Fourty cases of postpartum heart failure occuring in Korean women in the Dept. of Int. Med., Pusan University Hospital are reported, with and analysis of clinical data including frequency, age, time of onset, clinical features, hemoglobin, liver function, electrocardiographic and chest X-ray findings. The following results were obtained. 1. The cases were mostly found in 20-29 years age group, 50% of the total cases. 2. Eighteen cases of fourty patients experienced the symptoms during the 2nd week after delivery and 12 cases complained the symptoms during 4th week. 3. Only six cases noted onset of heart disease following the 4th pregnancy, whereas sixteen patients experienced this phenomenon after the first gestation, eight patients after the 2nd gestation and ten patients after the 3rd pregnancy. 4. At the admission the patients were suffering from paroxysmal nocturnal dyspnea, orthopnea, exertional dyspnea and generalized edema. Physical findings revealed cardiomegaly, gallop rhythm, edema, pulmonary fine rales and systolic murmur in frequency. 5. Electrocardiograms were available at the time of presentation in thirty patients. Twenty four patients had left ventricular hypertrophy, 18 patients sinus tachycardia, 3 patients lower voltage. 6. The chest X-ray film showed the cardiomegaly in 15 cases, 83.3% of the obtained 18 cases, 12 cases pulmonary edema, 3 cases pericardial effusion. 7. The venous pressure was measured in 15 patients. In 5 cases the venous pressure was normal and in 10 increased above 150mmH2O. 8. With prompt treatment by rest, salt restriction, diuretics and digoxin the clinical symptoms improved in 31 cases but 9 patients died at the admission.

PURPOSE: To compare the ability of three dimensional spectral-domain optical coherence tomography (3D OCT) and Stratus OCT to detect early glaucoma. METHODS: The optic disc topographic and retinal nerve fiber layer (RNFL) thickness parameters were measured by 3D OCT and Stratus OCT in 69 normal eyes and 48 early glaucoma eyes. The discriminating abilities of the two techniques for detection of glaucoma were compared by the area under the receiver operating characteristic curves (AUC). RESULTS: The best Stratus OCT parameters and criterion that differentiated normal from early glaucoma based on AUC were horizontal integrated rim width (0.85) for optic nerve head parameters, inferior quadrant (0.88) for RNFL parameters, and > or =1 clock-hour abnormal at the 5% level (0.81) based on the normative database for criteria. The best 3D OCT parameters and criterion that differentiated normal from early glaucoma were vertical cup-to-disc ratio (0.85), 11 o'clock RNFL thickness (0.86), and > or =1 clock-hour abnormal at the 1% level (0.78), respectively. When all corresponding the best parameters and criterion were compared, there were no significant differences between the AUCs for Stratus OCT and 3D OCT (p = 0.95, p = 0.73, p = 0.45, respectively). CONCLUSIONS: Stratus OCT and 3D OCT had similar diagnostic ability for detection of early glaucoma.

BACKGROUND: Neointimal hyperplasia, as the most important mechanism of restenosis after intracoronary artery stenting, its severity is closely correlated with the degree of local inflammatory reaction initiated by vasular injury during stenting procedure. So, we proceeded this study to determine whether inflammatory markers such as CD11b/CD18 (Mac-1) adehsion molecules of neutrophils, sICAM-1 (soluble intercellular adhesion molecule-1), ESR, and CRP increase or not in the peripheral circulation after coronary artery stenting, and whether there is any association between these findings and the degree of later restenosis. METHOD: 32 patients (chronic stable angina 4, unstable angina 17, acute myocardial infarction 11) underwent single vessel coronary artery stenting were enrolled in our study. Blood samples were obtained from peripheral vein just before coronary artery stenting and 48 hours thereafter. The degrees of CD11b/CD18 expression on the surface of neutrophils were analyzed by flow cytometry with monoclonal antibodies, and sICAM-1 by ELISA method. At each times, ESR and CRP were also measured. Follow-up coronary artery angiography was performed with QCA analysis at least 6 months later. We compared the each 48 hours values with the baseline (just before procedure) values. Percentage increments (as a ratio 48 hours values to baseline) of CD11b/CD18 expression, sICAM-1, ESR, and CRP levels were also compared with the results of follow-up QCA analysis. RESULTS: Restenosis (diameter stenosis > or = 50%) occurred in 6 patients (19%) at follow up angiography. 48 hours values of CD11b/CD18 expression, sICAM-1, ESR, and CRP were significantly elevated from the baseline values (each p values, CD11b : < 0.0001, CD18 : 0.01, sICAM-1 : < 0.0001, ESR : 0.005, and CRP : 0.001). The percentage increments of CD11b/CD18 expression were more elevated in restenosis group than nonrestenosis group (CD11b : 341+/-215%/74+/-95%, CD18 : 84+/-60%/17+/-37%, each p < 0.001, 0.001). There was some positive correlation between the percentage increments in the expression of CD11b and the late loss index at the follow up angiography (r=.43, p<0.05). CONCLUSIONS: Through this study, we found that the activation of neutrophils was occurred, and that sICAM-1 level was increased after coronary artery stenting in the peripheral blood. There was some correlations between the degree of CD11b expression on the surface of neutrophils and the severity of late lumen loss of inserted stents. The measurements of increased neutrophil adhesion molecules of CD11b/CD18 levels at 48hrs after coronary stenting may have a value as the predictor of subsequent late restenosis.

