2.A Clinical Study of Fracture and Dislocation of the Cervical Spine

The cervical spinal injuries may impose a fatal result or permanent neurological disability according to the severity of injury. Authors performed a clinical study consisting of 54 patients who have visited Daejeon Eulji General Hospital for the care of fracture and dislocation of the cervical spine from May 1981 to Jan. 1986, and obtained the following results. 1. The prevalent age distribution was between third and forth decade and the ratio between male and female was 3.5:1 and the most common cause of injury was traffic accident(53.7%). 2. The most common site of injury was CS, 6(22.2%) and the most frequent mechanism of injury was Aexion-rotation type(55.0%). 3. At initial examination, 39 patients(72.2%) had neurological damage and among these, 18 were complete paralysis below the level of injury, 12 were incomplete paralysis, 9 were nerve root injuries. 4. Operative treatment was performed on 24 patients, these were Rogers' posterior fusion on 19 patients, Brooks and Jenkins' atlantoaxial fusion on 3 patients, occipitocervical fusion on 1 patient and excision of hactured fragment on 1 patient. 5. There were no evidences of neurological recovery in completely paralyzed patients, but among incompletely paralyzed and nerve root injured patients, neurological recovery was found in 57.1% of conservatively treated patients and in 71. 4% of operated patients. 6. Radiological stability was found in entire patients who received operative treatment, but, in conservatively treated patients, remained 1 late instability.

3.Outpatient General Anesthesia for Mentally and Physically Handicapped Children Undergoing Extensive Dental Treatment.

BACKGROUND: Fear of dental treatment is a very real problem for many people. Very young patients or children that are mentally or physically handicapped have various problems that preclude routine dental treatment in the office and require general anesthesia for extensive dental restoration. In America, outpatient operations are performed in thousands of dental offices annually, but there is no report about outpatient general anesthesia in Korea. METHOD: A review of forty children treated under outpatient general anesthesia for extensive dental treatment between 1994 and 1996 inclusive was carried out to assess the patient selection, anesthetic method, recovery time and complication. RESULTS: The mean age was 8.4 years, and twenty-five percent of the patients were autism. The length of the postoperative observation period before discharge was 3.2 hours, and postoperative fever was major complication. CONCLUSION: For extensive dental treatment in handicapped children, we suggest that outpatient general anesthesia can provide reasonably safe treatment, while reducing its expense and requiring less hospital bed space.

4.Changing pattern of serum leptin concentration in women undergoing clomiphene citrate challenge test or controlled ovarian hyperstimulation.

OBJECTIVE: To evaluate the changing pattern of serum leptin level and the correlation between estradiol level during the clomiphene citrate challenge test(CCCT) or controlled ovarian hyperstimulation(COH) cycle. METHODS: Twenty-seven women who underwent CCCT and fourteen women who underwent controlled ovarian hyperstimulation were recruited to measure the serum leptin level. After correction of serum concentration with body mass index(BMI), changes of leptin level and correlation with serum estradiol level during CCCT and COH cycle were analyzed. RESULTS: Circulating leptin levels were significantly correlated with BMI at each time point in CCCT cycle(P<0.01). In CCCT cycle, leptin/BMI level was significantly increased at midluteal phase compared to that of menstrual cycle day 3 and 10(p<0.05). In women with regular menstruation, leptin/BMI level at midluteal phase was significantly higher than that of menstrual day 3 and 10, but this difference was not seen in women with irregular menstruation. The leptin/BMI level in COH cycle showed increasing tendency throughout ovarian stimulation. But there was no significant correlation between leptin/BMI and estradiol level in CCCT and COH cycle. CONCLUSION: There is a significant correlation between BMI and circulating leptin level. Midluteal leptin level is significantly higher than that of follicular phase in CCCT cycle, and there is an increasing tendency in leptin level after ovarian stimulation in CCCT and COH cycle without statistical significance. These findings suggest that circulating estradiol concentration has no major influence on circulating leptin level.

5.Immunohistochemical Localization of Adenohypophyseal Gonadotropes in Korean Native Goat , Capra hircus.

The localizations and morphological characteristics of gonadotropes in the adenohypophy-sis of Korean native goat were investigated with double immunohistochemistry. The gonadotropes were present in the pars distalis and pars tuberalis, but not in the pars intermedta. Gonadotropes occupied about 49.0% of the cells in the pars distalis in females, and about 40.8% in males. Three types of gonadotropes ; FSH immunoreactive cells[FSH cells], LH immunoreac-tive cells[LH cells], and FSH and LH immunoreactive cells[FSH/LH cell], were identified according to their immunoreactivities for FSH and LH antisera. The possessional perce-ntages of FSH cells, LH cells and FSH/LH cells were 1.1%, 40.6%, 58.3% in females and 1.8%, 30.0%, 68.8% in males, respectively. FSH/LH cells were large and oval or round in shape. These cells were distributed throughout the pars distalis, but were more abundant on the dorsal part adjacent to the hypophyseal cavity and along the lateral and ventral peripheral regions. LH cells were smaller than other gonadotropes and were observed throughout the pars distalis, but predominant in the central region. FSH cells were large and oval in shape. These cells were intercalated between FSH/LH cells.

