1.Comparative Studies on the Effects of Experimental Cryopexy and Diathermy Coagulation on Rabbits ERG.

Twelve albino rabbits were treated with cryoretinopexy on the left eye and surface diathermy coagulation on the right eye at the equatorial region in two parallel rows concentric with the limbus. Cryopexy was applied for 10 seconds with Krwawicz's probe (the diameter of the tip was 1.5 mm) after cooling with dry ice and alcohol mixture. Diathermy coagulation was performed with 40 mA currents for 2 seconds with needle electrode without perforating the sclera. After the treatment, ERG was recorded with varying length of intervals up to 30 days. The light stimulus was given by 100 watts incadenscent lamp and the illumination at the eye was 800 lux, its duration being 20 msec. Only the sizes of the b-potentials after 30 minutes of dark adaptation were considered. The patterns of the b-potential changes after cryopexy were roughly divided into 3 types. The first type was seen in 5 eyes, and it consisted of abrupt reductions, followed by slight augmentations, never reaching the pre-treatment level of b-wave even after 30 days. The second type (seen in 3 eyes) showed initial slight decreases followed by marked increases and remained supernormal at the end of 30 days. The third type(4 eyes) showed marked increases from the early post-operative days and returned to the original level after 30 days. These changes were demonstrated graphically in figures 3, 4 and 5. By diathermy coagulation, the same kinds of patterns were observed. The same animal, however, did not show the same pattern as with the cryopexy. The final b-potential after 30 days were 105.4% of the pre-operative value by cryopexy and 82.7% by diathermy coagulation. Thus the cryopexy produced less severe changes in ocular tissues than the diathermy coagulation. The histological observations performed also supported this conclusion.

2.Clinical Experiences with Sustained-Relase Diamox Sequels.

The tension lowering effects of Diamox and other carbonic anhydrase inhibitors are well known and they are widely used against a various forms of glaucomas. This paper deals with the clinical experiences in normal and glaucomatous human eyes with Diamox-Sequels, kindly supplied by Lederle Laboratories and Yuhan Corp. MATERIALS AND METHODS: Normal healthy persons, 15 females and 20 males, were studied on their 45 eyes. Glaucomatous disease had been ruled out by careful examinations of intraocular pressure, visual fields and fundus. They had been also checked for the diurnal variations of intraocular pressure during the daytime. One capsule (500mg) of Diamox Sequels was given to each individual and the changes in the intraocular pressure were studied at various time intervals up to 53 hours after the administration. Tonometry was carried out by the Goldmann's applanation tonometer. The effects of Diamox Sequels on the glaucomatous eyes were studied in the same way. The glaucomatous eyes included 10 eyes of 7 chronic simple glaucoma patients, 5 eyes of 3 chronic narrow-angle glaucoma patients and 5 eyes of 5 secondary glaucoma subjects. They received one capsule of Diamox Sequels and the intraocular pressure was measured one or two times daily. The results of long-term treatment were also studied by daily administration of one capsule of Diamox Sequels, follow-up periods being from 3 to 19 days. No medication, oral or local, of antiglaucomatous drugs were allowed for at least 3 days prior to this study. The differences due to age, sex and type of glaucoma were not considered here. At the same time, the effects of a single 500mg of ordinary Diamox tablet were studied in normal ~md glaucomatous eyes. RESULTS: (1) Normal eyes. The intraocular pressure began to fall 1 hour after the administration. The tension gradually fell and the maximal reduction of tension occurred after 9.8 hours and the extent of maximal fall was 4.5mm Hg on the average, 29.4 % of the original intraocular pressure. The tension gradually rose and returned to the original level between 23~50 hours (average 43 hours). Side effects were noted in 4 cases, e.g., 11.4 %. (2) Glaucomatous eyes. The effects were noted within the first 1~2 hours and became maximal after 13 hours, the extent of the maximal fall 40.1 % of the original pressure, e.g., 16.2 mmHg. The original tension was restored after 30 hours. By long-term administration of Diamox Sequels, 8 eyes(40 %) showed excellent results, their intraocular pressure maintained below 21 mmHg. Sede-effects were noted in 3 cases(20.0 %). COMMENTS: The effects of Diamox Sequels and Diamox tablet on normal and glaucomatous subjects were summarized in table 1. As shown, Diamox Sequels has the longer duration of hypotensive actions and the time of maximal fall of intraocular pressure delayed considerably, as compared with Diamox tablet. In almost all cases, its action continued for more than 24 hours. In glaucomatous eyes, the time of maximal reduction of ocular tension was slightly later and the maintenance of low tension was slightly shorter than those in normal eyes. The extent of the maximal fall, however, was almost the same after administration of Diamox Sequels and Diamox. Side effects were encountered in 11~20% of cases by Diamox Sequels, and in 40-60% of cases by Diamox. This is one of the most striking advantages of Diamox Sequels over Diamox. Thus it is concluded that Diamox Sequels has the longer hypotensive actions and fewer side effects.

