5.Avulsion of the Tibial Attachment of the Posterior Cruciate Ligament of the Knee Joint (Seven Cases Report)

The posterior cruciate ligament plays as basic stabilizer of the knee. Therefore the knee becomes unstable when tibial insertion of the posterior cruciate ligament is avulsed. The diagnosis may be delsyed or missed because of surgeon's 1sck of awareness or rare condition. The authors treated seven cases of avulsion fracture of posterior cruciate ligsment at the tibial attachment ares ranged from ten to thirty-two years. There were five male and two female. The results were as follows : 1. The cause of all seven cases injury was traffic accident. 2. The physical findings were seven positive posterior drawer test, seven cases of abrasion or laceration over the anterior proximal tibial surface and antero-medial surfsce of the knee joint. 3. The associated injury was comminuted fracture of the tibia and femur (1 case), rupture of collateral ligsment and comminuted fracture of the tibia (1 case) and zygomatic fracture (1 case). 4. The treatment methods were four cancellous fixation, one K-wire fixation after epiphyseal plate fusion but each one pull out wire and dexon suture before epiphyseal plate fusion. 5. The motion of knee joint was within normal limit and no instsbility was inspected at the three months after reduction.

6.The Treatment of Infected Nonunited Fractures of Long Bone

Many difficult problems are encountered in the treatrnent of infected nonunion of long bone. To solve these difficult fractures, authors adopted an active approach-thorough debridement of the infected and necrotic soft tissue and bone, rigid internal fixation with autogenous cancellous bone graft and opening of the wound to ensure continuous drainage until union of the fracture. Authors treated 28 cases of infected nonunions with this method from Mar. 1980 to Feb. 1989, and the results are as follows ;1. Femur(54%) and tibia(43%) were most frequently involved sites. 2. Most of the cases(86%) were treated by rigid internal fixation and cancellous bone graft. 3. In most cases(64%), infection was controlled spontaneously by the time of bony union, but in 5 cases which showed persistent drainage, implant removal controlled the problem, and residual soft tissue and bony defect were solved by Papineau's technique or musculocutaneous flap. 4. Revision was required in 5 cases(18%) because of insercure fixation. 5. Union could be obtained in average 4.8 months, and infection could be controlled in all except 1 case.

7.A Clinical Study of Arthroscopic Meniscectomy

Ninety cases of meniscus injury were treated by arthroscopic meniscetomy between March, 1988 and March, 1992. Eighty four cases were treated by arthroscopic partial, 5 cases by subtotal, and 1 case by total meniscectomy. All cases were followed up for more than 1 year. The results were as follows. 1. The ratio of male to female was 57 to 31, and that of right to left was 52 to 38. 2. There were 41 cases of medial meniscus and 49 cases of lateral meniscus. Longitudinal tears were 35 cases (39%), oblique tears were 20 cases (22%), horizontal tears were 15 cases (17%), and radial tears were 9 cases (10%). There was no difference in the shape of tear between medial and lateral meniscus. 4. The most common associated injury was anterior cruciate ligament tear. 5. The Tapper and Hoover scale was used to record the clinical results, and satisfatory results (exellent and good)were in 86% of cases.

8.The Comparison of Effect of Epidural Morphine and of Morphine-Fentany1-Bupivacaine Mixtere for Analgesia Afrer Cesarean Section.

The purpose of this study is to demonstrate a new postoperative analgesic which minimize the physiological changes of patients. Recently, it has become popular to administer opioids to epidural space rather than intravenously or by intramuscular administration. However, the side effects of opioids have limited the usefulness of this procedure. We tried to find out a way of providing postoperative analgesia while minimizing the side effects of opioids. This study has tested the effects of administering a mixture of small doses of morphine, fentanyl and bupivacaine and epidural morphine alone. The data of this study has taken from 20 paturients who have scheduled for cesarean section. We have classified into two groups which consist of 10 paturients each. We have administered epidural morphine alone to the first group and a small doses of morphine-fentanyl-bupivacaine mixture to second group. We have evaluated analgesic effects by comparing both groups' pain scores of the operative day and of the first postoperative day and evaluated such side effects as the degree of itching, nausea, vomiting, dizziness and respiratory depression by asking to paturients. This study has found the followings: 1) Pain scores of the first group in the operative day and the first postoperative day are 1.2+/-0.4 and 0.4+/-0.5. The second group's pain scores are 0.2+/-0.4 and 0, and the second group's pain scores have significantly reduced (p<0.05). 2) Frequencies of such side effects as itching, nausea and vomiting are higher in the first group (8) than in the second group(4). In conclusion, small doses of morphine-fentanyl-bupivacaine mixture has demonstrated a more effective postoperative analgesia with minimum side effects in pain controlling after cesarean section than epidural morphine alone.
