1.Faun Tail.

We report herein a rare and grotesque case of a 19-year-old male with faun tail on the lumbosacral area of the back, presented from birth. The lesion was a 10 × loan sized patch with the circumscribed tuft of coarse terminal hairs about 10 to 25 an in length downward like a horse-tail. In the center of the patch was a 4 × 3cm sized brown macule in which was a 2 × lan scar. The skin biopsy from the brown macule revealed mild hyperkeratosis with basilar hyperpigmentation. Lumbar radiography showed findings of spina bifida occulta affected from L-4 to total sacral segments. There was mild saddle-hypoesthesia in the neurologic study.

3.Clinicohistopathologic Study of 121 Patients with Subcutaneous Lipomas.

We reviewed the clinico-histopathologic aspects. of 121 patients diagnosed as subcutaneons lipamas in 1981~1986, Additionally, the size of lipoma cells was statistica,lly compared with normal controls. The results were obtained as follows: The majority of patients visited the surgical department, specially general surgcry, whereas only 7 visited dermatology. The ratio of solitary to multiple type was 10:1. Females predominated in the patients with solitary type, outnumbering males in multiple type. While solitary type consisted of 98 patients (89.1%) with SL, 10 with FL and 3 with AL, multiple type had 7 with SL and 4 with AL. There were prominent incidences in the 5th and 6th decades followed by a slow decline. Faur of all 6 AL patients were multiple type. Males were affected in all but one. All AL patients showed non-infiltrating. The torso was mostly involved in 60.4% In 7 of all 17 AL lesions, the upper extremity was predorninantly involved. Sixty sevenr of the 136 masses were less than,3cm in diameter. In AL lesions, 76.5% was less than 3cm and likely to be smaller than other 2 types. The presumed time of onset was most common in 1~3 years The size of liporna cells was found to be slightly larger in only SL specimens than in controls(63.65+432pm and 56.32+6.50um respectively; p<0.0l).

4.Four Cases of Porphyria Cutanea Tarda.

Porphyria cutanea tarda(PCT) is a photocutaneous disorder due to excessive porphyrins in the skin. We experienced 4 male patients who showed typical clinical manifestations. Histologic findings revealed subepidermal bullae with festooned derrnal papi11ae, and typical porphyrin excretion pattern of PCT was detected. One case had on 500mg daily of chloroquine with r.linical and biochemical improvement 4 months later without any adverse effects. Other 2 cases had on 125mg of chloroquine twice a week and the other one had perforrned phlebotomy.

5.A Case of Subungual Exostosis.

Subungual exostosis is not a true exostosis, but an outgrowth of normal bone tissue, which is traumatic disputedly in causative factor, involving mainly the great toe. We experienced a case which a hard nodule, l. 0 crn in diameter, of the great toe of the left foot, developed in a 24-year-old woman. Her lesion had begun about 6 months earlier, accompanying the dull pain or tenderness when wearing the shoes or walking. Confirmatory diagnosis was biopsy with biopsy and X-ray findings after nail avulsion. We performed the lesion with curettage on therapeutic trial without effect.

6.A Case of Granuloma Gluteale Infantum.

Granuloma gluteale infantum is a granulomatous lesion in the diaper area of infants. Athough usually seen in infants, it has been described in incontinent elderly patients. The causes of granuloma gluteale infantum are not clear, although child care equipment(ditergents1, diapers2, and starch3), corticosteroids2,4,5,6, candidial infection3 urine and feces7 have been postulated for its etiology. We report a case of granuloma gluteale infantum in a 5-month-old boy. He was healthy except for an asymtomatic 1.5×1.5cm sized, round, Rat-topped brownish and firm nodule on the perianal area, and two, 1×3cm and 1×6cm sized brownish plaques on the intertriginous area of the neck. The lesions were resistant to the treatment of systemic antibiotics. These nodules completely resolved in the three months after we tried to keep the diaper area clean without using any corticosteroids or other ointments and after changing from plastic diapers to cotton o A histological examination of the nodule on the perianal area showed mild hyperkeratosis, spongiosis, and focal infiltration of neutrophils in the epidermis. 7.Clinical and Micological Studies on Microsporum Canis Infections.