3.A Morphological Study of the Branches of the Axillary Artery in Korean Female.

The branches of the axillary artery have been studied in 38 Korean female cadavers. 1. The superior thoracic artery arose from the first part and the second part of the axillary artery in 84% of sides. 2. The thoracoacromial artery arose from the axillary artery near the superomedial border of the axillaly artery more often (91%) than from any other source. 3. The lateral thoracic artery was found on 64% of the sides as a main axillary artery, usually arising from the second part of the axillary artery(61%). It arose from the subscapular artery in 28%. 4. The subscapular artery was seen as the lagrgest of the axillary artery. It arose from the second part (42%) and the third (58%) of the axillary artery. 5. The posterior circumflex humeral artery was a direct branch of the third part of axillary artery in 37% of sides. In 21% fo sides the posterior circumflex humeral artery arose by a common stem from the third part of the axillary artery. In 33% of sides the posterior circumflex humeral artery arose from the subscapular artery. 6. The anterior circumflex humeral artery was found more constantly at the third part of the axillary artery than the posterior circumflex humeral artery was. It arose from a direct branch of the third part of the axillary artery in 70% of sides.

4.A study on improvement of school lunch program in a demonstration school (II).

It is obvious that adequate nutrition is essential for growth and development of school children, and many elementary schools in this country have already practiced it. Therefore, it would seem apparent that the school feeding program would have a significant effect on the growth and development of school children. This paper presents a two-year experimental school-feeding program from 1973 to 1974, and attempts to evaluate its effects by before-and-after nutrition surveys conducted in two elementary schools, one experimental and the other as a control. The two schools are both located in the same county (Yongin-Kun, Kyunggi-Do), and the families of their students are presumed to share the same socio-economic level. To assess the effect of school-feeding, we measured height, weight, chest circumference and grasping power. Physical examination was done foresigns of nutritional deficiency. A stool examination for parasites and blood examinations for hemoglobin, hematocrit and serum protein were included. Analysis were done for 150 students selected randomly at the beginning of the program. These students attended the school throughout the program period. Results are as follows: 1. The amount of increase of height, weight, chest circumference and grasping power were greater in the experimental school than in the control school, but the differences are not statistically significant. 2. Signs of vitamin deficiency decreased in both experimental and control schools during the two-year program period. 3. At the time of the 1974 post-survey, values of Hb. & Hct. revealed no significant differences between the two schools, but serum protein level was a little higher than that of general Korean rural children of the same age. 4. Infestation rate of parasites had increased in both schools during the two-year program period. 5. Each student of the two schools was classified into three major classes, according to the level of economic condition of his or her parents, namely higher, middle and lower. The results of each class of the experimental school was compared with that of the corresponding class of the control school, expecting the relative magnitude of change largest in the lower economic class of the experimental school. However, change was greatest in the middle class, still not being statistically significant. Finally, the authors concluded that the two-year period for such a program is not sufficiently long for its beneficial effects to be demonstrated and measured. As long as the growth and development of children are concerned, planning with a more distant perspective is required, as well as the development of new methods of evaluation.

5.A Study on Peripheral Blood T and B Lymphocytes in Leprosy.

