2.Effect of Increased Intracellular pH by HMA on the Cytotoxicity of Combined Treatment of Hyperthermia and Chemotherapy.

PURPOSE: The enhanced cytotoxic effect of combined treatement of hyperthermia and chemotherapy by increasing intracellular acidify with HMA was investigated. MATERIALS AND METHODS: Fsall tumor cells were injected on the hindlegs of female C3H mice. When the tumor volume reached about 200mm3 , experiments were performed on the groups classified as follows : Group I : Control Group II : Melphalan alone (2.5 mg/kg, 5 mg/kg, 10 mg/kg, 15 mg/kg). Group III : Heat alone (42.5degree C for 1 hour) Group IV : Melphalan + Heat (42.5degree C for 1 hour) Group V : HMA(10 mg/kg) + Melphalan (5.0 mg/kg) + Heat (42.5degree C for 1 hour) Each group included 8-12 mice on each experiment. HMA (3-amino-6-chloro-5(1-homopiperidyl)-N-(diaminomethylene)-c-pyrazinecarboxamide), an analog of amiloride which increases intracellular pH(pHi) was dissolved in dimethyl sulfoxide(DMS) and injected into the tumor-bearing mice through the tail vein. 10mg/kg of HMA and each dose of melphalan were injected into peritoneum of the tumor-bearing mice 30 minutes before heating. Tumor growth delay was calculated when the tumor vlme reached at 1500mm3 . Excision assay was performed on each group and repeated 2-4 times. RESULTS: Tumor growth delay of each experimental groups at 1500 mm3 were 9, 10, 13 and 19 days respectively. In vivo-in vitro excision assay using Fsall tumor cells, the cytotoxicity of each experimental groups was 1.2 X 107 , 1 X 107 , 6 X 106 , 1.7 X 106 and 1 X 105 clonogenic cells/gm respectively. When HMA was added to the combined treatment of heat and chemotherapy, the tumor growth ws delayed more than combined treatment without HMA i.e., 6 days tumor growth delay at 1500 mm3 of tumor volume. CONCLUSION: he combined effect of cytotoxicity by heat and chemotherapy can be much more enhanced by HMA.

3.MRI of Vertebral Compression Fractures: Differentiation between Benign and Malignant Causes.

PURPOSE: To evaluate the MR image in the differentiation of benign and malignant lesion in compression of the vertebral body. MATERIALS AND METHODS: MR images of 47 benign(acute traumatic within one month:19, chronic traumatic longer than one month or nontraumatic:28) and 21 metastatic compression fractures were respectively reviewed in terms of margin of lesions, signal intensity, paraspinal mass formation, soft tissue change, and involvement of posterior element of vertebra. MR images of TI-(T1WI) and T2*-weighted gradient echo (GE T2WI) sequences were obtained on 0.5T unit in sagittal and axial orientation with 5mm section thickness. RESULTS: The margin of benign compression fracture was usually indistinct (acute fracture:90% (17/19), chronic fracture:68% (19/28)), whereas it was sharply delineated in metastatic compression fracture (92%, (12/13) (p<0.001). Paraspinal mass was seen in both acute traumatic and metastatic compression fractures (acute fracture :26% (5/19), metastatic fracture: 52% (11/21). Soft tissue change was seen only in acute cornpression fractures (58%, 11/19). Involvement of posterior element of vertebra was noted in metastatic fracture (71%, 15/21), acute fracture (32%, 6/19) and chronic fracture (7%, 2/28) CONCLUSION: On MR imaging, involvement of entire portion of a given vertebral body, sharp margin between normal and abnormal areas in partially involved cases, paraspinal mass formation, and posterior element involvement are more frequently seen in metastatic compression fractures, which are considered to be useful in differentiation between benign and malignant causes of compression fracture.

4.A comparison of the safety and efficacy of intravaginal prostaglandin E1 ( Misoprostol ) and prostaglandin E2 ( Dinoprostone ) to induce labor.

