2.Tacrolimus ointment: An Open study for Effects on Severe Facial Atopic Dermatitis in Korean.

BACKGROUND: In recent studies, tacrolimus ointment has been shown to be effective for the treatment of atopic dermatitis with an excellent safety profile. OBJECTIVE: This study was done to assess the efficacy and side-effects of tacrolimus ointment for the facial atopic dermatitis(AD) in Korean. PATIENTS AND METHODS: Open-label, non-comparative study with 2 months' follow-up was done to assess the efficacy and safety of tacrolimus ointment (Protopic0ointment 0.1%, Fujisawa, Japan) in moderate to severe facial AD. Patients were instructed to apply it two times daily for 8 weeks. Facial lesions were evaluated at baseline, 4 and 8 weeks of treatment with in-tensity score by investigator. RESULTS: In comparing of intensity scores and each clinical score at baseline with those of 4 and 8 weeks of treatment, a significant decrease was noticed at all follow-up periods. Burning sense(54.5%) and pruritus(18.2%) were detected. CONCLUSION: Tacrolimus ointment is effective in treatment of severe facial AD and has tolerable mild adverse effects at the site of application in Korean.

3.The Effects of Basic Fibroblast Growth Factor(bFGF)on Type I and VII Collagen Gene Expression in Cultured Dermal Fibroblast.

BACKGROUND: bFGF, a member of the fibroblast growth factor family, potently induces vascular smooth muscle cell proliferation and decreased synthesis of the collagens. OBJECTIVE: For further investigation of the effect of bFGF on extracellular matrix homeostasis in the skin, we evaluated the expression of type I and type VII collagen gene at the transcriptional levels. METHOD: We examined that recombinant human bFGF affects the expression of genes involved in ECM synthesis and remodeling in human dermal fibroblasts cultures as judged by Northern blot analysis. RESULTS: The steady state levels of type I and VII collagen gene mRNA were decreased with age dependent pattern up to 0.13 and 0.44 folds respectively. The transcriptional levels of type I collagen mRNA were increased by TGF-B, treatment but markedly decreased by bFGF as well as TNF-a. But there were no synergistic effects bFGF and TNF-a on type I collagen gene expression. The levels of type VII collagen gene expression were increased by both bFGF and TGF-B,. The TNF-a showed slightly antagnostic effects on type VII collagen gene expression. CONCLUSION: The type I and VII collagen gene expression in dermal fibroblasts is clearly subjected to modulation by the cytokines including bFGF with uncoordinate regulatory pathway. In addition to its function of vascular proliferation, bFGF also may play a major role in physiologic skin condition and in repair process such as formation of a stable dermoepidermal junction during skin wound healing.

4.Correlation between Microalbuminuria Checked by RIA & Micral-Test®.

It is evident that an elevation of airway albumin excreation rate without clinical proteinuria strongly predicts a later progression on diabetic renal disease. So we studied the correlation between Microalbumin checkly RIA & Micral-Test®. We collected urine between 08:00 h and 08:00 h next day and then checked microalbuminuria by radioimmunoassay method and Micral-Test® The results are as follows: 1. There was significant correlation between microalbuminuria checked by RIA & Micral-Test® 2. There was poor correlation between diabetes duration or HV-A1c and maximal change in albumin excreation rate. 3. So we concluded that Micral-Test® can be used in laboratory instead of RIA.

5.Slipped Femoral Capital Epiphysis: Report of Two Cases

Slipped femoral capital epiphysis is a rare entity in Korea and only three cases have been reported so far1,2,3). One of our cases was a unilateral mild chronic slip in a 14 year old boy with Frohlich body type. It was treated by gentle closed reduction followed by knowles' pin fixation. The other case, a 20 year old female, presented chondrolysis of the left hip. Scrutinization of roentgenograms revealed typical features of an unrecognized slipped femoral capital epiphysis. It was treated by Wagner resurfacing replacement arthroplasty.

7.A Case of Acute Purulent Pericarditis with Pericardial Performation by Esophageal Foreign Body.

Purulent pericarditis is an infrequent but fulminant and frequently lethal disease. Purulent pericarditis tends to occur as direct extension of bacterial pneumonia or empyema in past. In recently, purulent pericarditis tends to occur in adult via contiguous spread from an early postoperative infection after thoracic surgery or trauma, infection related to infective endocarditis, extension from a subdiaphragmatic suppurative source, and hematogenous spread during bacteremia. Endogenous causes of purulent pericarditis are frequently characterized as esophageal perforations. Common causes of esophageal perforations related to purulent pericaditis which usually develop in association with mediastinitis, pneumonia and empyema include corrosive esophagitis, complication after esophageal and tracheal instrumentation and Boerhaave's syndrome. There is very little reference to the development of pericarditis in associated with esophageal perforation which does not directly communicate with the pericardium. while, although most uncommon, it is well documented that the esophagus can perforate directly into the pericardium and produce pericarditis. We experienced a case of acute purulent pericarditis after esophageal and pericardial perforation by a small fish bone in a previously healthy man. The patient was treated successfully with systemic antibiotics and pericardiotomy.

8.Obesity Indices and Obesity-Related Quality of Life in Adults 65 Years and Older.

BACKGROUND: Obesity is known to influence on physical, mental, functional health problems but there have been no study of relationship between body mass index (BMI) and quality of life (QOL) in Korean elderly. METHODS: A total of 216 elderly who had been attending two geriatric welfare facilities in Seoul answered the Korean version of obesity-related quality of life (KOQOL) questionnaire. Height, weight, waist circumference were measured. Body fat (%) was measured with HTM20 by impedance technique. Sociodemographic and medical factors were interviewed. QOL was assessed using KOQOL (ver.1) after excluding two sexual life related questions and one work related question. The total score is 48 points. The higher is the score, the poorer is the QOL. RESULTS: In multivariable analysis, BMI, age, history of diabetes mellitus, history of osteoarthritis were associated with KOQOL score. Especially, BMI was significantly associated with KOQOL score after adjusting for waist circumference, body fat (%) (P = 0.002). CONCLUSION: The more BMI increases, the worse the obesity related QOL is in Korean ambulatory elderly.

9.Host-parasite relationship in trematodes I. Susceptability and development of Clonorchis sinensis in rabbits.

In order to study host-parasite relationship in trematode, the susceptibility in rabbits of Clonorchis sinensis were examined. A certain number of metacercariae of C. sinensis were administered orally to rabbits, and rats and dogs were used to compare with rabbits. the results were summerized as follows: The rabbits were successfully infected with metacercariae of C. sinensis in all case of the groups given 10 to 500 each. But the highest recovery rate of the worm was 33.0 percent in the case given 100 metacercariae. In the early stage of infection, the worms were hardly found, but 10 days after the infection, the number of worms were detected easily. Three months after the infection, the recovery rate of worms was 31.3 percent in the rabbits given 300 metacercariae. The recovery rates of the worms in two months after the infection were 27.4 percent in rabbits, 51.0 percent in rats and 54.6 percent in dogs. Morphologically the body length and the body width markedly increased in the period until 10 days after the infection, and after that the genital organs began to get mature gradually while the growth of the body showed down. The measurement of adult C. sinensis recovered from the animals two months after the infection was the largest in rabbits followed by rats and the worms from dogs showed the smallest measurement.
