1.Anatomical Reduction of All Fracture Fragments and Fixation Using Inter-Fragmentary Screw and Plate in Comminuted and Displaced Clavicle Mid-Shaft Fracture.

PURPOSE: To report the treatment results of anatomical reduction of all fracture fragments and internal fixation using an inter-fragmentary screw and plate in displaced mid-shaft clavicle fracture with comminution. MATERIALS AND METHODS: Between June 2005 and August 2011, 13 consecutive displaced clavicle fractures with comminution (Edinburgh classification IIB2) treated by anatomic reduction and internal fixation using inter-fragmentary screw and plate were retrospectively evaluated. There were 11 male and 2 female patients with a mean age of 37.4 years (15~55 years). The right clavicle was injured in 4 patients and the dominant arm was involved in 46%. The mean duration from trauma to surgery was 7.0 days. The cause of injury was a traffic accident in three, a fall in two, and sports activity or direct injury in eight patients. All of the fracture pieces were anatomically reduced and fixed with inter-fragmentary screws. An additional plate was applied to maintain and reinforce the reduction of the fracture. Radiographic assessments for the numbers of fragments and the amount of shortening and displacement were performed. To verify the fracture healing and determine the time from fracture surgery to union and complications, all of the radiographs taken after surgery were evaluated. RESULTS: The number of fragments was 2 in 7 cases, 3 in 5 cases, and 6 in one case. The mean shortening of the clavicle was 1.1 cm (0.3~2.1 cm) and mean displacement between the main fragments was 2.6 cm (1.3~4.5 cm). The mean duration of follow-up was 16.5 months (8~26 months). Radiographic union was achieved in all patients with a mean time to union of 10.8 weeks (8~14 weeks). There were no complications including metal failure, nonunion, or infection. CONCLUSION: Anatomical reduction of all the fracture fragments and fixation using inter-fragmentary screws in addition to the usual plate fixation showed good fracture healing in displaced clavicle fracture with comminution.

2.Analysis of Clinical Characteristics and Correct Diagnosis Rate Associated with Spontaneous Intracranial Hypotension in the Emergency Department.

PURPOSE: Spontaneous Intracranial Hypotension (SIH) is rare condition and may accompany other clinical symptoms which inhibit accurate diagnosis in an emergency department (ER). Only a few studies have reported the clinical characteristics and root causes associated with SIH. This study evaluates the rate of accuracy of SIH diagnosis and compares clinical characteristics and diagnostic test results for correct versus incorrect diagnosis groups. METHODS: Medical records of SIH patients admitted to an emergency department (ER) over a twelve year period were retrospectively reviewed. Patients were grouped as having received correct or incorrect diagnosis, and their clinical characteristics and diagnostic test results were compared. In the incorrect diagnosis group, the number of times they were misdiagnosed, and the specialties of the clinic (s) they visited prior to arrival at this ER were reviewed. RESULTS: Adhering to the inclusion criteria of our study, 72 patients were enrolled with 54 patients in the correct diagnosis group and 18 patients in the incorrect diagnosis group. Of the twenty one cases in the incorrect diagnosis group, the majority 7 cases (33.3%) had been examined by an emergency physician. Among the clinical symptoms observed, there was significant variability in the location of the headache (p=0.020) and time interval between symptom onset and diagnosis (p=0.035). CONCLUSION: There were no differences in most of the clinical observations and diagnostic test results between the correct and incorrect diagnosis groups. To improve the correct diagnosis rate, it is suggested to have 'SIH' included as a differential diagnosis when encountering patients reporting headache in the emergency department. Emergency physicians should be required to recognize clinical SIH characteristics such as orthostatic headache.

3.The Usefulness of Multidetector CT Arthrography in the Diagnosis of Shoulder Pathology: Comparison with MR Arthrography and Arthroscopic Findings in the Same Patient.