OBJECTIVE: Our purpose was to compare the safety and efficacy of intravaginally administered misoprostol(PGE1) versus dinoprostone(PGE2) for labor induction in a prospective controlled trial. METHOD: One hundred eleven patients for labor induction (including preterm rupture of membranes) were randomly assigned to receive either misoprostol 50microgram or dinoprostone 3mg intravaginally. The interval of doses was 8 hours after first dose, with a potential maximum of six dose until active labor pain was achieved. RESULTS: Among 111 patients enrolled, 55 were randomized to receive misoprostol 50microgram and 56 to receive dinoprostone 3mg with every 8 hours interval intravaginally. There were no significant differences in demographic characteristics except in Bishop score(3.81+/-1.52 vs 4.38+/-1.29, P<0.05). There were no significant differences in indications for labor induction. The interval of induction to delivery was shorter in the misoprostol group(750.8+/-518.8min) than in the dinoprostone group(1264.1+/-730.7min). Delivery within 24 hours after administration occurred more often in the misoprostol group than in the dinoprostone group (86.3% vs 65.2%, P<0.05). More than two doses for completion of delivery were frequent in dinoprostone group(36.96% vs 9.8% in misoprostol group, P<0.05). Additional Oxytocin augmentation was needed more commonly in the misoprostol group(3.64%) than in the dinoprostone group(14.29%)(P<0.05), but no significant differences were noted between two groups in total oxytocin doses and indications. Tachysystole occurred more often in the misoprostol group (12.73% vs 1.79% in the dinoprostone, P<0.05) Precipitating delivery occurred more commonly in the misoprostol group(21.82% vs 7.14% in the dinoprostone group, P<0.05). No differences in the cesarean section rate, fetal heart rate abnormalities, hyperstimulation syndrome, meconium staining, fetal outcome (Apgar score at 1 and 5 minutes, birth weight), and marternal outcome (nausea and vomitting, postpartum bleeding) were noted. The cost of misoprosl is one hundredth cheaper than dinoprostone. There was no significant differences in the fetal outcome and maternal outcome. CONCLUSION: These results suggest that intravaginal misoprostol(PGE1) is a safe drug for labor induction which is more effective and cheaper compare to intravaginal dinoprostone(PGE2).

5.A radiologic observation of male infertility by vao-epididymography and vasoseminal vesiculography

The vaso-epididymography and vasoseminal vesiculography is the only diagnositc method for investigating thepatency and abnormal lesion of male genital system radiologically. The author performed 53 cases ofvaso-epididymography and vasoseminal vesiculography in Busan National University Hospital during the 8 years from1973 to 1981. All patients complained of primary infertility more than a year and were confirmed azoospermia onsemen examination, and 23 cases of them underwent testicular biopsy. The results are as follows; 1. The agedistribution were mainly between 26 and 35 years old (83.0%). 2. The infertile duration were almost from 1 to 5years(86.8%). 3. The past history associated with infertility was gonorrhea, tuberculosis and scrotal trauma. 4.Of the 23 cases of confirmed by testicular biopsy, the pathologic findings were normal spermatogenesis in 12 cases(52.2%) and hypospermatogenesis or arrest type in 11 cases (47.8%). 5. Non-filling sites of contrast media onvas-epididymography and vasoseminal vesiculography were epididymis(83.0%), ejaculatory duct (41.5%) and seminalvesicle (24.5%), were commonly bilateral (68.0%) and were more prevalent in the right side than left (1.2:1.0). 6.The abnormal findings of ejaculatory ducts, seminal vesicles, vas ampullas and vas deferens were mainly strictureof obstruction, obstruction of dilatation, beaded appearance or obstruction and obstruction respectively. 7. Inthe epididymis, there was almost obstructive lesion at the tail portion(83.0%).

6.A clinical and radiological observation of shoulder arthrography

The authors analyzed 23 cases of shoulder arthrography performed at Busan National University Hospital and Inje Medical College Paik Hospital for about 2.5 years from March 1981 to Sept. 1983, both clinically and radiologically. The results obtained were as follows; 1. Of the 23 cases, 20 cases (87.0%) were male and 3 cases(13.0%) were female, and the most prevalent age group was second decade. 2. Right shoulder arthrography was performed in 16 cases (69.6%), and left in 7 cases (30.4%). 3. The number of cases which had had certain historyof trauma was 13(56.5%), and remainder of 10 cases (43.5%) had no history of trauma. 4. Frequent symptoms andsigns were pain in shoulder region in 16 cases (69.6%) and limitation of motion in 14 cases (60.9%). 5. On plainfilm findings, 18 cases(78.3%) were normal, and 5 cases (21.7%) were abnormal. 6. On shoulder arthrographicfindings, 16 cases (69.6%) were normal, and 7 cases (30.4%) were abnormal, Those abnormal cases were consisted of2 case (8.7%) of adhesive capsulitis, 2 cases (8.7%) of chronic shoulder dislocation, 1 case(4.3%) of rotator cuff tear, 1 case(4.3%) of loose bodies in joint cavity, and 1 case(4.3%) of rupture of biceps tendon sheath. 7. Among routine views of shoulder arthrography of normal cases, the external rotation view revealed axillary recess &biceps tendon more distinctly, and the internal rotaion view revealed subscapular bursa more clearly. 7.Three cases of earty dyskeratoma.

