1.Mating Study of Microsporum canis Isolated in Korea.

A total of 135 strains of M canis, composed of 1R4 from the patients and 1 from an infected pet cat, were crossed with the 2 tester strains of eVannizzia otae VUT 77054 "/" and VUT 77055 "-" supplied by Dr. A. Hasegawa. These 134 human strains were originated from 77 casea of tinea capitis, 20 of tinea faciei and 37 of tinea corporis from Taegu city, Kyungpook and neighboring provinces during the period from December 1975 to August 1982. Out of 134 hurnan isolates, 112 (83. 6%) were of "-" mating type of N. otae and 22 (16.4%) were nonreactive. None were compatible with the + mating type. One strain from a pet cat proved also to be of type. The majority of the tested strains were revealed to be sexually degenerated.

2.Is Pancapsular Release More Effective than Selective Capsular Release for the Treatment of Adhesive Capsulitis?.

BACKGROUND: We assessed the effectiveness of arthroscopic capsular release for the treatment of adhesive capsulitis. Further, we tried to ascertain the clinical benefits, if any, of pancapsular release over selective capsular release, where the two differ by performing or not performing a posterior capsular release, respectively. METHODS: Thirty-five consecutive patients with either primary or secondary adhesive capsulitis who failed conservative treatment for more than 6 months were enrolled in the study. A total of 16 patients allocated in group 1 received a pancapsular release that comprises the release of the rotator interval, anteroinferior capsular, and the posterior capsular release, whereas 19 patients in group 2 received a selective capsular release that comprises only the release of the rotator interval release and anteroinferior capsular release. The clinical outcomes, visual analogue scale (VAS) score, Constant score, and range of motion, were assessed preoperative and postoperatively. RESULTS: In both groups, the preoperative VAS score, Constant score, and ROM showed a significant improvement by the 6-month follow-up. We found that the immediate postoperative internal rotation was significantly higher in group 1 than group 2. Despite significant differences seen between the two groups at the initial postoperative period, there were no significant differences in Constant score, VAS score, and the ROM at all the subsequent follow-ups between the two groups. CONCLUSIONS: Arthroscopic capsular release for the treatment of adhesive capsulitis is very effective. However, pancapsular release did not show any advantage over selective capsular release in terms of overall clinical outcome.

3.A Case Combined Germ Cell Tumor in Testis.

Primary germ cell tumor of the testis is rare, which occupies 1 to 2% of all reported malignant male neoplasms. Combined primary germ cell tumor of the testis composed of embryonal carcinoma and seminoma is more rare tumor. The authors experienced a case of 50-year old male who presented with painless enlargement of right testicle. He has had radical orchiectomy and diagnosed by pathologically as combined germ cell tumor of testis. The testicle is measuring 180 gm in weight and 9×6×5 cm in dimension, and almostly replaced by tumor mass. Grossly the tumor is rubbery solid smooth tumor mass, with variegated cut surface with geographically outlined diffuse necrotic area. Histologically the tumor is composed of two components of tumor, which are solid growth pattern of large round to polyhedral cells with clear or granular cytoplasm and distinct cell border, and anastomosing glandular and papillary arrangement of anaplastic epithelial cells. The former corresponds to seminoma, and the latter to embryonal carcinoma. Each tumor lobule is separated by abundant fibrous stroma.

4.Effects of Mitomycin C on Cultured Rabbit Osteoblasts.

PURPOSE: To evaluate the effect of mitomycin C (MMC) on osteotomy site as an adjunctive therapeutic agent during dacryocystorhinostomy, the effect of MMC on cultured rabbit osteoblasts was tested. METHODS: Cultured osteoblasts which was obtained from the iliac crest of rabbits, were treated with MMC (0.2 mg/ml) for 5 or 30 minutes, washed and changed with fresh osteogenic media (Opti-MEM), and then cultured for 24 hours. To observe the effect of MMC dose dependency on cultured osteoblasts, four different concentrations of MMC (0.2 mg/ml, 0.02 mg/ml, 0.002 mg/ml, 0.0002 mg/ml) were applied into the cells and cultured for 24 hours. The effect of fibroblast growth factor (FGF) and transforming growth factor-beta(TGF-beta) on the MMC-treated cells was evaluated. In control group, osteoblasts were cultured in osteogenic media without exposure of MMC for 24 hours. Cell viability was measured using trypan blue staining method. RESULTS: As compared with control group, the growth of osteoblasts was inhibited by MMC (0.2 mg/ml) treatment, 30-minute treatment group demonstrated marked suppression twice as much as 5-minute treatment group. Growth rate of 0.2 mg/ml MMC-treated cells was highly suppressed to 7.7% of control and 0.02 mg/ml MMC-treated cells was inhibited to 15.4% in number. Growth rate of 0.002 mg/ml, 0.0002 mg/ml MMC-treated cells was diminished to 53.8%, 84.6% in number, respectively. Both growth factors had promotive effect on the growth of osteoblasts in 0.002 mg/ml MMC-treated cells, especially in TGF-beta. CONCLUSION: Osteoblasts which were treated for longer time and with higher concentration of MMC showed more severe suppression in growth rate. These results suggest that MMC could have some therapeutic effect on osteotomy site of dacryocystorhinostomy.