PURPOSE: The purpose of the study is to evaluate the usefulness of CT arthrography (CTA) by conducting a comparative study of CTA and MR arthrography (MRA) with the arthroscopic findings in the same patients. MATERIALS AND METHODS: Forty nine patients who suffered from shoulder disease underwent MRA and CTA concurrently. The sensitivity and specificity of CTA was evaluated, as compared to MRA, in the case of four types of shoulder pathological lesions. The accuracy of CTA was evaluated by analyzing the arthroscopic findings of thirty four patients. RESULTS: Compared to MRA, CTA showed a sensitivity of 90% and a specificity of 100% for diagnosing supraspinatus tendon (SST) full thickness tear, and CTA showed a sensitivity of 100% and a specificity of 97.5% for making the diagnosis of superior labrum anterior to posterior (SLAP) lesion. For diagnosing partial articular side supraspinatus avulsion (PASTA) lesion, some studies have shown the usefulness of CTA with a sensitivity of 71.4% and a specificity of 97.7%. However, for diagnosing SST bursal side partial tear, the sensitivity of CTA was as low as 10%. CTA has been shown to be relatively accurate when the diagnoses were verified with the arthroscopic findings; diagnosing SST full thickness tear and SLAP lesion with CTA has shown an accuracy of 100% and87.5% respectively, and CTA showed 71.4% diagnostic accuracy for PASTA lesion. CONCLUSION: CTA was a useful tool and it was equivalent to MRA for the assessment of SST full thickness tear, SLAP lesion and PASTA lesion, but not bursal side partial tear. Thus, it may be used preferably to diagnose shoulder pathology and for follow up as an inexpensive tool after operation.

4.Effect of the New Emergency Fee Schedule on the Pattern of Emergency Patients.

PURPOSE: This study was conducted to solve the problems due to overcrowding of emergency medical centers (EMC) and to help EMCs to improve their financial status. Korean government has implemented a new emergency fee schedule into National Health Insurance (NHI). This policy is aimed at reducing non-emergent visits to EMCs. METHOD: This study was conducted to analyze the impact of the new policy by using a before-after comparison of the patterns of patients' visit to an EMC, Asan Medical Center (AMC). Data electronically recorded at the EMC of AMC were reviewed retrospectively. RESULT: The new emergency fee schedule reduced the rate of increase of non-emergent visit to EMC. CONCLUSION: However, this policy could not succeed in limiting total number of EMC visits. It means that this pricing policy made a partial success in solving the problem of overcrowding of EMCs.

Talocalcaneal coalition known as the most common tarsal coalition is a congenital failure of segmentation. Talocalcaneal coalition with complete bone bridge is very rare, and there are few references in the literature about the clinical results and the operative method for this type of tarsal coalition. We report a case of 15 years old female with complete talocalcaneal coalition, who showed good clinical results by lateral sliding calcaneal osteotomy.

OBJECTIVE: Hibiscus protocatechuic acid (PCA) is a food-derived polyphenol antioxidants used as a food additive and a traditional herbal medicine. In this study, PCA was to determine its effect on cell proliferation and cell cycle progression in primary cultured human uterine leiomyoma cells. METHODS: The effect of PCA on cell proliferation and cell cycle progression was examined in the primary cultured human uterine leiomyoma cells. MTT reduction assay was carried out to determine the viability of uterine leiomyoma cells. Cell cycle analysis for Hibiscus protocatechuic acid treated leiomyoma cells was done by FACS analysis. DNA fragmentation assay was performed to determine fragmentation rate by PCA in leiomyoma cells. Western blot analysis was done using anti pRB, anti-p21(cip1/waf1), anti-p53, anti-p27(kip1), anti-cyclinE, anti CDK2 antibodies to detect the presence and expression of these proteins in PCA treated myoma cells. RESULTS: PCA induced growth inhibition in a dose dependent manner, treatment with 5 mmol/L PCA blocked 80% cell growth. FACS results showed that there was increased the percentage of cells in sub G1. DNA fragmentation assay by ELISA was done to find the rate of apoptosis. Apoptosis took place but in a dose dependent manner. From Western blot analysis it revealed PCA induced the expression of p21(cip1/waf1) and p27(kip1) increasingly and was not mediated by p53. Caspase-7 pathway was activated and dephosphorylation of pRB took place. CONCLUSION: In Conclusions, PCA, a polyphenol antioxidant, inhibited cell proliferation and induced cell cycle arrest at sub G1 phase by enhancing the production of p21cip1/waf1 and p27kip1. These results indicate that PCA will be a promising agent for use in chemopreventive or therapeutics against human uterine leiomyoma.

7.Femoral Component Rotation of Total Knee Arthroplasty in Korean Subjects: A Study Using Grand Piano Sign and Computed Tomography.