Microsporurn canis was fjrst reportaR m Korea in 1957, but no animal contacts were reported and since then, only sporadic occurrences of M. canis have reported. However, cat and human infections from M. canis have gradually become rnore prevalent lately. Recently author reported 3 cases of T. capitis, 2 cases of Kerion Celsi, 13 cascs of T. e,orporis in human and 4 cases of infected kittens, with M. canis isolated as the causative organisrn. Clinical and mycological observations of both patients and kittens were reported as follows: The infections with M. canis were exclusively reported in kittens, not in adult cats. Two of the infected kittens showed obvious signs of undernourishment, physical retardation and prostration. Infectivity among cats or kittens was very low while cat to human infectivity was very high. The human T. capitis exhibited. round, erythematous lesions with complete 1oss of hair, very similar to lesions of alopecia areata, These lesions were uniformly smaller than a walnut in size. The T. corporis exhibited typical, moderately inflamed ringshaped, singular or multiple lesions. The incubation period was estirnated at approximately 3-7 days. In humans, ectothrix growth was reported while the opposite endothrix growth was observed in the kitten and rabbit. Experimental animal inoculations were performed: Rabbit inocuIated with M. can- is resulted in typical ringshaped Iesions appearing 10 days after inoculation. T4ese lesians were cornpletely gone in 4 weeks but infected hairs still remained. Guinea pig inoculation resultel in non-typical lesions covered with serosanguinous crusts, but rat inoculztion showed no signs of infectivity.

8.Simultaneous Bilateral Spontaneous Pneumothorax.

The simultaneous bilateral spontaneous pneumothorax is very dangerous, so the chest drain insertion should be done urgency. Contrary to the unilteral pneumothorax, the patient with simultaneous bilateral pneumothorax complains a severe respiratory distress, cyanosis and chest pain without the tension pneumothorax. Now, we present two cases of the simultaneous bilateral pneumothorax. The prompt insertion of chest drains relieved symptoms. One case is resolved by the bilateral thoracotomy, the other by the insertion of chest drains only.

9.Perfect State of Trichophyton mentagrophytes and Their Related Infections.

A mating study was performed by crossing 333 isolates of Trichophyton mentugrophytes with+and tester strains of Arthroderma vanbreuseghemii, A. benhamiae and A. simit from January 1980 to December 1983 Three hundred and thiirteen were compatible with A. vanbreuseghemri consisting of 167 of+type and 146 of type and twenty were indeterminate. The cleistothecia formed by crossing the isolates with tester strains of A. vanbreaseghemii were fewer in number and smaller in size than those of tester strains. The sexual dogeneration was observed in all of the isolates compared with tester strains. The colonial morphological features of the isolates of T. mentagrophytes were observed as 24p isolates of the granulosum-asteroides form, 66 of the powdery form and 7 of the downy form on 1/10 Sabouraud agar with salts in culture. The mat- ing type showed the higher type in the granulosumasteroides forrn and + type in the powdery and downy forms. The + type increased from 1981 to 1983, and the type showed a marked increase in 1983. The + type was predominant in tinea pedis et unguim and tinea cruris, and type in tinea faciale and tinea manuurn. The + type showed nearly equal incidence in the spring, winter and summer, and the lowest in the autumn, compared to the highest in the winter and the lowest in the summer in the type.

10.Study of Fingerprint Arches in Alopecia Areata.

Alopecia areata(AA) is a common dermatologic disease, the origin of which is still unknown. At least, it has been associated with some genetic influences in the etiopathogenesis. We have now evaluated AA patients to study the actual association of the genetic role by fingerprint analysis. The incidences of six or more arches were determined in 74 female and 86 male patients excluding alopecia totalis and universalis, and in healthy 167 female controls, based on Selmanowitz's thesis (1974). The results of this study are follows: 1. The difference between the frequency in AA patients(7. 5%) and in controls (3.3%) reaches the statistical significance(p<0.05 by x2-test). 2. The association of genetic background in AA is strengthened in the group of female patients(p<0. 01), recurrent episode(p<0.01). and multiplex farm (p<0.01). 3. Twenty patients(12. 5%) have family history. The incidence in 3(15.0%) of these patients is considerably common but not statistically significant. A correlation of biologic relevance and family predisposition in AA is not precluded. 4. The frequency of arches in patients and in controls is increased on both second fingers and decreased on both fourth fingers. It is more common on both second and left third fingers in female patients. The incidence of arches averages about 6 67% in patients and 4. 58% in controls.
