6.A Survey of the Controlling System for the Pulmonary Tuberculosis in Jejudo, 2002.

BACKGROUND: The nationwide tuberculosis registry system in Korea has been operated with the dual reporting system - public health center (PHC) and private medical services (PMS) - since 2000. While three-fifths of the registered cases in 2002 was reported by PMS, it was reported that the success rate of patients at PMS was lower than that at PHC. To identify the problems of the patients' management at the PMS as well as the PHC in a community is the aim of this paper. METHODS: The subject was selected from the registered cases of the pulmonary tuberculosis in Jejudo, 2002. The needed items for this study were gathered by the reviewing the medical charts directly. METHODS: Of the 262 patients included in the subject, 92 cases (35%) were treated initially at PMS. The sputum smear as an essentially diagnostic test was carried out with 69.2% at private clinics compared with 98.2% at PHC. The success rate of the treatment at PMS was 59.8% so that it had a statistically significant difference from that at PHC (80.0%; p<0.01). CONCLUSION: These results showed that there were several problems in diagnosis and treatment for the patient of pulmonary tuberculosis at PMS. To improve the success rate of the treatment at PMS, the nationwide clinical guideline for control of pulmonary tuberculosis should be systematically disseminated and strongly recommended at PMS by the government.

7.Intracranial and Extracranial Fusiform Aneurysms in a Patient with Neurofibromatosis Type 1: A Case Report.

Neurofibromatosis type 1 (NF-1) has a variety of localized or systemic manifestations. Among them, Cerebrovascular dysplasia can be very rare finding of neurofibromatosis which can be very rarely seen. Here we report a case of 17-year-old boy representing bilateral giant fusiform aneurysms of extracranial internal carotid arteries and intracranial aneurysms of left middle cerebral artery. He showed no related symptoms at all, but screening for vascular lesions and close monitoring is warranted in NF-1 patients considering that it can be symptomatic unexpectedly.

BACKGROUND: Metronidazole has been known as the most effective drug for treatment of Trichomonas vaginalis-related diseases. However, it has been reported that metronidazole has adverse effects and incidence of metronidazole-resistant T. vaginalis (CDC085) has increased. Development of new drug, which is effective against metronidazole-resistant T. vaginalis and showing no adverse effects, has been required. METHODS: The purpose of this study was to investigate effects of various extracts from herbs such as Quisqualis indica, Gleditsia sinensis, Prunus armeniaca, Morus alba, Platycodon grandiflorum, Ailanthus altissima, Stemona japonica, Biota orientalis, Dryobalanops aromatica, and Cimicifuga heracleifolia on metronidazole resistant strain of T. vaginalis in vitro (CDC085). RESULTS: Anti-Trichomonas activities were observed in T. vaginalis treated with G. sinensis, P. armeniaca, and P. grandiflorum on the growth and fine structure of metronidazole resistant strain of T. vaginalis. Of the three standard extracts that showed the most effective anti-trichomonas activity, G. sinensis was the most effective. The inhibitory effects of fraction extracts of this drug were shown on the growth of T. vaginalis. The fine structure of the cytoplasm was changed after application of G. sinensis extract. The number of polyribosome and hydrogenosome decreased whereas the number of food vacuole and vacuole in the cytoplasm increased, compared with that of untreated control group. CONCLUSION: The results of our study indicate that G. sinensis may induce the inhibition of cell multiplication as well as impairment of protein synthesis of metronidazole resistant strain of T. vaginalis in vitro.

10.Arthroscopic Treatment on the Osteochondritis Dissecans of the Adolescent Elbow.

PURPOSE: The purpose of this study was to evaluate the effect of arthroscopic management of osteochondritis dissecans(OD) of the elbow. MATERIALS AND METHODS: Seven patients rangeing from 13 to 20 years of age, and average age 15.7 years were evaluated in this study. All patients underwent arthroscopic removal of loose bodies followed by multiple drilling into the defect of the capitellum. The average follow-up period was 19.7 months (range, 16 to 28 months). Results were analyzed clinically using the elbow scoring system of Broberg and Morrey. RESULTS: There were five good and two excellent results. Three cases, however, showed slight medial instability while performing the valgus stress test. All patients had no or slight pain, and regained a full range of motion. Although four amateur athletes showed good results, they were unable to recover their pre-injury sports activity level. CONCLUSION: Arthroscopic removal of loose fragments for late stage OD enabled the evaluation of lesions of the anterior and posterior aspects of the joints, and minimized morbidity.