6.A Case of Wasp Sting of the Eye.

We presented a case of wasp sting of 42 year old, farmer who got stung on the cornea directly. At the time of first examination, the patient had visual acuity of 0.02 in the right eye. Slit lamp examination revealed corneal epithelial desquamation and vesicle, striate keratitis, diffuse stromal edema and fine keratic precipitates on the corneal endothelium. The iris was depigmented and atrophic, and pupil was mydriatic and nonreactive. The lens was opaque at the center of anterior caspsule and fundus was not visualized. 4 months later, the corneal lesion was slightly improved. Clinical findings of iris and lens still unchanged.

7.Lung findings in experimental paragonimiasis.

A pathological study was done to elucidate sequential changes of the lungs in various time intervals following experimental paragonimiasis in 15 dogs and 15 cats. The dogs and cats were fed with 30-50 metacercariae of Paragonimus westermani, and were sacrificed at 15, 20, 30, 45, 60, 90 and 120 days after infection respecively. Autopsies were performed immediately after death. Gross and microscopic examination of the lungs showed following findings: There were no qualitative difference in pathological findings between dogs and cats. Pathological findings were first noticed at 20 days of infection in thoracic cavity, which consisted of fibrinous plueritis along with superficial hemorrhage. Although no worm was found in the lung parenchyma at this period, juveniles were seen in pleural cavity together with turbid effusion. Paragonimus juveniles were first recognized inside the lung parenchyma by 30 days of infection. This was the period when the lungs showed multiple areas of hemorrhage and probably active penetration by smaller worms. Hemorrhagic bronchopneumonia was quite pronounced from this stage through 45 days of infection. Paragonimus worm cyst was essentially composed of fibrous scar and heavy inflammatory cellular infiltrate. The lining epithelial cells were first became noticed by 2 months of infection. And these epithelial cells were thought to be probably transformed alveolar lining cells rather than bronchiolar epithelial cells. As the infection progress, the cyst wall became more stabilized and often showed squamous metaplasia. Fibrinous pleuritis with pleural effusion was very prominent finding in early periods of infection. Bronchiolitis and focal vascular sclerosis were often seen in experimental paragonimiasis.

8.Effect of Inorganic Polyphosphate on Cultured Periodontal Ligament Cells.

This study was designed to examine the effect of polyP on the periodontal ligament fibroblasts. The cells were incubated for 24 and 48 hours with various concentrations (0.001%, 0.002%, 0.005%, 0.010%, 0.020%, 0.040%) of polyP. Cell activity, alkaline phosphatase activity, and protein synthesis activity of the cells were examined. The results were as follows: 1. Twenty four hours after incubation with polyP, the cell activity of the periodontal ligament fibroblasts was increased. Forty eight hours after polyP application, the activity of the cells was similar to that of control group. 2. Twenty four or forty eight hours after incubation with polyP, the alkaline phosphatase activity of the periodontal ligament fibroblasts was increased. 3. The protein synthesis of the periodontal ligament fibroblasts was not changed after polyP application. These results suggest that, polyP may increase the cell activity and alkaline phosphatase activity of periodotal ligament fibroblasts and polyP may not affect the protein synthesis of the cells.

9.The One-Year Prognostic Value of the Pre-Discharge ECG after the First Acute Myocardial Infarction.

To assess the prognostic value of the 14 ECG variables obtained from the pre-discharge ECG, 35 patients were followed up for at least one year among the 80 patients of the acute myocardial infarction admitted at Seoul Paik Hospital from Sep. 1983 to Aug. 1986. The following results were obtained. 1) The overall in-hospital mortality rate was 20% and the mortality rate tended to decline year by year. 2) The one-year mortality and morbidity rate of the followed patients was 31.4%. 3) Among the 14 variables obtained from the pre-discharge ECG, T-negativity was the only statistically significant (p<0.05) one to predict the one-year prognosis in the patients after the first myocardial infarction, and we could not derive any meaningful datd from the analysis of the combined effects of the three ECG variables (PTF, ST depression and ST elevation).

10.Factors Associated with Smartphone Dependence of Late School-Aged Children: A Focus on Grit and Family Strengths

Background:
This study was to identify the factors influencing smartphone dependence among late schoolaged children, focusing on grit and family strengths.
Methods:
A cross-sectional descriptive study was conducted with 218 children in grades 5 and 6 who were recruited from elementary schools in U and Y cities. The data were collected through self-reported questionnaires, including smartphone dependence, grit, family strengths and general characteristics, for the month of December 2019, just before the novel coronavirus pandemic after obtaining approval from IRB. The collected data were analyzed using multiple regression.
Results:
Smartphone usage time, the main purpose of smartphone usage (game and social networking), and grit were identified as significant factors of smartphone dependence. This model explained 38% of the total variance showing the variable of grit with the greatest variance.
Conclusions
A strategy to develop grit is needed to reduce smartphone dependence among late school-age children.