Warty dyskeratoma is a rare, well-circumscribed epidermal tumor that frequently aries as a single lesion on the skin of the head or neck of middle aged or older persons. Its histopathological features include : suprabasilar clefting with villous proliferations of the connective tissue papillae into the clefts ; acantholytic and dyskeratoic epithelial cells,basilar hyperplasia, and hyperleratosis. We report three cases of warty dyskeratoma describing clinical and histochemical fea tures.

8.An Experimental Study on Mixing of Chemoembolic Material for Hepatocellular Carcinoma.

PURPOSE: The chemoembolization with Lipiodol and doxorubicin hydrochloride is used in patients with hepatocellular carcinoma. What condition is the ideal emulsion of Lipiodol and doxorubicin for excellent anticancer effect? METHOD AND MATERIALS: Microscopic evaluation was performed on the emulsions, which were varied with different specific gravities of doxorubicin solutions, degrees in mixing of the emulsion, and amount of Lipiodol. RESULT: 1. Maximal amount of doxorubicin solution was contained in Lipiodol droplets and the release of doxorubicin from the droplets were delayed, when specific gravity of doxorubicin was equal to that of Lipiodol (SG, 1.28). 2. The optimal therapertic ratio of Lipiodol and doxorubicin was 3:2 at least, as in the emulsion less than 3:2, unmixed free forms of doxorubicin solution were increased. 3. The emulsion mixed by pumping 50--100 times had smaller Lipiodol droplets and contained larger amount of doxorubicin solution in the droplets than by pumping 20 times. CONCLUSION: We recommend the emulsion with specific gravity of doxorubicin equal to Lipiodol (SG. 1.28), the ratio of Lipiodol and doxorubicin closo to 3:2, and the mixture prepased with puming 50--100 times.

9.The Palliative Radiotherapy in Bone Matastases.

To objectively compare the response of the palliative radiotherapy in bone metastatic patients which decreases pain and prevents pathologic fractures, we introduced and applied the RTOG pain and narcotic measure system. From Oct in 1991 to July in 1993, thirty-two patients with painful bone metastases, 17 of them were solitary lesions and others were multiple lesions, were treated with mainly 6 MV photon otherwise 15 MV photon. Radiation doses to bone metastatic sites ranged about from 2000 to 4600cGy. Responses of radiation therapy were compared with days of pre-RT, RT finish, 3, 6, 9 months after the start of RT and solitary versus multiple lesions and follow up scores according to the RTOG measure system. Survival analysis was done. Pain and narcotic score of the entire patients were 7.3, 7.8 at the pre-RT period and 2.6, 3.9 at the immediate or 2 weeks after RT, which was 64%, 50% decrement compared with the pre-RT score. Pain scores of 3, 6 and 9 months after the beginning of irradiation were 3.6, 3.7 and 3.3. The best response found in the breast and prostate primaries was 84%, 78% decrement of pain score as compared with pre-RT score (statistically insignificant). Median survival was 5.5 months and mean survival was 5 months. We conclude that the RTOG pain and narcotic measure system in relatively effective scale in the comparison of before and after palliative irradiation to the painful bone metastatic sites but more detailed parameters will be required in the narcotic scoring system. More aggressive but less or similar toxic radiotherapy is needed in the patients having relatively long life expected time.

10.Analysis of TIGR Gene Mutation in Glaucoma.

In the present study, we have evaluated the mutations of the TIGR[Trabecular meshwork Inducible Glucocorticoid Response] gene, which has been reported to be associated with the glaucoma, in primary open angle glaucoma[POAG], normal tension glaucoma[NTG], and steroid-induced glaucoma[SIG]. We have analyzed the TIGR gene in 18 members of 3 families affected with POAG, 28 familially unrelated patients with POAG, 32 patients with NTG, 30 patients with SIG, and 45 normal subjects.DNA was extracted from the blood samples of each patient, exon 2 and exon 3 of the TIGR gene were amplified by PCR and DNA sequencing was performed.No mutation was found in familially unrelated patients with POAG.Two kinds of mutation[Ser341Pro, Gly367Arg]were found in 3 families affected with POAG.Another mutations, located in exon 3, were detected in one NTG patient and in one SIG patient, but they were silent substitution.Identification of TIGR gene mutation will provide early diagnosis of POAG before irreversible visual impairment develops in cases of positive family history of glaucoma.