PURPOSE: To evaluate the usefulness of automated measurements of the localized retinal nerve fiber layer (RNFL) defects area in patients with glaucoma. METHODS: Fifty one patients with localized RNFL defects in RNFL red-free photographs and 53 healthy subjects were included. All participants were imaged with 3D spectral-domain optical coherence tomography (OCT). The area of defects was measured with the RNFL significance map (red = p < 1% and yellow = p < 5%) using Image J manually and Matlab software automatically. The area under the receiver operating characteristic curve (AUC) was calculated for the RNFL defect area of the RNFL photograph and RNFL maps, circumpapillary RNFL thickness, optic disc parameter, and macular inner retina thickness. RESULTS: High correlation was observed between manually and automatically measured defect areas in the significance map (red area r = 0.904, red and yellow area r = 0.890). The AUC for manually and automatically measured defects area (0.987, 0.966; p < 5%, p = 0.31, respectively) in the significance map was comparable. The latter demonstrated slightly higher but insignificant difference in AUC for inferior quadrant circumpapillary RNFL thickness (0.936; p = 0.22) and was significantly higher than the inferior ganglion cell layer plus inner plexiform layer thickness (0.894) and vertical cup to disc ratio (0.869) (p = 0.018, p = 0.008, respectively). CONCLUSIONS: The automated measurements of the RNFL defect area in the significance map performed adequately in detecting localized RNFL defects and had a better performance than macular inner retina and optic nerve parameters.

PURPOSE: We investigated the effectiveness and safety of DA-3030 for prophylatic use in patients receiving chemotherapy for malignant disease. MATERIALS AND METHODS: Seventy cancer patients were randomized to receive chemotherapy alone (36 patients) or with DA-3030 administered (34 patients) after stratified block randomization according to chemotherapeutic regimen. DA-3030 was subcutaneously administered at the dose of 100 pg/m/day for 10 days from 24 hours after the completion of chemotherapy. RESULTS: Of the 70 enrolled patients, 62 patients were evaluable. The neutropenia (absolute neutrophil count [ANC] <1,000/mm) occurred in 9 of 32 (28.1%) of the DA-3030 group and 21 of 30 (90.0%) of the control group, giving relative risk for control group of 0.154 (95% confidence interval [CI], 0.05 to 0.45; p-0.0001). Severe neutropenia (ANC 500/mm') occurred in 4 of 32 (12.5%) of the DA-3030 group and in 20 of 30 (66.7%) of the control group (relative risk for control group of 0.316 [95% CI, 0,18 to 0.55]; p=0.0001). The mean duration of neutropenic period (+/-standard error) was 1.13+/-0.34 days in the DA-3030 group and 6.73+/-0.69 days in the control group respectively, and was significantly shorter in the DA-3030 group (p<0.0001). And, there was higher nadir ANC in the OA-3030 group than that in the control group (p=0.0001); the mean nadir ANC was 2,547+/- 343/mm and 442+/-120/mm, respectively. The DA-3030 group had significantly higher incidence of myalgia in comparison to the control group (43.8% compared with 3.3%; p=0.001). However, it was tolerable and was easily managed by conservative therapy CONCLUSION: The use of DA-3030 was effective in preventing chemotherapy-induced neutropenia.
