pressure which had been low daring the first few days, normalized within this period of time. No complication was observed. Implantation of homologous vitreous which had been stored in frozen state for one to three months, resulted in clear vitreous in eight out of ten eyes operated. Bleeding and blood contamination occurred in two eyes. From the results, it can be concluded that vitreous implantation is relatively safe and effective procedure.

4.Ultrasonography in experimental vitreous Opacities.

Vitreous hemorrhages of white rabbit eyes were obtained by the following three methods and their ultrasonographies were checked several times for four weeks. 1 st group: fresh human blood was injected into the vitreous cavity through the sclera, 2 nd group: oxalated human blood was injected into the vitreous cavity in the same way, 3 rd group: a 26 gauge needle was inserted into the vitreous cavity and retinal vessels were mechanically ruptured to cause vitreous hemorrhage. Ultrasonography of the above groups revealed no definite differences between groups, but a high echo from the vitreous hemorrhages in initial stage changed to several low echoes in later stage. This fact was well matched with funduscopic observation for gradual absorption of the vitreous hemorrhage.

7.Mercury Contents of Scalp Hair by Consumption Pattern of Fishes, Shellfishes and its Products.

This study was conducted to examine the levels of total and organic mercury in the scalp hair of the elementary school children and adults, and their relalionship with the consumption pattern of fishes and shellfishes. The scalp hair samples were collected from the occipital part of 115 children and 131 adults in costal, urban and rural areas of kyungpook province from June to August 1991. The mercury content was analysed by the atomic absorption spectrophotometer (model IL. 555)with atomic vapor accessory (model IL. 440). The total and organic mercury contents of hair were significantly higher (P<0.01) among the children who prefer fish and/or shellfish (7.728ppm, 6.610ppm), and canned fish and/or fish pastes (6.969 ppm, 5.885ppm) than those who prefer meat(4.822ppm, 3.905ppm) and vegetables(3.974ppm, 3.224 ppm). The children who prefer to eat the canned fish without cooking showed a higher mercury content than the children who prefer to eat it as stew or mixed with vegetables (P<0.01l). There was a dose-response relationship between the intake frequency of canned fish, raw fish and cooked fish and the content of total and organic mercury of hair; the children who eat fish almost everyday showed 2 times higher than those who eat rarely (P<0.0l). The mercury content in the hair of the children who eat raw fish was significantly higher than that of the children who eat boiled or broiled fish (P<0.01). The total and organic mercury contents of adult scalp hair increased with age up to the forties and slightly decreased in the fifties. The mercury contents of those who were engaged in the fishery and raw fish restaurant were 2 times higher than those of the farmers. The mercury content of the persons who were favorite dishes of fish and shellfish were most high, and who prefer raw fish were 2 times higher than those of the persons who prefer vegetables and broiled fish. The contents of total and organic mercury in adult scalp hair showed also a increasing tendency with the intake frequency of raw and cooked fish.

9.Clinical Observation on Poor R-Wave Progression.

Poor R-Wave Progression(PRWP) of precordial leads is frequently encountered electrocardiographic findings of uncertain significance and has simply been deemed as suggestion of anterior myocardial infarction without concrete ground. 217 cases with poor R-Wave Progression have been analyzed on clinical records and results are as follows. 1) PRWP was most frequently found in fifties and sixities, comprising 63.9% of the subjects. 2) Co-existent disease entities with PRWP were classified into three categories, cardiovascular diseases, chronic lung diseases and normal variants. 3) The cardiovascular diseases related with PRWP were mainly hypertensive diseases, comprising 59.8% of cardiovascular diseases, followed by ischemic heart disease, valvular heart disease and cardiomyopathies. 4) PRWP may be an early sign of acute myocardial infarction in a certain part of cases, which was endorsed by typical clinical symptoms and enzyme studies. 5) As the criterion of PRWP, V3R equal to or less than 3 mm was thought more adequate for higher specificity rather than 4 mm.

10.Stomal Complications in Infants and Children.

We performed this study to analyse the morbidity and mortality of stoma formation in infants and children over a 17-year period. Thirty-seven stoma formations were performed in 37 patients: 21 for anorectal malformation, 9 for Hirschsprung's disease, 3 for necrotizing enterocolitis, 2 for multiple ileal atresia, 1 for volvulus neonatorum with perforation, and 1 for diaphragmatic hernia with colon perforation. There were 26 boys and 11 girls with a mean age of 0.4 years. Complications after stoma formation were encountered in 12 patients(32.4%) and included stomal prolapse, stenosis, retraction, dysfunction, skin excoriation and parastomal hernia. Four patients(10.8%) required stomal revision. The incidence of complications was neither related to the age nor to the primary indication for the stoma formation, but sigmoid colostomy was associated with a lower complication rate compared to transverse colostomy(22.1% versus 42.1%, P<0.05). Five patients died, but only one(2.7%) was dead, which was directly related to stoma formation. Eighteen of these children subsequently underwent stoma closure which was associated with complications in six patients(33.3%). The most common complication after stoma closure was wound sepsis in 4 children. In conclusion, because the significant morbidity of stoma formation still exists the refinements in surgical technique may help in reducing the incidence of complications and a sigmoid loop colostomy should be used whenever possible.