PURPOSE: We aimed to obtain a reference for the optimal rotational alignment of femoral component in Koreans through an analysis of grand piano sign observed during total knee arthroplasty (TKA) and the angle between the clinical transepicondylar axis and the posterior condylar axis (TEA-PCA angle), by computed tomography. MATERIALS AND METHODS: The study subjects included 24 patients (30 cases), who underwent the anterior femoral resection, which was applied at an external rotation 3degrees relative to the posterior condylar axis during TKA. The relationship between anterior femoral resection with external rotation of 3degrees and the morphological pattern of Grand piano sign was evaluated. On postoperative computed tomography images, we evaluated the relationship between the clinical transepicondylar axis, posterior condylar axis of femoral component and grand piano sign. Moreover, a total of 28 Korean patients with 41 arthritic knees scheduled for a TKA had a preoperative computed tomography. We measured the TEA-PCA angle to evaluate the normal range in Korean subjects. RESULTS: The morphological pattern of grand piano sign observed after the anterior femoral resection, which was applied at an external rotation 3degrees, showed statistically significant correlation with TEA-PCA angle. On preoperative computed tomography images, the TEA-PCA angle was 6.01 degrees (range from 3.90 to 7.86). CONCLUSION: The TEA-PCA angle of Koreans was different from that of the westerners. More external rotation was needed for the optimal femoral rotational alignment theoretically, and we could confirm the correlation between Grand piano sign and rotational alignment of femoral component by measurement of grand piano sign and computed tomography.

8.Seasonal distribution of cockroaches, a major source of indoor allergens, in seoul metropolitan area.

PURPOSE: Home exposure to aeroallergens are an important environmental factor in allergic sensitization and in the development and exacerbation of asthma. Most studies of aeroallergens were dust mite in Korea, but cockroach distribution was little been known in Korea. The aim of this study was to evaluate species of cockroaches and seasonal distribution of cockroaches in Seoul area. METHODS: The study was performed during April 2000 through January 2001, from 63 houses in Korea. The total of 63 residential homes from different districts in Seoul were selected for the home environment survey. Bait traps (10 x 20 x 4 cm2, Zoro Zoro, Taisho Pharmacy, Tokyo, Japan) were placed on four different places of kitchen floor and one in the side of balcony in the house and left for 5 days to capture cockroaches. RESULTS: The cockroaches were captured in 23 houses(36.5%) out of 63 houses. And the inhabitant density of cockroaches was highest in the autumn(43.3%) followed by summer (29.2%), spring(18.3%) and winter(9.2%). In only 3 species were collected in Seoul area. The most common species were Blattella germanica showing 72.5% of the collected cockroaches, followed by Periplaneta japonica(25.2%) Periplaneta americana(2.3%). The total number of captured cockroaches was 504 and the number of nymph was 314(62.2%). Isolated house type showed higher trapping rates than the apartment. Positive skin test rates were 46.2% in the healthy control group and 43.8% in the allergy patients. CONCLUSION: Periplaneta americana was captured in Seoul area, which had not been found previously. The inhabitant density of cockroaches was the lowest in the winter.

9.Measurement of Dog and Cat Allergens Detected During Normal Domestic Activity; in Seoul Metropolitan Area.

PURPOSE: Allergens that cause asthma include those derived from indoor allergens such as animal dander (dog and cat). The aim of the study is to provide baseline data on characteristics of home environments in Korea, which will be used for future comparative studies of indoor environmental factors between populations with contrasting asthma prevalence. METHODS: The study was performed during September through November (Autumn) 1999. A total of 206 residential homes were volunteers from different districts in Seoul. They participate in home environment survey and skin prick tests. The dust specimens were collected by vacuum cleaner (V-582T, 520W; LG). We detected animal dander (Can f 1 and Fel d 1) by monoclonal-antibody based enzyme-linked immunosorbent assays (ELISA). RESULTS: The average indoor temperature was 25.1+/-2.9 degrees C and the relative humidity was 54.0+/-9.6%. The positive rate of dog (Can f 1) was 35.4% and cat (Fel d 1) was 33.5%. It is the same between Can f 1 and Fel d 1 distributed within dust samples from the four sites of the homes. And the distribution level of Can f 1 and Fel d 1 was, for the living room 26.2%, 17%, for the bedroom 20.9%, 15%, for the children's room 20.4%, 10.2%, for the kitchen 16%, 8.7 %, in descending order. CONCLUSION: The positive rate of Can f 1 was higher than Fel d 1 in dust samples. The living room has highest distribution of dust samples among the four sites of a home. And it has similar distribution between Can f 1 and Fel d 1.