In leprosy, it is generally believed that the defense against Mycobacterium leprae (M. leprae) is largely dependent on cell-mediated immunity (CMI) and there is an accumulating evidence that the patients with lepromatous leprosy have not only defective CMI specific for M. leprae, but also have generalized impairment of CMI. In contrast, the humoral immune reactivity, both specific and nonspecific, have been found to be normal or slightly increased. Recently, several investigators have enumerated the peripheral blood T and B lympocytes in patients with leprosy; however, there is no agreement as to the results. Furthermore, leprosy has been found to differ in its clinicai manifestations and immunological behaviors in different geographic areas and races. Nevertheless, there is yet no report in Korea concerning the T and B lymphocyte enumeration. The authors considered it is necessary to enumerate the peripheral blood T and B lymphocytes in Korean patients with leprosy to evaluate the immune status and the differences in the immunologic reactivity between the lepromatous and the tuberculoid spectrum of leprosy. 6.Ocular Thermometer Utilizing Thermistor.

A sensitive thermometer utilizing thermistor as a sensing element has been constructed in order to measure the temperatures in various ocular tissues. The Wheatstone bridge was used to detect the changes in thermistor resistance. The out-put of the bridge was fed into a single-stage differential DC amplifier and read by an micro-ammeter. There was a linear relationship between temperature and meter scale. The thermistor was inserted into the inferior conjunctival sacs of 158 healthy normal human eyes. The average temperature was found to be 35.14 +/- 1.08 (in C). No difference due to sex and age distribution was found to exists. Preliminary temperature measurement has been done on rabbits. Their results are shown graphically in Fig. 3.

8.The Influence of Miotic and Mydriatic Drugs on the Choroidal Blood Flow in Rabbits.

The changes in the choroidal blood flow induced by the intravenously administered miotics (pilocarpine, 1mg/kg) and mydriatics(atropine, O.1mg/kg) were investigated in rabbits with the use of the blood flow measuring apparatus according to the principle of Grayson's internal calorimetry, thermistors as sensing elements. Atropine caused slow increase in the choroidal blood flow after transient decrease, the maximal increase being about 85% of the pre-injection level. Pilocarpine also produced moderate increase of the blood flow by more than 100% of the original level. From these data it is concluded that both atropine and pilocarpine caused significant increase in the choroidal blood flow in rabbit.

10.Electroretinographic Studies in the Experimental Retinal Degeneration.

In order to investigate the changes in the scotopic and photopic ERG induced by the acJrninistrations of retino-toxic agents, 30mg/kg of body weight of sodium iodate and sodium iodoacetate were given in albino rabbits intravenously. Animals were anesthetized with intra venous urethane injection (1.5g/kg) and the pupils were dilated with atropine. The eye was stimulated with a single light flash (800 lux at the animals eye) of 200 msec duration. Prior to the recording of the scotopic ERG, animals were preadapted to light of 150 lux for 30 minutes. During dark adaptation, b-potential increased rapidly for the first 6-8 minutes followed by slow augmentation in normal eye. On the cessation of dark adaptation, b-potential dropped immediately, its decrease amounting up to 80% of the maximal potential attained during dark adaptation. By the administration of sodium iodate, c-wave disappeared immediately accompanied by apparent increase in the slow cornea-negative potential. No changes were noted in the b-potential during the first hour after iodate. Slow recovery phase during dark adaptation disappeared. Thereafter, b-potential diminished progressively with deepening of the slow negative wave. In some cases, b-wave almost extinguished 3 hours after iodate, and in another cases, some recovery of the b-wave was noted 48 hours after iodate administration. Iodoacetate produced immediate fall of b-potential with slow decrease of negative potential which progressed to the complete disappearance of the whole ERG responses. In some cases, b-wave reappeared 5 hours after iodoacetate, with prolongation of its duration. At 24-48 hours after the administration, no electrical response to light stimulus was elicited. From the obtained ERG findings, it can be deduced that the functions of the pigment epithelium could be disturbed secondary to the degenerations of visual cells. No correlation could be demonstrated between the ERG changes and the EOG changes after administration the toxic agents. Histological examinations of eyes at various intervals after administrations of retinotoxic agents revealed the degenerations of the pigment epithelium, sensory organelles of the visual cells and inflammatory changes in the choroid by iodate; and destruction of visual cells with preservations of the pigment epithelium by iodoacetate.