5.A study on the T lymphocyte subsets, plasma neopterin and serum lgE in patients with atopic dermatitis.

BACKGROUND: Many physiologic, pharmacologic and immunologic abnormalities were reported in atopic dermatitis but the cause and pathogenesis of the disease remain obscure. OBJECTIVE: This study was done to investigate the systemic immunologic abnormalities in atopic dermatitis. METHOD: To evaluate the cell mediated immunity, me quantified pei ipheral blood T lymphocytes and their subsets, using flow cytometery, and assessed plasma neopteiin levels by means of radioimmunoassay. To evaluate the abnormal humoral immunity, we assessed the serum IgE levels by means of enzyme-immunoassay. RESULTS: Mean proportions of peripheral blood T lymphocytes and, heir subsets in atopic Dermatitis patients were within normal limits. Hut the suppvessor/cytotoxic T lyrphocytes(T8) were significantly decreased in the group of se"ere atopic dermatitis compared with the group of mild atopic dermatitis(P<0.05). Plasma neopterin lervels in the group of atopic dermatitis were found to be significantly elevated as compared vith the control group(P<0.01), but no significant cifference was found between the mild and severe group of atcpic dermatitis(P>0.05). Mean serum IgE levels in the patients with atopic dermatitis were higher than reference value. But there was no significant difference between the mild and severe atopic dermatitis group. Serum IgE levels ivere negatiiely correlated with T8(r=-0.3774, P<0.05) and positively with T4/T8 ratio(r =0.5007, P<0.05). Conclusions : These data;uggest that the atopic der matitis has abr ormalities in cell mediated immunity as well as elevated IgE level.

7.A case of cutaneous sarcoidosis.

We report a case of cutaneous sarcoidosis which occurred in a 32-year-old female who had, in addition to some cutaneous nodules, recently developed an acute spontaneous erythematous area of induration in old scars on her forehead and lower lip. Histopathologic findings revealed well-demarcated islands of epitheloid cells with a few giant, cells. In addition, slight admixture of lyrnphoid cells with present. at the margins of the epitheloid cell granulomas. Complete regression of the skin lesions was obtained with systemic steroid therapy without recurrence for some 8 months.

8.A Case of Spontaneous Remission of Histiocytosis X.

Histiocytosis X(Langerhans cell histiocytosis) is a rare prolifertive disorder of Langerhans cells that includes Lettere-Siwe disease, Hand-Shiiller-Christian his ase and eosinophilic granuloma. Since many authors have reported in anintermediate and poarl classified form histiocytosis X has a namenclatural had nosologic problem. We report a case benign cutaneous variant of histiocytisis X. A four-month-old boy had shown multiple skin-colour papules on the trunk, head and nik for one month. Extensive in- vestigations failed to detcct any systemic involvement. The clixron microscopic findings of the skin biopsy specimen were Qefinitely diagnostic for histiocytosi. X Since he did not sbow evidence of internal organ involvernent and any further progression of he skin lesion, no therapy was given. Over the next two months the disease underwent spontane us and complete remission.

10.Change of Postoperative Distance Stereoacuity in Intermittent Exotropic Patients.

To determine whether distance stereoacuity improved following strabismus surgery in patients with intermittent exotropia, we tested distance stereoacuity of 107 patients preoperatively and postoperatively using the Mentor B-VAT IIVideo Acuity Tester. Following successful surgical alignment there was significant improvement of postoperative distance stereoacuity in binocular vision contour circles[BVC]and binocular vision random dot E[BVRDE]test[p<0.05 ]. Based on BVC testing, performance improved 45.8%of the patients after surgery, remained unchanged in 43.0%, and decreased in 11.2%. Based on BVRDE testing, performance improved 29.9%of the patients after surgery, remained unchanged in 66.4%, and decreased in 3.7%. But improvement of postoperative distance stereoacuity was not related statistically to age, operative methods, suppression, and preoperative size of deviation. From the results of this study, it is concluded that the BVC test is a useful test for assessing control in intermittent exotropic patients. Distance stereoacuity improved after successful operation, thus distance stereoacuity test may be helpful in evaluation of preoperative and postoperative intermittent exotropia control.
