1.Serous Cystadenoma of the Pancreas: A case report.

Serous cystadenoma of the pancreas, also known as microcystic adenoma or glycogen-rich cystadenoma, is an unusually benign tumor. It is usually large and composed microscopically of many small cysts lined by small, cuboidal or flattened cells containing abundant glycogen. It has been suggested that serous cystadenoma probably arise from the ductular cells or centroacinar cells. Herein, we report on a case of serous cystadenoma of the pancreas in a 55-year-old female. The tumor, measuring 13.5x11.5x10.0 cm, was located in the head of the pancreas and the cut surface revealed a sponge-like appearance due to innumerable tiny cysts containing clear serous fluid. Microscopic analysis showed cystic spaces lined by cuboidal cells with intracytoplasmic glycogen.

A perplexing developmental error is 46,XX congenital absence of vagina ( Mayer-Rokitan-sky-Kster-Hauser syndrome, Mullerian dysgenesis, vaginal aplasia ). The Mayer-Rokitansky-Kster-Hauser syndrome refers to the climical entity consisting of primary amenorrhea associated with congenital absence of the vagina, 46,XX karyototype, a rudimentary uterus or complete absense, normal overian function and normal ovulation, normal female breast development, body proprotion and body hair, frequent association of renal, skeletal and other cpngenital anomalies. This syndrome results from agenesis of both Mllerian ducts or from a failure of the Mllerian ducts to estavlish a proper communication with that part of the vagina from the urogenital sinus. A case of mayer-Rokitansky-Kster-Hauser syndrom was reviewed briefly.

6.Expression of NMDA Receptor Subunit mRNAs in the Developing Circadian Pacemaker of the Rat.

Hypothalamic suprachiasmatic nucleus (SCN) is a circadian pacemaker which controls diurnal behavioral and hormonal rhythms in mammals. The SCN receives environmental light signals through the retinohypothalamic tract, and glutamate is the major excitatory neurotransmitter in the retinohypothalamic tract. In the present study, we investigated the developmental expression of the mRNAs for N-methyl-D-aspartate type glutamate receptor (NR)1, NR2A, NR2B and NR2C subunits in the rat SCN using in situ hybridization with specific riboprobes. At postnatal day 2 (P2), P8, Pl5 and P45, the high level of NRI transcripts was observed in both ventrolateral and dorsomedial subdivisions of the SCN, and the distinct expression of NR2C mRNA was principally found in the dorsomedial SCN. The weak NR2B mRNA expression was clearly found in both subdivisions of the SCN at P2 and P8, whereas specific NR2B hybridization signals were not found at Pl5 and P45. There was no specific hybridization signal of NR2A in the SCN throughout the postnatal life. These findings implicate that NR may play an important role in the neonatal SCN. In addition, this study suggests that NR1, NR2B and NR2C might be the major NR subunits in the developing SCN, whereas NRI and NR2C could be the subunit components of NR in the adult SCN.

7.A study on the pre-eruptive positional change of the lower third molar following orthodontic treatment.

Most of orthodontic cases are treated with extraction of certain teeth, which influence the pre-eruptive movement of the lower third molar. The purpose of this study was to evaluate the positional change of lower third molar following orthodontic treatment. Pre- and post-treatment pantomograms of 163 orthodontic patients (77 nonextraction group, 78 first premolar- extraction group. 8 second molar- extraction group) were analyzed in terms of the mesiodistal and buccolingual angles of lower third molar. The results were as follows. 1. The change of the mesiodistal angle of lower third molar by orthodontic treatment was significant in second molar-extraction group. 2. The mesiodistal angle of lower third molar in pre-treatment was significantly correlated to the mesiodistal angle in post-treatment and/or the change of the mesiodistal angle by treatment. 3. The change of the buccolingual angle of lower third molar by orthodontic treatment was significant in non-extraction group or first premolar-extrction group. 4. The change of the buccolingual angle of lower third molar by treatment was significantly correlated to the mesiodistal angle in post-treatment, the change of the mesiodistal angle by treatment, the buccolingual angle in pre-treatment or the buccolingual angle in post-treatment.

8.Differentiation Between Acute and Old Fractures in The Thoracic and Lumbar Spine: Observer Agreement.

Differentiation between acute and old fractures in the thoracic and lumbar spine is often difficult. We have evaluated the intraobserver agreement and interobserver variability of interpretations. We have prepared 50 cases from 40 patients with thoracic or lumbar spine fracture for evaluation. At first, two radiologist interpreted each of the 50 cases, Only by using plain roentgenograms. Then, they interpreted the cases again using also spine CT, MRI, bone scintigrams and follow-up roentgenograms. Intraobserver agreement was found to be 78% in Doctor A, and 76% in Doctor B. Interobserver agreement was only 62% at first. It was 88%, when they used CT, MRI, bone scintigrams and follow-up roentgenograms. It was not easy to differentiate the old fractures from acute one only by plain roentgenograms. Differentiation was more difficult, when the patient was female or over 55 years old. Plain roentgenography in hyperextension posture seems to be a simple but useful method for the diagnosis of the acute fracture. MRI was more valuable than CT or bone scintigraphy to differentiate the old fractures from the acute ones.

9.MRI Finding of Reversible Pituitary Hyperplasia Due to Hypothyroidism: A Case Report.

We report a case of reversible pituitary enlargement due to hypothyroidism in a 3-year-old male. OnT1-weighted images, the mass was located in the anterior lobe of the pituitary gland, and the signal intensity ofthe mass was equal to that of brain cortex. Gd-DTPA enhanced T1-weighted images showed homogeneous enhancement ofthe mass. After supplemental therapy with thyroid hormone for 8 months, MRI showed that the pituitary gland wasmarkedly smaller.
